This chapter examines the characteristics of poverty in the United States, focusing particular attention on demographic aspects and ways of measuring poverty; family constitution and poverty; child poverty and elderly poverty; the urban and rural poor; and the connections between poverty and work-related issues such as the minimum wage, structural unemployment, dual labor markets, job training programs, and the alternative financial sector or the fringe economy. Last, key strategies developed to combat poverty will be surveyed.

Jennifer Lee is the Senior Program Officer for the Blue Cross Blue Shield of Massachusetts Foundation, where she manages grant portfolios for the Connecting Consumers with Care, Going Beyond Health Care, and Catalyst Fund areas. In this role, she facilitates the review processes of the different grant programs and supports grantees' ongoing learning and technical assistance needs. She also participates in and supports the Grantmaking Department's overall initiatives. She previously worked as Outreach and Enrollment Manager at Health Care For All, a Massachusetts-based health care consumer advocacy organization, where she oversaw education initiatives about health care reform. Prior to this position, she was the Team Lead and Program Associate in the Children's Division, where she coordinated the Massachusetts-based Covering Kids and Families Initiative, part of a national effort to enroll children and adults in low-cost or free health care coverage programs. She also served as an AmeriCorps Fellow for the Massachusetts Promise Fellowship Program at Northeastern University, where she developed Teens Leading the Way, a statewide coalition that sought to develop the policymaking skills of youth leaders. Jennifer is a past fellow of Grantmakers in Health's Terrance Keenan Institute for Emerging Leaders in Health Philanthropy and a graduate of the Foundation's Massachusetts Institute for Community Health Leadership. She has held previous leadership roles as Chair of the Board of Directors for the Massachusetts Association of Community Health Workers and Co-Chair of Asian Americans/Pacific Islanders in Philanthropy. She currently serves on the Board of Directors for Associated Grant Makers, a regional association of philanthropic organizations. She holds a Master's in Public Health from Tufts University's School of Medicine, and a Bachelor’s in Science from Boston College.

This paper will describe how environmental health is both a public health and social injustice issue and some of the environmental injustices in the United States. It will also explain the functions of environmental health movements in the US. Finally, the paper will describe some of the intervention opportunities in curbing environmental injustices including health education, advocacy, policy, leadership opportunities, and community focus and give my future leadership role as far as environmental health is concerned. Keywords Environmental health, public health, social injustice, minority groups Section 1: Topic Description The topic that I chose is Environmental Health. This topic is a public health issue because the environment in which we live in shapes our health every moment of every day. Our health can be affected by what we eat, where we live, and how we interact with the world around us. That is where environmental health professionals, programs and policies, all come into play. Environmental health is a large field in public health because of the numerous ways exterior factors can impact how we live, grow and, even eat. These factors concern how we address our natural environment like sanitation and clean water, but they are also the consequences of our actions as human beings (Krometis et al, 2017). Environmental Health is also a social justice issue because all the conversations about protecting habitats, mitigating climate change and recycling are not just about saving and protecting the earth, they are more about the struggle for protecting and providing basic human rights to good health. Every human being is entitled to quality air, clean water, and sanitation (Marmot, 2017). Section 2: Social Injustice Two Environmental Social Injustices One major environmental social injustice is the disproportional placing of dangerous waste amenities in poor and largely ethnic and racial minority localities. Research shows that there are ethnic and racial discrepancies in the locations of dangerous waste across the United States. As a result of these injustices people living in such an environment have developed diseases like cancer. Another social injustice is the environmental influence on healthy eating, physical activity, and obesity in ethnic and racial minority communities with low income. The principal concern here is the deprivation amplification such that in locations where have little individual resources, the local amenities that help people to live healthily, are poorer than those in non-socially and non-impoverished deprived places. Focusing on how inequities in the built environment – places where we work, live and play; transportation; food; and parks and green spaces - impact health, Dr. Frumkin described the dimensions of healthy communities and community design principles and the opportunities for effective interventions. He described the work of the Centers for Disease Control and Prevention in promoting health equity through healthy places. Presentation by Camara Jones, MD, MPH, PhD at the 2009 Virginia Health Equity Conference. Dr. Jones presents the “Cliff Analogy” for understanding four levels of health intervention: medical care, secondary prevention, primary prevention, and addressing the social determinants of health. She described how health disparities arise on three levels (differences in quality of care, differences in access to care, and differences in underlying exposures and opportunities) and expand the “Cliff Analogy” to illustrate the relationship between addressing the social determinants of health and addressing the social determinants of equity, which is a fifth level of health intervention. She identifies racism as one of the social determinants of equity and a fundamental cause of “racial”/ethnic health disparities in the United States, with racism defined as a system of structuring opportunity and assigning value based on the social interpretation of how one looks, which is what we call “race.” She described how racism impacts health on three levels (institutionalized, personally-mediated, and internalized) and animate understanding of these levels of racism with her “Gardener’s Tale” allegory. Finally, using data from the “Reactions to Race” module on the 2004 Behavioral Risk Factor Surveillance System, she examined the relationship between responses to “How do other people usually classify you in this country?” and self-rated general health status to provide evidence of the impacts of racism on health. Dr. Jones challenges us to broaden the scope of our public health interventions by asking the question “How is racism operating here?” and then working to create a system in which ALL people are highly valued and ALL people are able to develop to their full potential.
