5. Estadiamento e escolha terapêutica no

O estadiamento do CHC deve se basear em:
• Variáveis prognósticas de estudos sobre história natural de CHC e
cirrose
• Variáveis de estudos baseados em evidências sobre a terapêutica
Recomendações
Sistemas de estadiamento para decisões clinicas no CHC devem incluir:
• Carga Tumoral
• Função Hepática
• Performance status
Alto Forte
Sistema de estadiamento BCLC tem sido repetidamente validado e é
recomendado para predição prognóstica e escolha do tratamento
O Conceito de ‘migração de estágio’ de tratamento deve ser aplicado
Alto Forte
Pacientes devem ser discutsdos em times multidisciplinares para
individualizar opções de tratamento

6. TNM para Estadiamento HCC/CHC
• Em oncologia, a classificação padrão do câncer = TNM.
• Limitações para HCC:
- Biópsia necessárias para avaliar a invasão microvascular,
(disponível apenas em tratados por cirurgia (20%)).
- Não informa sobre o status funcional hepático ou
performance status
- Valor prognóstico em tumores não precoces é limitado.

Invasão direta da veia porta 29-65% e veias hepáticas 12-54% dos casos,
• Tumor pode se estender das veias hepáticas para a VCI e, em seguida, para o AD, e às
vezes até para o VD
• Tumor no coração pode embolizar e resultar em infartos pulmonares.
TROMBO TUMORAL NA
RM com contraste:
trombos normalmente têm baixa
intensidade de sinal T2 e trombos
tumorais, têm intensidade de sinal
intermediária nas imagens de RM
ponderadas em T2.

Diagnóstico não pode se basear no “washout” e sim
na biópsia do nódulo hepático
BCLC não se aplica (nem CHILD-PUGH)
Buscar a invasão vascular e o “alem fígado” de forma
similar aos pacientes cirróticos.
De forma geral a resseccção é mais indicada e
respeita o performance status do paciente e a
possibilidade de ressecção de segmentos
(indocianina green).

Ao estadiar um cirrótico com HCC/CHC
Primeiro observar o performance status e CHILD-PUGH
Observar em seguida tamanho e número de nódulos
Buscar doença invasiva dentro do fígado e além fígado
