Dozens of U.S. states are currently suing Meta for contributing to the youth mental health crisis by inserting addictive features into their products, while the U.S. Surgeon General is touring the nation to bring awareness to the growing epidemic of loneliness and isolation. The country has endured periods of low national morale, such as in the 1970s when high inflation and the energy crisis worsened public sentiment following the Vietnam War. The current mood, however, feels different. Gallup recently reported that national mental health is at an all-time low, with few bright spots to lift spirits.
To better understand how Americans are feeling and their attitudes towards mental health in general, ThinkNow conducted a nationally representative quantitative survey of 1,500 respondents and found some interesting differences among ethnic, age and gender groups.
Technology
For example, 52% agree that technology and social media have a negative impact on mental health, but when broken out by race, 61% of Whites felt technology had a negative effect, and only 48% of Hispanics thought it did.
While technology has helped us keep in touch with friends and family in faraway places, it appears to have degraded our ability to connect in person. Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).

Introdução
A maioria dos testes laboratoriais empregados no estudo das hepatopatias refletem
somente determinados aspectos de comprometimento do órgão. Os testes bioquímicos
mais utilizados para o estudo do fígado são as bilirrubinas, as proteínas séricas e
algumas enzimas. Esses testes podem ser usados como triagem para detectar ou
excluir alterações hepáticas, bem como ajuda no diagnóstico das hepatopatias ou no
acompanhamento da evolução das doenças hepáticas. Por vezes esses testes são
chamados, incorretamente, de testes de função hepática, mas na verdade eles não
avaliam efetivamente o estado global da função hepática, com exceção da albumina,
do tempo de protombina e da pseudocolinesterase que podem indicar diminuição na
capacidade de síntese pelo fígado. Provas para a avaliação da função do fígado,
baseadas no clareamento plasmático de substâncias exógenas, pelo fígado, tem sido
utilizadas em trabalhos de investigação e também na prática clínica para a escolha do
tempo certo para indicação de transplantes de fígado e para prever a sobrevida em
casos de doença hepatica.

Avaliação do perfil hepático
Os testes de função hepática são exames de sangue que representam uma maneira não
invasiva de detectar a presença de doença hepática (por exemplo, hepatite em sangue
doado) e medir a gravidade e progressão da mesma, bem como sua resposta ao
tratamento.
Exames laboratoriais são geralmente eficazes para o seguinte:
✓ Detectar inflamação, lesão ou disfunção do fígado;
✓ Avaliar a gravidade da lesão do fígado;
✓ Monitorar a evolução de uma doença do fígado e a resposta da pessoa ao
tratamento;
✓ Especificar o diagnóstico
Os testes de função hepática são realizados em amostras de sangue e medem os níveis
de enzimas e de outras substâncias produzidas pelo fígado. Essas substâncias incluem
✓ Alanina aminotransferase (ALT);
✓ Albumina;
✓ Fosfatase alcalina (FAL);
✓ Alfafetoproteína (AFP);
✓ Aspartato aminotransferase (AST);
✓ Bilirrubina;
✓ Gama-glutamil transpeptidase (GGT);
✓ Desidrogenase láctica (LDH)
✓ 5'-nucleotidase
Os níveis de algumas dessas substâncias medem a presença e o grau da inflamação
hepática (por exemplo, ALT e AST). Os níveis de outras substâncias medem a
eficiência com a qual o fígado executa as suas funções normais de síntese de proteínas
e secreção de bile (por exemplo, albumina e bilirrubina). O que constitui um valor
normal para muitos desses testes pode ser encontrado na tabela Exames de sangue. No
entanto, alguns desses valores podem estar acima do normal em pessoas com
distúrbios não relacionados ao fígado.

