1. O MAIOR GRUPO DE CRIANÇAS COM BAIXA ESTATURA LINEAR
INCLUINDO INFANTIL, JUVENIL E MESMO ADOLESCENTE APRESENTA
CAUSA DESCONHECIDA E SITUA-SE NUMA FAIXA CONSIDERADA COM
VARIANTE DO CRESCIMENTO NORMAL, ESTÃO ABAIXO DO 3º PERCENTIL
DE ALTURA E APRESENTAM BAIXA VELOCIDADE DE CRESCIMENTO,
GERALMENTE COM RETARDO NA MATURAÇÃO DO ESQUELETO, E
CRESCIMENTO MENOR QUE O ESPERADO NA FASE PUBERAL.
Essas crianças, infantis, juvenis e adolescentes não apresentam doenças
crônicas ou qualquer endocrinopatia, têm secreção normal de GH e os
familiares podem ou não apresentar história com essas características
clínicas, mas apresentam baixa estatura linear (altura). Entre as mais
frequentes destacaremos algumas que devem ser observadas com muito
cuidado.
2. RETARDO CONSTITUCIONAL DE CRESCIMENTO E DA PUBERDADE:
O Retardo constitucional de crescimento e da puberdade (RCCP) ocorre
em crianças com baixa estatura na infância, retardo puberal, idade óssea
atrasada e crescimento puberal tardio que atingem altura dentro da
faixa normal na idade adulta. Embora os distúrbios mais comuns da
puberdade atrasada sejam idiopáticos, as preocupações levantadas por
atraso da puberdade são muitas e variavelmente por pessoas diferentes.
Estes incluem possibilidade de patologia subjacente, o medo de que a
puberdade nunca ocorrerá e a perturbação emocional e psicossocial de
imaturidade,
especialmente a
da baixa
estatura
associada. Além
disso, as
preocupações
com sequelas de
longo prazo que
compreendem a
redução da
altura adulta
final (Ht)- altura
terminal,
mineralização
óssea e fertilidade também podem ser levantadas. O padrão de herança

3. A idade de início da puberdade é adiada por uma média de 2,5 anos nas
meninas e 3 anos nos meninos. O déficit de Ht altura terminal no início
da puberdade se correlaciona com a Ht altura terminal final. O tempo
entre o início da puberdade e o surto de crescimento puberal é menor
do que em crianças, infantis, juvenis e adolescentes normais e o pico da
velocidade de crescimento é atenuado. Portanto, a média final Ht é
geralmente mais curta do que ambos os alvo Ht altura terminal e Ht
altura terminal adulta prevista. O Ht altura terminal final fica, em média,
1,85 DP abaixo da média de adultos saudáveis, com grandes variações
individuais. A melhor Ht altura terminal final, parece ocorrer em
crianças, infantis, juvenis e
adolescentes que tiveram
seu crescimento lento
tardio durante a sua
infância, enquanto que
aqueles com redução
precoce e progressivo da
relação Ht altura terminal
final na primeira infância
têm comprometida a Ht
altura terminal final. Além
disso, o atraso
constitucional do
crescimento e puberdade
(ACCP) é combinada com a
baixa estatura familiar em torno de 40% das crianças baixas com Ht
altura terminal final <3 º percentil para idade e sexo. A secreção de GH
pode estar transitoriamente diminuída na época do crescimento puberal
atrasado, por provável produção diminuída do esteroide gonadal.
BAIXA ESTATURA GENÉTICA OU FAMILIAR:
4. A altura e o padrão de crescimento dos familiares determina o
crescimento do indivíduo, de tal forma que uma criança crescendo com
padrão diferente dos irmãos e dos pais deve ser avaliada clinicamente.
Transmissão genética de doenças orgânicas pode levar à baixa estatura
linear (altura). A insensibilidade ao GH decorrente de mutação do gene
receptor de GH, deleções do gene de GH, mutações do gene PROP-1,
pseudo-hipoparatireoidismo, diabetes mellitus, algumas formas de
hipotireoidismo, osteocondrodisplasia, erros inatos do metabolismo,
doença renal e talassemia constituem em anomalias hereditárias que
podem induzir baixa estatura (altura) linear. Baixa estatura genética ou
familiar constitui-se em um grupo diverso do descrito, tendo como
característica clínica o crescimento na infância abaixo do 5º percentil,
mas com velocidade normal.
5. A puberdade ocorre normalmente, ou ligeiramente precoce, e mais
rápida que o normal, com idades óssea e cronológica concordes, mas
que levara a uma baixa estatura linear. Os pais são baixos e a altura final
dos pacientes é baixa, situada na faixa da família, não havendo
anormalidade do eixo GH-IGF, mas deve-se levar em consideração que a
correção da estatura para os pais, eventualmente, devido à terapia
Padrão Ouro ser relativamente recente ao alcance de todos, pode ter
ocorrido injunções que não permitiram os pais terem o padrão alvo ideal
ou de acordo com ancestrais até poucas gerações atrás recentes
cronologicamente, deve ser pelo menos uma percepção subjetiva e não
pensar de forma cartesiana.

adult height Ht end. The age of onset of puberty is delayed by an
average of 2.5 years for girls and 3 years for boys. Ht terminal deficit at
the beginning of puberty height correlates with height Ht terminal end.
7. The time between the onset of puberty and pubertal growth spurt is
smaller than in normal juvenile and adolescent children's children and
the growth rate peak is attenuated. Therefore, the final average Ht is
usually shorter than both target Ht terminal height and predicted adult
height terminal.
The height Ht terminal end is on average 1.85 SD below the mean for
healthy adults, large individual variations. Best Ht terminal end point,

organic diseases may lead to short stature (height). Insensitivity to GH -
growth hormone caused by mutation of the GH receptor gene - growth

hypoparathyroidism, diabetes mellitus, some forms of hypothyroidism,
Osteochondrodysplasia, inborn errors of metabolism, renal disease and
thalassemia are in hereditary abnormalities that can induce short stature
(height) linear. Low genetic or familial stature is in a different group
from that described, with the clinical characteristics growth in childhood
below the 5th percentile but with normal speed. Puberty normally
occurs, or slightly earlier, and faster than normal, with bone and
chronological ages in agreement, but that led to a linear low stature.
Parents are low and the final height of patients is low, situated on Family
Band, with no abnormality of the GH-IGF axis, but one should take into
account that the correction of stature for parents, possibly due to gold
standard therapy be relatively new to everyone, injunctions may have
occurred that did not allow parents to have the ideal target or according
to ancestral little newer generations chronologically default, must be at
