nascidos da união com o deus. Em seguida, jogou-se no rio Sangário.
• Foi transformada por Zeus em uma fonte.
• Αὔρα AURA
• Aura = "brisa“, “vento suave”.Termo usado para descrever a “atmosfera”
associada a alguma coisa.
impressionantes.

9. 1.4.2 Persistent aura without infarction
• A. Aura cumprindo o critério B

10. 1.4.2 Persistent aura without infarction
• Comments:
• Persistent aura symptoms are rare but well documented.They are often bilateral
and may last for months or years.The one-week minimum in criterion B is based on
the opinion of experts and should be formally studied.
• Diagnostic work-up must distinguish: 1.4.2 Persistent aura without infarction from:
• 1.4.3 Migrainous infarction,
• and exclude symptomatic aura due to cerebral infarction of other causes.
• Attacks with prolonged aura lasting less than one week and not fulfilling criteria
for 1.2.1 Migraine with typical aura are coded:
• 1.5.2 Probable migraine with aura.

13. Persistent Migraine Aura: New Cases, a Literature
Review, and Ideas About Pathophysiology
Thissen S, et al. Curr Pain Headache Rep (2015) 19: 23
• Luda et1 al were the first to report PMA in, a 65-year-old patient with a history of
migraine.
• Lance and Goadsby2 distinguished two types of PMA.
• PMA with typical aura (PMA-TA): a persistent typical migraine aura with
oscillation, scotoma, and fortification in one hemifield.
• Persistent primary visual disturbance (PMA-PPVD). visual snow or “television
static” in the whole visual field of both eyes, in addition to intermittent scotoma or
oscillating lights
• The precise significance of this distinction is unknown.
• Besides a significant difference in duration of symptoms, we have been unable to
find any differences between these groups in the present review.

2. Mechanism and Management of Headache, 7th edition. Oxford: Butterworth- Heinemann; 2004).14
14. PMA-PPVD case
• 66-year-old man without a history of migraine
• Seeing white dots in front of both eyes since for the last 3 years.
• “snow” in both visual fields as in “television static.”Another comparison: “rain on a
window.”
• At firs,t these symptoms occurred intermittently until they persisted.
• Laboratory work-up:
• Paraneoplastic antibodies and lumbar puncture, visual evoked potentials (VEP),
Transcranial and extracranial Dopplers were normal, as well as FDG-PET.Cerebral
computed tomography (CT; magnetic resonance imaging was contraindicated) did not
show occipital abnormalities
• Treatment: DVP and LMT: no effect
16
15. Literature Review
• n= 54 possible cases, 22 articles, published between 1991 and 2014.
• Seven were excluded → 47 cases.
• The mean age of onset was 30 (7 to 74) years.
• PMA: 75% in women
• The duration of symptoms: from 9 days to 28 years (median 365 days).
• PMA forms (47 elegible cases) :
• 19 patients with PMA-TA
• 27 with PMA-PPVD
• 1 No enough data.

17. Visual auras: Positive visual phenomena
• colored dots;
• scintillating scotomas;
• disturbed, blurry vision;
• geometrical figures,
• circles or rims (círculos ou aros);
• flickering flashing lights;
• blobs of white and gray (bolhas de
branco e cinza);
• black cracks of lines (linhas negras
quebradas);
• television snow (chuvisco);
• rain-like patterns;
• micropsia;
• palinopsia;
• Squiggles (rabiscos);
• zigzag patterns;
• Blind spots.

18. Structural and Functional Investigations.
• Structural imaging techniques and neurophysiological investigations: exclude underlying
pathology.
• Functional investigations: get more insight in the underlying pathophysiology of PMA.
• Techniques that have been applied: MRI,Tc99m-HMPAO-SPECT, FDG-PET, MR
angiography, MR perfusion,CT, EEG,VEP, ERG, EOG, Doppler, transcranial Doppler, and
cerebrospinal fluid analysis.
• It was not always clear what these investigations were based on.
• Tc99m-HMPAO-SPECT (11 cases) and FDG-PET (4 cases) scanning
• Liu et al: the inconsistent localization of the SPECT abnormalities and the subjectivity of
SPECT interpretations, did not allow them to draw any definite conclusions in their cases
Thissen et al. Curr Pain Headache Rep (2015) 19: 23

20. Crítica à ICHD-III Migrânea com aura
persistente
24
21. Pathophysiology
• The specific pathophysiology of PMA without infarction is still unknown,
• Theories on the underlying pathophysiological mechanism:
• Abnormal energy metabolism in the brain,
• Brain magnesium levels being significantly lower than that of nonmigraineurs,
• Greater reactivity of N-methyl-D-aspartate receptors to glutamate,
• A lowered threshold to CSD,
• Loss of inhibitory GABA-ergic interneurons;
• Other authors oppose a sustained hyperexcitability of the visual cortex without
significant dynamic modulation.
27
22. PMA:TREATMENT
• The treatment is purely based on case reports and
expert opinion.
• There are no double-blind, placebo-controlled studies.
• Only a few drugs have been described to be effective:
• Acetylsalicylic acid,
• Baclofen, Divalproex sodium,
• Frosemide,11,20,25 (RoldãoAlmeida)
• Lamotrigine,12,17,25
• Nifedipine,

25. Proposed criteria for the visual snow syndrome, modified from Schankin et al.
Patients compare visual snow often with TV static or TV snow.
_______________________________________________________________
A. Visual snow: dynamic, continuous, tiny dots in the entire visual field lasting longer than 3 months.
B. Presence of at least two additional visual symptoms of the four following categories:
i. Palinopsia (persistence of a visual image after the removal of the exciting stimulus
ii. Enhanced entoptic phenomen (arise from the structures of the visual system).
At least one of the following:
1-excessive blue field entoptic phenomenon (uncountable little grey/white/black dots or rings shooting
over visual field in both eyes when looking at homogeneous bright surfaces, such as the blue sky),
2.excessive floaters in both eyes,
3.self-light of the eye (coloured waves or clouds when closing the eyes in the dark),
4. spontaneous photopsia.
iii. Photophobia.
iv. Nyctalopia (impaired night vision).
C. Symptoms are not consistent with typical migraine visual aura.
D. Symptoms are not better explained by another disorder.

26. Compreendendo melhor...
• a-flutuadores (agregações proteicas no corpo vítreo,
