No momento do nascimento, com a interrupção do fornecimento de
glicose via cordão umbilical, o recém-nascido é exigido no sentido de
continuar a suprir as necessidades dos seus diversos órgãos da economia
orgânica e, muito
particularmente, do
cérebro. É
extremamente delicado
o equilíbrio entre
produção e utilização de
glicose no recém-
nascido e na criança
pequena, e este fato
poderá perdurar em
todas as fases por que passam as crianças, infantil, juvenil, adolescente e
se estender por toda a vida, por uma situação que parece inocente, mas
que comprometerão de forma grave os indivíduos: a produção hepática
de glicose é duas a três vezes a do adulto, ao mesmo tempo em que o
cérebro consome até 6 vezes mais glicose, sempre em relação ao peso
corpóreo. Além disso, nessa faixa etária a hipoglicemia apresenta-se mais
com irritabilidade, dificuldades alimentares, letargia, cianose, taquipnéia
2. e/ou hipotermia que com os
sintomas adrenérgicos e
neuroglicopênicos típicos vistos
em adultos, que dificulta o
reconhecimento do quadro e
expõe o sistema nervoso central
(SNC) a lesões que podem levar
a convulsões ou retardo mental.
No entanto, não existem dúvidas
quanto ao comprometimento
das ações hormonais que podem
mudar de acordo com o estádio
de desenvolvimento. Por
exemplo, o hormônio
tireoideano, tireóide, glândula
mestra que distribuirá uma
quantidade de hormônios para todas as células, sem esta ação a logística
não ocorrerá com eficiência e acabará comprometendo diversos setores
chaves do organismo humano durante toda a sequência. Para se ter uma
leve percepção podemos perceber que o mínimo descolamento de setores
de placenta de forma quase imperceptível, ou mesmo micro-infartos
placentários possivelmente se constatará no hipotireoidismo subclínico da
mãe ou doença auto-imune ou síndrome de Hashimoto, que propiciará ao
feto o quadro clínico de um intraútero e pós- natal característico de P.I.G.
(pequeno para idade gestacional), e um desses detalhes promoverá déficit
pondo- estatural em sua evolução por todas as fases, principalmente a
baixa estatura que se não tratada precocemente será perene;
o hormônio de crescimento-GH tem pouco efeito intraútero, pois o fator
primário para o crescimento é o IGF-2, e diversos pesquisadores já se
convenceram de outras ações do IGF-2 no sistema nervoso central, que
em caso de comprometimento neo-natal terá efeitos pós-natal
sequencialmente nas diversas fases de desenvolvimento e mais tarde
assumem papel relevante. As induções de enzimas específicas, tão
3. necessárias à adaptação do recém-nascido à “parada de infusão contínua
de glicose” promovida pelo cordão umbilical são dependentes das ações
integradas de cortisol, glucagon, hormônio tireoideano e insulina, este
fato irá comprometer uma situação vital no desenvolvimento corporal por
abrangerem todo o eixo GH, IGFs e IGFBPs, e como consequência além da
defasagem cronológica e mental também alcançará o metabolismo de
crescimento, levando a baixa estatura com deficiência do crescimento
linear e deficiência de um dos mais importantes hormônios para o
crescimento linear, GH, HGH OU SOMATOTROFINA. No feto, a insulina é
um hormônio crítico para o crescimento e ganho de peso, enquanto no
neonato passa a ter um profundo efeito no metabolismo de hidratos de
carbono. Tanto no feto quanto no recém-nascido e obviamente provocará
uma deficiência grave em criança, infantil, juvenil e adolescente
culminando com a fase adulta, mas a insulina tem efeitos sobre o
metabolismo de fosfolípides e sobre as vias neoglicogênicas que faz parte
desse “Mecanismo em Totum”. Nessa condição quanto mais precoce se
corrigir tais sequelas, menos características estigmáticas ficarão de formas
residuais.
