  3. Overall, ADEQUATE hwt INCREASED with WEALTH. However, there are exceptions to the rule, : Benin, Guatemala, Macedonia= NO DIFFERENCE In certain countries the OPPSITE TREND was observed: LAO, Indonesia and Panama used DECREASED with INC wealth.
