KEY POINT: This diagram shows how the micturition cycle correlates with activities of the detrusor, urethra, and pelvic floor. ADDITIONAL INFORMATION: During the normal process of storage of urine in the bladder, the detrusor muscle of the bladder is relaxed to support bladder filling whereas the bladder neck, urethral rhabdosphincter and pelvic floor muscles contract to prevent leakage of urine. During the normal voiding process, a decrease in the intraurethral pressure and subsequent relaxation of the urethral rhabdosphincter precedes the contraction of the detrusor muscle of the bladder. At the same time, the pelvic floor muscles relax and the bladder neck forms a funnel. Stimulation of the detrusor muscle causes it to contract and the flow of urine begins. The contraction of the detrusor muscle is maintained throughout the voiding process. Em condições apropriadas as inibições suprapontinas são suprimidas e centro pontino é a tivado induzindo a micção. O relaxamento do assoalho pévico é mediado por interneurônios GABA-nérgicos nos segmentos sacrais, sob controle do núcleo de Barrington. Há evidências sugerindo que o relaxamento do músculo liso uretral é mediado pela liberação de óxido nítrico.

ADDITIONAL INFORMATION: The alternating activity of the detrusor during voiding and the bladder neck, urethral sphincter and pelvic floor muscles during bladder filling is coordinated by the pontine micturition centre in the central nervous system which is under cerebral cortex control. During bladder filling the sympathetic fibres of the hypogastric nerve ensure relaxation of the bladder and closure of the bladder neck. The urethral rhabdosphincter remains closed due to somatic pudendal nerve activity. The somatic sacral nerve ensures that the pelvic floor muscles are also contracted and as such support the compression of the urethra and prevention of leakage. During voiding, the activity of the hypogastric, pudendal and sacral nerves is inhibited whereas the parasympathetic pelvic nerve is stimulated. This results in relaxation of the bladder neck, urethral rhabdosphincter and pelvic floor muscle and contraction of the detrusor and therefore bladder emptying.
Os achados clínicos tipicamente dependem da localização da lesão, entretanto, freqüentemente os pacientes com lesões neurológicas se apresentam com alterações complexas que resultam de lesões completas e incompletas nos vários níveis de controle do trato urinário inferior. Muitas vezes os pacientes são avaliados tardiamente e apresentam alterações resultantes de complicações como refluxo vesico-ureteral, infecções urinárias, insuficiência renal, etc. A obtenção da história pode ser dificultada por alterações do humor, afasia ou demência. Diários miccionais podem ser muito úteis caracterizando hábitos alimentares, freqüência, volume das micções, urgência, episódios de urgência, urge-incontinência, dor, etc. A função sexual deve ser avaliada. 95% dos pacientes com lesões supra-sacrais apresentam ereções reflexas , porém a disfunção erétil, quando presente, pode traduzir problemas vasculares não relacionados à lesão neurológica. A presença de obstipação pode comprometer a função ao do trato urinário , abolindo o reflexo miccional, resultando em dificuldade de esvaziamento vesical, especialmente em homens.
A avaliação urodinâmica engloba medidas de extrema simplicidade como o registro de diários miccionais até complexas avaliações monitorizadas por microcomputadores ligados a periféricos que convertem as variações de pressão em dados. O estudo pode ser acompanhado através de imagens obtidas por intensificadores de imagem e/ou ecografia, permitindo uma avaliação visual da pelve e trato urinário inferior.
Conceitualmente pode-se entender as conseqüências de lesões completas em vários níveis do sistema nervoso. A transecção medular suprasacral completa não só elimina qualquer inibição de centros suprapontinos mas também, resulta em perda do atividade pontina facilitatória comprometendo a sustentação da contração vesical. A função do esfíncter uretral estriado também é afetada levando a uma “dissinergia”entre a contração vesical e o relaxamento dessa estrutura. Lesões supra-pontinas resultam hiper-reflexia vesical uma vez que há uma abolição das influencias suprapontinas inibitoras. Como regra, não há dissinergia detrusor-esfincteriana. Lesões infrasacrais incluindo o cone medular afetam primariamente a inervação periférica ou o arco reflexo sacral. Podem ser motoras e ou sensitivas e, dependendo da extensão, resultam em uma bexiga acontrátil com uma função normal ou relaxamento das estruturas de continência (esfínceter denervado ou hipoativo’.
O controle do centro pontino da micção por centros mais superiores é extremamente complexo e lesões desse tipo freqüentemente causam hiper-reflexia com diminuição da capacidade vesical. OS infartos cerebelares por outro lado frequentemente se traduzem por arreflexia vesical.
Tolterodine withdrawal rates were similar to placebo. Tolterodine patients tolerated treatment much better than oxybutynin patients. The majority of the difference in withdrawal rate was due to dry mouth. Oxybutynin patients withdrew due to adverse events at over three times the rate of tolterodine patients. The primary metabolic pathway of oxybutynin chloride is the cytochrome P3A4 system, and the half-life of oxybutynin chloride is approximately 3 hours.24 The half-life of the XL formulation is 12 to 13 hours. The primary metabolite of oxybutynin is N-desethyl oxybutynin.23 Both parent and metabolite have antimuscarinic activity; however, the metabolite is thought to be the primary cause of adverse effects associated with oxybutynin ingestion. After oral ingestion of immediate-release oxybutynin, levels of metabolite increase to a level 6-fold higher than the parent compound. Newer delivery methods have attempted to decrease these high serum levels of metabolite. Oxybutynin chloride (immediate release) undergoes extensive first-pass metabolism in the proximal gut wall owing to resident cytochrome P3A4 in that location which degrades a percentage of the ingested oral dose. This metabolic step limits drug bioavailability and produces high levels of desethyloxybutynin. 25 Bioavailability with the XL formulation is higher, suggesting a reduced first-pass effect. This reduced first-pass effect occurs due to decreased absorption in the proximal small gut and increased absorption in the distal gut and large bowel.
