Lean / Six Sigma Cultural Awareness Survey TEMPLATE
Instructions The purpose of this survey is to measure employees' perception of the process improvement culture at (company name).  Please select the answer that most closely matches your overall experience in your work environment.  Try not to think too deeply about the question.  The survey will work best if you answer each question with your first instinct.    Remember, this survey is blinded and completely confidential.  Please be honest.  Every question must be answered.
