MoreLife – Family Weight Management

Self-Referral service for children aged 5-17 and/or their
parents. Children aged 5-12 attend with their parents
and children aged 13-17 have the initial assessment with
parents. Parents only can attend (without children) as
long as they have a child aged 0-17
Pharmacy Weight Management Service –
A series of 5 one-to-one sessions conducted by trained
pharmacy staff. Anyone aged over 16 with BMI>25 and
no medical contra-indications are eligible.
It is a self-referral service at participating pharmacies.
No medical contra-indications to exercise or weight loss
(e.g. Unstable angina, pregnancy, eating disorder).

Exercise Referral to increase physical activity levels
£2.20 per session – 12 weeks low to moderate intensity
exercise. Only people with a BMI < 40.

Complete the AUDIT C followed by the full AUDIT if
positive. If the Patient scored 8-15 on full AUDIT, deliver
brief intervention and give patient a Public Information
Leaflet (PIL).
If the AUDIT Score was 16 or above, refer Patient to the
Team Around the Practice Lifeline Worker (TAP) who are
part of the Lifeline Service and who are based in GP
Practices across Stockton.
If Audit score > 20 then refer directly to Lifeline services
If a patient is concerned at all about their alcohol intake
they can be seen by Lifeline

The Stockton Service Navigation Project (SSNP) is for
anyone living in Stockton aged 16+ with common
mental health problems such as stress, depression,
anxiety or those with long term health conditions. The
SSNP can help people in taking the first steps to improve
their health and wellbeing by supporting them to access
activities and services. We can help find an activity in
groups, classes or fitness groups to support health and
wellbeing. We can also signpost and support to access