Um teste de função hepática é o tempo de protrombina (TP), usado para calcular a
razão normalizada internacional (RNI). Tanto o TP quando a RNI são medições do
tempo necessário para o sangue coagular (o fígado sintetiza algumas proteínas
necessárias para a coagulação sanguínea, denominadas fatores de coagulação
sanguínea). Um resultado anormal de TP ou RNI pode indicar uma doença aguda ou
crônica do fígado. Tanto na doença aguda quanto crônica do fígado, a elevação do TP
ou RNI geralmente indica progressão para insuficiência hepática.
Enzimas hepáticas elevadas podem refletir dano hepático ou fluxo biliar prejudicado.
Pode ocorrer em um paciente com sinais ou sintomas consistentes com doença
hepática, ou pode ocorrer isoladamente, como um achado inesperado, durante um
estudo laboratorial de rotina.
As doenças hepáticas muitas vezes resultam em enzimas elevadas, com níveis que
excedem os valores normais em dois desvios padrão, pois sua diminuição, com
exceção da albumina, não tem significado clinic.
O figado
É o principal órgão no metabolismo de carboidratos, gorduras e proteínas. Além
disso, sintetiza e secreta bile, lipoproteínas e proteínas plasmáticas, incluindo fatores
de coagulação.
Com exceção da maioria dos lipídios, os produtos que são absorvidos dos alimentos
passam diretamente do intestino para o fígado através do sistema porta.
Microscopicamente, a unidade funcional primária do fígado é o ácino, um território
suprido por cada ramo terminal da artéria hepática e veia porta.
O ácino hepático é dividido em 3 zonas de acordo com o suprimento sanguíneo; a
zona 3 é a de menor perfusão e a que abriga o maior número de mitocôndrias.

Testes de função hepática
É aplicado a uma variedade de exames de sangue para descobrir a condição geral
do fígado e do sistema biliar.
Além dos exames hepáticos usuais obtidos por meio de painéis de química
sorológica automatizados de rotina, podem ser necessários exames hepáticos
mais específicos, como testes de sorologia viral ou testes autoimunes que,
quando positivos, podem determinar a causa específica da doença hepática.
Os testes hepáticos podem ser divididos em dois tipos:
1º) Aqueles que dão valores reais da função hepática, como albumina sérica ou tempo
de protrombina;
2º) Aqueles marcadores de doença hepática ou do sistema biliar, como diferentes
enzimas hepáticas,
Por fim, existem exames de sangue específicos que permitem um diagnóstico
preciso para as hepatites A, B, C e D.
Principais enzimas hepáticas
Alanina aminotransferase (ALT): É uma enzima de extravasamento que está livre no
citoplasma dos hepatócitos, esta enzima é essencialmente hepato-específica para cães
e gatos. O aumento da atividade sérica dessa enzima indica uma
lesão celular liberando-a para a circulação. Pequenos aumentos da sua atividade não
têm relevância, pois o grau da lesão hepática é responsável pelo aumento da atividade
da ALT. Há lesões que permitem a passagem da enzima através da membrana sem
perda de função celular; por exemplo, hipóxia decorrente de choque. Assim como há
lesões, com perda total da função célular nos casos de necrose de hepatócitos, por
exemplo, causada pela infecção do vírus da hepatite infecciosa canina. Entre os testes
de função hepática é considerado o mais comum e o melhor para detecção da lesão
hepática (THRALL, 2007).

Aspartato aminotransferase (AST)
É uma enzima de extravasamento, parte dela livre no citoplasma de hepatócitos, nota-
se sua maior concentração nas membranas das mitocôndrias, encontra-se presente
também em uma ampla variedade de tecidos como células do músculo cardíaco,
esquelético e fígado. O teste desta enzima geralmente é feito para diagnosticar
doenças musculares, pois ela não é considerada uma enzima hepato–específica
(BUSH, 2004; TRALL, 2007).
Fosfatase Alcalina (FA)
É uma enzima de indução sintetizada no fígado, nos osteoblastos, nos epitélio
intestinal, e renal e na placenta. Em cães, a meia-vida de FA intestinal, renal e
placentária é de, aproximadamente seis minutos; em gatos, é cerca de dois
minutos. O aumento da produção e de sua atividade sérica pode ser notado em
casos de maior atividade osteoblástica, colestase, indução por drogas como
corticóides e fenobarbital e várias doenças crônicas, inclusive neoplasias
(TRALL, 2007). Gamaglutamilitransferase (GGT)
Provas de síntese e metabolismo hepático
Albumina
Geralmente não se observa hipoalbuminemia até que ocorra perda de 60 a 80% da
função hepática, esta é muito comum em cães com doença hepática crônica, mas não
parece ser um achado comum em eqüinos com hepatopatia crônica (TRALL, 2007).
Globulinas
Embora a maior parte das globulinas que participam do sistema imune seja sintetizada
nos tecidos linfóides, outros tipos o são no fígado. A insuficiência hepática pode
resultar em menor síntese, portanto, em menor concentração sérica de globulina. Em
insuficiência hepática é comum se notar menor proporção albumina: globulina. Em
alguns casos de hepatopatia crônica, por exemplo, na cirrose hepática, a concentração
de globulina pode aumentar (BUSH, 2004; THRALL, 2007).

