21. Life or death? Sometimes it comes down to knowing your numbers! September 2006: Six premature babies were given an overdose of Heparin at an Indiana Hospital. The preemies were administered a 10,000 unit dose instead of 10 units. Two of the six babies died within hours, one was transferred to another hospital and died two days later, three survived the overdose.

22. November 2007: The twins born to Hollywood actor, Dennis Quaid, and his wife Kimberly were given an overdose of Heparin at Cedar-Sinai Hospital in Los Angeles. At only 12 days old, the twins were administered a 1,000 unit dose instead of the 10 units prescribed. “Fighting for life.”

23. July 2008: A dozen premature babies were overdosed with Heparin at a Hospital in Corpus Christi, Texas. Two of the babies died in that instance.
