1 - Initially there was a 7 developer to 1 QA ratio.
2 - No QA team members were assigned until after the 3rd Sprint.
1 – QA ran a sprint behind, Ratio swamped QA each Sprint
2 – When one feature was done, dev moved to the next (QA not in DoD)
3 - QA was hopelessly behind even with added QA team members.
4 - Untested items frequently escaped the Sprints resulting in many defects.
