COMMUNITY DAY
On Monday, October 17th, 2011 the Imaging Centers of Mid-Atlantic Women’s Care,
PLC will be offering screening mammograms at no cost to women who have NO
INSURANCE. Ask your provider if you are eligible for a screening mammogram and
enroll today!! Space is limited.
Requirements:
- Uninsured women, 40 years old and up, who need a screening mammogram.
- Must NOT have any current breast problems such as pain, lumps, or discharges.
(Please see your provider for an evaluation if they exist.)
- Access to a primary care provider for results and necessary follow-up.
- Must be available Monday, October 17, 2011 for the mammogram.
A basic eligibility form will need to be completed.
Also, consent forms will be signed for transfer of mammogram results.