A. Darifenacin is a selective muscarinic M3 receptor antagonist. B. DOSING INFORMATION : The recommended starting dose of extended-release darifenacin is 7.5 milligrams orally once daily. The dose may be increased, two weeks after starting therapy, to 15 milligrams orally once daily if needed. C. PHARMACOKINETICS : Following oral administration of controlled-release darifenacin, bioavailability is 15% to 25%, protein binding is 98%, and the volume of distribution is 163 L. Darifenacin is extensively metabolized in the liver by CYP3A4 and CYP2D6 and excreted in the urine (58%) and feces (44%). The elimination half-life of darifenacin is 13 to 19 hours. D. CAUTIONS : The most common adverse effects associated with darifenacin are dry mouth and constipation. Abdominal pain and dyspepsia have also been reported. The majority of adverse events were mild or moderate in severity and most occurred during the first 2 weeks of treatment. E. CLINICAL APPLICATIONS : Darifenacin is FDA-approved for the treatment of overactive bladder with symptoms of urge urinary incontinence, urgency and frequency. 1.4 PEDIATRIC DOSAGE 1.4.1 NORMAL DOSE A. ORAL 1. Safety and efficacy have not been established in pediatric patients (Prod Info Enablex(R) extended-release tablets, 2004). CONTRAINDICATIONS A. ASTHENIA occurred in 1.5% and 2.7% of patients on darifenacin 7.5 milligrams/day (mg/day) and 15 mg/day, respectively, compared with 1.3% of patients on placebo. DIZZINESS occurred in 0.9% and 2.1% of patients on darifenacin 7.5 mg/day and 15 mg/day, respectively, compared with 1.3% of patients on placebo (Prod Info Enablex(R) extended-release tablets, 2004). In one study, the incidence of HEADACHE was 0.9% at a darifenacin dose of 7.5 mg/day and 3.5% at a dose of 15 mg/day compared with placebo 2.4% (Haab et al, 2004). 3.3.5 GASTROINTESTINAL A. GASTROINTESTINAL EFFECTS 1. The most common side effects were DRY MOUTH and CONSTIPATION, which occur during the first 2 weeks of treatment and were, in general, mild to moderate severity. DRY MOUTH occurred in 20% and 35% of patients on darifenacin 7.5 milligrams/day (mg/day) and 15 mg/day, respectively, compared with 8% of patients on placebo. CONSTIPATION occurred in 15% and 21% of patients on darifenacin 7.5 mg/day and 15 mg/day, respectively, compared with 6% of patients on placebo. The discontinuation rate was less than 1.2% for patients who experienced dry mouth and constipation while on darifenacin (Prod Info Enablex(R) extended-release tablets, 2004). 2. ABDOMINAL PAIN, NAUSEA, and DIARRHEA occurred in 2% to 4% of patients on darifenacin (Prod Info Enablex(R) extended-release tablets, 2004). In one study, the incidence of DYSPEPSIA was 1.7% at a darifenacin dose of 7.5 mg/day and 7.8% at a dose of 15 mg/day compared with placebo 2.4% (Haab et al, 2004). 3.3.8 OCULAR A. DRY EYES 1. Dry eyes occurred in 1.5% and 2.1% of patients on darifenacin 7.5 milligrams/day (mg/day) and 15 mg/day, respectively, compared with 0.5% of patients on placebo (Prod Info Enablex(R) extended-release tablets, 2004). 3.3.12 OTHER A. Tratamento medicamentoso intravesical: é realizado através de instilação intravesical de substâncias. As mais usadas na atualidade são: oxibutinina, capsaicina e resiniferotoxina. A instilação vesical de oxibutinina deve ser feita no mesma posologia oral. Tem a mesma eficácia do adimnistração oral porém menos efeitos colaterais. Pelo incoveniente do uso geralmente só é usada em pacientes neurológico que já fazem cateterismo vesical para esvaziar a bexiga. A instilação vesical mais efetiva usada atualmente é com a capsaicina e a resiniferotoxina. A capsaicina é extraída das pimentas vermelhas e a resinferotoxina de uma espécie de cactus africano. A aplicação intravesical é única por 30 minutos para as duas drogas no volume correspondente a metade da capacidade cistométrica máxima (capsaicina 1-2 mM e resiferotoxina 50-100 nM). A capsaícina produz uma piora inicial dos sintomas nos primeiros dias seguido de melhora progressiva que dura de 6-9 meses e pode chegar a 65% de sucesso terapêutico. A resiniferotoxina produz melhora progressiva desde o primeiro dia do tratamento com efeito que dura cerca de 12 meses e pode chegar a 85% de sucesso terapêutico.
neste tratamentos são usadas aplicações de toxina botulínica no músculo detrusor por cistoscopia. A toxina botulínica age nas placas neuromusculares bloqueando a liberação de acetilcolina impedindo a contração muscular. A dose a se usada em bexiga hiperativa é de 200-300 UI que devem ser divididas em 20-30 pontos de aplicação na parte supratrigonal da bexiga. O efeito dura de 3-6 meses e depois devem ser feitas re-aplicações após este período. Os resultados terapêuticos variam entre 50-70%