Níveis plasmáticos dos fatores da coagulação (tempo de protrombina)
É um exame laboratorial para avaliar a via extrínseca da coagulação. O tempo de
protrombina normal é de cerca de 12 a 15 segundos. Quanto maior for o TP, menor
será a concentração de protrombina no sangue. O TP mede os fatores II, V, VII, X e o
fibrinogênio (COLES, 1994).
Amônia
O aumento do nível de amônia no sangue pode indicar uma insuficiência hepática,
pois no fígado ela é metabolizada em uréia sendo eliminada pela urina, com o órgão
lesado não ocorre essa transformação acumulando amônia no sangue podendo trazer
lesão em sistema nervoso central (encefalopatia hepática), porque ela é extremamente
tóxica ao tecido nervoso (MEYER et al., 1995).
Pode ser encontrado cristal de biurato de amônio na urina isso ocorre sempre que
o fígado deixa de converter amônia em uréia aumentando o seu nível, portanto nos
casos de desvio portossistêmico e insuficiência hepática (MEYER et al., 1995).
Uréia
É sintetizada no fígado a partir da amônia; a maior parte vem do catabolismo de
proteína (BUSH, 2004). Uma diminuição da uréia usualmente acompanha aumentos
na concentração de amônia no sangue (COLES, 1994).
Colesterol
Sua excreção é feita pela bile, portanto um distúrbio no fluxo biliar (colestase) pode
aumentar o seu nível no sangue ocorrendo hipercolesterolêmica. O fígado é o
principal órgão de síntese de colesterol, em alguns tipos de insuficiência hepática
pode ocorrer à diminuição no sangue (hipocolesterolemia) (TRALL, 2007).
Glicose
O fígado tem participação importante no metabolismo de glicose sendo ela absorvida
no intestino delgado e é transportada através do sistema porta chegando aos

hepatócitos que transformam glicose em glicogênio mantendo assim o seu nível ideal
no sangue. Em animais com insuficiência hepática a glicemia pode estar baixa ou alta
isso tem relação devido à menor atividade da glicogenólise nos hepatócitos resultando
na sua diminuição. O aumento da glicose pode ocorrer quando tiver menor absorção
hepática resultando em hiperglicemia pós-prandial prolongada (COLES, 1994;
TRALL, 2007)
Provas de Transporte e Excreção Hepática
Bilirrubina: É um subproduto do metabolismo da hemoglobina formada através de
hemáceas velhas fagocitadas por macrófagos e liberadas e transportada para o fígado
por uma proteína. O aumento de bilirrubina pode ocorrer por vários fatores como
doença hemolítica, insuficiência hepática e colestase. Na insuficiência hepática ocorre
aumento de bilirrubina, porque parte dela vinda do intestino é eliminada pela bile.
Com o fígado comprometido não ocorre eliminação, tendo um acúmulo de bilirrubina
no sangue, ocasionando impregnação das mucosas (icterícia) (TRALL, 2007).
Ácidos Biliares: os ácidos biliares são produzidos no fígado e excretados pela bile.
Eles são encontrados em baixa concentração no sangue periférico. Qualquer distúrbio
envolvendo componentes estruturais do fígado pode causar o escape de ácidos biliares
para a circulação periférica. O teste de avaliação dos níveis de ácidos biliares no
sangue é muito mais sensível de função hepática do que a bilirrubina, ele aumentará
antes do desenvolvimento da icterícia. Os valores normais em jejum no gato são
menos que 5µmol/L, nos cães menos do que 10µmol/L e nos eqüinos é menos que
15µmol/L (BUSH, 2004).

Conclusão
Pode-se concluir que o exame da função hepática na medicina veterinária é de
extrema importância, no diagnóstico das injúrias hepáticas na Medicina Veterinária,
porém que poucos deles são específicos, portanto devem sempre ser realizados
concomitantemente.
