[SUMMARY]
This clinical trial tests whether providing early parenteral nutrition (PN) in addition to enteral nutrition (EN) improves outcomes for critically ill patients compared to EN alone. Patients are randomly assigned to receive either EN only, with PN started if EN is insufficient after 7 days, or a combination of EN started on day 2 and PN started on day 3, with PN amount based on calories not provided by EN. The study aims to determine if early PN completion of EN leads to better patient outcomes.

CALCULATION
Energy needs: 2000 kcal/d. Fluid needs: 2400 mL/d.
Protein needs: 120 g/d. TPN: Protein contains 4 kcal/g, so 120 g protein = 480
kcal.2000 total kcal - 480 protein kcal = 1520 kcal (to provide as nonprotein kcal).
Lipid: 1520 kcal × 0.25 = 380 kcal. Provided as 20% Intralipid: has 2 kcal/mL (380
kcal ÷ 2 kcal/mL) = 190 mL Intralipid.
Dextrose: 1520 kcal × 0.75 = 1140 kcal. Dextrose has 3.4 kcal/g (1140 kcal ÷ 3.4
kcal/g) = 335 g dextrose. Volume (using 10% amino acid, and 70% dextrose
solutions).
Volumes:
Protein: 120 g ÷ 0.1 = 1200 mL.
Dextrose: 335 g ÷ 0.5 = 670 mL.
Lipid: 190 mL.
Electrolytes and other additives: approximately 100 mL.
Total volume = 2160 mL.

35. IMPACT OF EARLY PARENTERAL NUTRITION COMPLETING ENTERAL NUTRITION IN ADULT CRITICALLY ILL

Arms Assigned Interventions EN only: Experimental Other: Withholding PN during the
first week of ICU stay Patients in this arm will receive exclusively enteral nutrition. If
enteral nutrition is insufficient after the seventh day of ICU stay, parenteral nutrition
will be started. EN plus early PN: Active Comparator Drug: Oliclinomel N71000 OR
N71000E // Clinimix N17G35 OR N17G35E PN will be started the morning of the third
ICU hospitalisation day. The amount of PN to be given will be calculated to
cover the caloric needs of the patient, based on the enteral energy intake the
previous 24 hours.
Detailed Description:
As initial nutritional support, patients randomised to the 'EN combined with early PN'
group will receive glucose 20% at 40 ml/hr. EN will be initiated in the evening of the
second ICU hospitalisation day, PN will be started the morning of the third ICU
hospitalisation day. The amount of PN to be given on any particular day will be the
difference between calculated caloric needs and the calories delivered by EN the
previous 24 hours. When EN covers 80% of calculated caloric needs PN will be stopped.
When the patient is able to eat, the parenteral regimen will be reduced and eventually
stopped. Whenever oral (+ enteral) intake is below 50% of calculated caloric needs, the
PN will be (re)-started.
Trace elements, minerals and vitamins will be administered daily intravenously (IV) to
all patients from the day of admission onwards. IV substitution will be stopped in
patients receiving at least 1500 ml of EN. All patients will be treated following the
intensive insulin therapy schedule - targeting a blood glucose level of 80 - 110 mg/dl -
from admission until discharge or oral feeding.
