هر فرآیند FMEAباید بخشي از یك سیستم كیفي فراگیر
باشد.اگر چه FMEAبه تنهایي مفید است اما یك شركت
بدون سیستم هاي پشتیباني و ارتقاء دهنده استفاده لزم را
از FMEAنمي برد.

رهبر گروه پس از تشكیل گروه ‘ توسط تیم یا مدیریت انتخاب
مي شود.مسئولیت رهبر گروه به شرح ذیل است :

نظارت بر تهیه منابع مورد نیاز گروه
اطمینان از پیشروي گروه FMEA
تصمیم گیري نهایي با رهبر گروه نیست
تنظیم هصورتجلسه در جلسات گروه الزامي است.

Gain Participants’ Attention 6 Minutes Ask why would we do an FMEA? WHY: Appropriate actions were recommended, but unfortunately were not taken Reference Ford studies (Booze-Allen) WHY: FMEAs can actually save time and reduce changes
Define FMEA 5 Minutes Ask the total class of participants to define what an FMEA is Document participant responses on Metaplan cards or flipcharts (hand written) Ask group to concur on a tentative definition Give the formal definition of FMEA
FMEA -- Purpose 1 Minute Graphically discuss the purpose of an FMEA Typically changes occur just before and just after Job 1 when FMEAs are not done, done poorly, no action is taken or actions are not communicated By doing FMEAs properly, failures, design changes and workload are evenly distributed and occur prior to Job 1
FMEA -- Purpose, continued 1 Minute Customer expects the right product which fulfills all functions DISASTER happens if we don’t deliver what customer wants and the product does not reliably function UNRELIABLE PRODUCTS result if we deliver what the customer wants, but the product does not reliably function RELIABLE PRODUCTS / CUSTOMER DOES NOT WANT happens when product fulfills all functions, but is not what customer expects EXCELLENT PRODUCTS results when we deliver what customer wants and product fulfills all functions QFD tells us the voice-of-the-customer FMEAs get the failure out of the products
FMEA -- Types 2 Minutes As a prevention tool, FMEA examines three phases where potential failures can occur CONCEPT FMEA (CFMEA) is used to analyze concepts in the early stages before hardware is defined (most often at system and subsystem) Focuses on potential failure modes associated with the proposed functions of a concept proposal caused by design decisions that introduce deficiencies Includes the interaction of multiple systems and interaction between the elements of a system at concept stages DESIGN FMEA (DFMEA) is used to analyze products before they are released to production Focuses on potential failure modes of products caused by design deficiencies Identifies potential designated characteristics called “Special Characteristics” DFMEAs are done at three levels -- system, subsystem and component levels PROCESS FMEA (PFMEA) is used to analyze manufacturing and assembly processes at the system, subsystem or component levels Focuses on potential product failure modes of process caused by manufacturing or assembly process deficiencies
FMEA Working Model 5 Minutes Direct participants to the Working Model pocket card or wall chart Explain that this is a model that will help team members do FMEAs Explain that the course purpose says participants will learn to become FMEA team members -- this model will be the major tool they will need The model directly relates to columns on the tabular FMEA form Explain that there is a form unique to each of the three types of FMEAs (See TRG Appendix A)
Generating FMEAs 7 Minutes Ask and answer commonly asked questions about the FMEA process Note: Answers are found on TRG 2-9 through 2-12

Team Approach to FMEA 2 Minutes Explain that for an FMEA to be effective, it is essential that a team approach be taken Team members who have a valuable contribution to make to the analysis are brought in at the appropriate time There is likely to be a core team supported by a wider support team Throughout the FMEA, the core team should always remain the same The support team will change to meet the needs of the analysis

PFMEA Team 5 Minutes Ask participants who should lead a PFMEA Team? Ask, Who else should be on the core team? Explain that the core team is established and begins by defining the scope of the PFMEA It is important that the correct skills and qualities are brought to the core team, and that roles are clearly defined Ask, Who might make up the supporting team? Support team provides specific expertise as needed Support team composition may change during analysis Again facilitation assistance can be obtained from FMEA Express facilitation pool TRG Appendix D Ensure that all team members are documented in the header of the FMEA form Point out where names are documented on FMEA tabular form on the wall chart

Block Diagram Example 1 Minute SAY: Imagine a team is charged with creating or revising an FMEA on a revised method to the headlamp Illustrate a block diagram using the vertical trimmer on the headlamp assembly Pass around actual headlamp assembly Point out the problem of boundary as shown on this slide being too large; not all this has changed Explain that block diagrams will probably look different on each FMEA team, but Each team must agree on symbols used (legend) Consistently use the same symbols
Block Diagram Example , continued 1 Minute Point out the components of the vertical trimmer as shown here This refined scope and boundary for analysis is now set at a more manageable level (than on previous slide)
Process Flow -- Macro 2 Minutes Flowcharting is a useful quality tool to identify both the macro and the micro levels of a process This flowchart shows all the macro operations in the manufacture of the headlamp assembly All nine of these operations would need to be included in a full Process FMEA There is a change that is going to be made to operation 30 where the vertical trimmer is assembled to the reflector Draw a boundary on the macro flow chart to indicate the area being looked at
Process Flow -- Micro 1 Minutes Create a second flow chart, a micro flow chart This micro flow chart of OP-30 shows three subsets: 30.1 Locate and fix (2 screws) base plate to reflector 30.2 Insert nose and positively locate pre-assembled screw and spring 30.3 Visually inspect the trimmer assembly Establishing the boundaries, thereby determines the scope of this FMEA This also determines the relevant composition of the support team which may to change during the analysis -- operation by operation
Define Process Function/Purpose 3 Minutes Stress that it is essential that all process functions/purposes are considered If one is omitted, the associated potential failures will not be considered Team needs to: Know what the process does at this operation Ensure that all process purposes are considered Share understanding of function/purpose and requirements The function/purpose of a process operation is what is achieved at that operation What the operation is there to do It needs to be precise, expressed in verb-noun-measurable format
Process Flow with Function/Purpose 2 Minutes Process purposes should be documented on the FMEA Refer to the Item Function Worksheet on TRG 3-20 Note that item function worksheet is laid out in the TRG for design Teams can change wording for process Transfer micro steps/specifications and any special operating conditions Don’t forget, the process must operate under certain conditions: i.e., Ford rates, line speed, manpower, etc. Show where purposes are recorded on the PFMEA tabular form
Consider Effects of Failure 2 Minutes Suggest FMEA Team use the Effects List Table to consider all possible effects for each failure mode Refer to Effects List Table on Flipchart and job aid Form should be filled out for each failure mode Effects are identified by asking questions regarding Next user Downstream users Ultimate customer, what customers see, feel, experience Vehicle Operations, driveability, safety Operator safety Compliance with government regulations -- related to vehicle/environment Others Refer to the tabular form and indicate that effects column comes before cause column Identify the consequences (effects) of each failure mode on each item: assembly, system, vehicle, etc. Note: like on DFMEA, when you transpose the effects from this table to the tabular form, that severity of each can be parenthetically included
Severity Ranking Table 3 Minutes Refer to Severity Ranking Table in TRG 6-16 or on pocket card Read from the Criteria: Severity of Effect Column, noting differences Stress that a criteria of 10 should be:“may endanger the operator of the machine” or “the operator in the assembly area,” not “may endanger machine”
