CRESCIMENTO DESPROPORCIONAL LEVANDO A UMA FALTA DE
UNIFORMIDADE NO DESENVOLVIMENTO DE CRIANÇA-INFANTIL-
JUVENIL.
Baixa estatura: crianças e adolescentes cujas alturas e velocidades de
crescimento desviam dos percentis normais em tabelas de crescimento
padrão apresentam um desafio especial para os médicos. A altura que é
menor que o percentil 3 é considerada baixa estatura e superior ao
percentil 97 é considerada de estatura alta. A velocidade de crescimento
fora da faixa do percentil 25 a 75 pode ser considerada anormal.
Medidas de altura de série ao longo do tempo documentadas em um
gráfico de crescimento são fundamentais na identificação de
crescimento anormal. A baixa ou alta estatura geralmente é causada por
variantes de um padrão de crescimento normal, embora alguns
pacientes possam ter graves patologias subjacentes. A história completa
e o exame físico podem ajudar a diferenciar os padrões de crescimento
anormais de variantes normais e identificar características dismórficas
específicas de síndromes genéticas. Resultados de história clínica e
exame físico devem orientar os testes laboratoriais. Médicos de
cuidados primários desempenham um papel importante na identificação
de crianças com crescimento anormal. Na maioria dos casos, a baixa
estatura ou alta é causada por variantes de um padrão de crescimento
normal, no entanto, uma grave patologia subjacente está presente em
alguns pacientes. A história clínica completa e o exame físico devem ser
realizados em todas as crianças com crescimento anormal e estudos
laboratoriais devem ser com base nestes resultados. Ainda devemos
2. focar em alguns detalhes subjacentes tais como: os seguintes padrões
devem ser parte da avaliação das baixas estaturas:
(1) Circunferência occiptofrontal da cabeça- este fato é de suma
importância, pois diversos pesquisadores têm observado p. ex.,
vários relatos indicam que o transtorno do espectro do autismo (TEA)
está associado com um aumento da taxa de crescimento da cabeça na
primeira infância. O aumento da taxa de processos aberrantes do índice
de crescimento pode durante o desenvolvimento inicial, preceder o
início dos sintomas, e pode prever a gravidade do curso da doença.
Examinamos taxa de mudança na medida da circunferência
occiptofrontal (occiptofrontal: refere-se da região occipital aos ossos
frontais e outras estruturas compartilhadas). No início da vida, medidas
de circunferência occiptofrontal correlacionam com volume de
substância cinzenta observados em investigações de ressonância
magnética em 2 a 3 anos de idade e pode ser um bom indicador precoce
para o volume do cérebro. Em idades mais avançadas, a circunferência
3. occiptofrontal não é mais tão fortemente correlacionada com o volume
do cérebro;
(2) Parte inferior do corpo: mede-se a distância do topo da sínfise púbica
(é uma articulação semimóvel que une o púbis formando a bacia-cintura
pélvica, que é composta pelo sacro e cóccix, e dois ossos do quadril, que
por sua vez, cada osso, é composto por três ossos compactados: ílio,
ísquio e o osso púbico), da sínfise púbica até o chão;
(3) Parte superior do corpo: mede-se a altura na posição sentada (a
altura do assento deve ser subtraída da altura na posição ortostática-em
pé;
(4) Envergadura: existem médias publicadas para essas medidas de
proporções
corporais que
devem ser avaliadas
de acordo com a
idade do paciente.
A relação entre a
parte superior e a
parte inferior do
corpo, p. ex., varia
de 1.7 no recém-
nascido a pouco
menos de 1.0 no
adulto. De forma
objetiva devemos levar em consideração desde as primeiras fases das
crianças a preocupação de uma propedêutica rigorosa completa, seja
clínica, instrumental ou laboratorial.

Short stature: children and adolescents whose heights and growth
velocities divert normal percentiles on standard growth charts present a
special challenge for physicians. Height that is less than the 3rd
percentile or above the 97th percentile is considered a low or high
height, respectively. The growth rate outside the range of 25 to 75
percentile may be considered abnormal. Serial measurements of height
over time in a graphical documented growth are critical in identifying
abnormal growth. Short stature or high is usually caused by variants of a
normal growth pattern, although some patients may have severe
underlying diseases. A complete clinical history and physical
examination can help differentiate abnormal growth patterns of normal
variants and identify specific dysmorphic features of genetic syndromes.
Results clinical history and physical examination should guide laboratory
testing. Primary care physicians play an important role in identifying
children with abnormal growth. In most cases, short stature or height
are variations caused by a normal growth pattern; however, serious
underlying pathology is present in some patients.
5. A complete clinical history and physical examination should be
performed in all children with abnormal growth, and laboratory studies
should be based on these results. Although we focus on some underlying
details such as; the following standards should be part of the evaluation
of short stature:
(1) The Occiptofrontal Head Circumference: this fact is of paramount
importance, as several researchers
have noted, e.g.; several reports
indicate that autism spectrum
disorder (ASD) is associated with
increased rate of head growth in
early childhood. The increase in
growth rate of abnormal processes
during early development, may
precede onset of symptoms, and can
predict the severity of the disease
course. We examined the rate of
change in the measure of the
occiptofrontal circumference
(occiptofrontal referring to the
frontal and occipital bones and other
shared structures). At the beginning
of life measures occiptofrontal
circumference correlated to gray matter volume seen in MRI
investigations in 2 to 3 years of age and can be a good indicator for the
early brain volume. At older ages, occiptofrontal circumference is not as
strongly correlated with brain volume;
(2) Lower body: distance from the top of the pubis (pubic symphysis is a
semi-mobile joint that connects the pubis forming the basin (pelvis),
which is composed of the sacrum and coccyx, and two hip bones, which
6. in turn each bone consists of three compressed bones: ilium, ischium,
and the pubic bone), the pubic symphysis to the ground;
(3) Upper body: sitting height (the height of the seat position should be
subtracted from the height in the standing position;
(4) There wingspan averages reported for these measures of body
proportions that must be evaluated according to age of patient the
relationship between the upper and lower body, e.g., varies from 1.7 in
the newly-born a little less than 1 in adult objectively we must take into
account from the earliest stages of children the concern of a complete
rigorous workup, whether clinical, instrumental and laboratory.
