21. I. Chọc hút kim nhỏ
• Sự không phù hợp lâm sàng và tính trạng BN thì FNA
có thể dùng để phân biệt nhiễm khuẩn hay không
• Lưu ý: bất kỳ một chọc hút nào cũng có thể đưa từ vô
khuẩn đến hữu khuẩn
• Không có vai trò của FNA trong giai đoạn đầu tụ dịch.
• Không đi qua đường dạ dày nếu chỉ chẩn đoán
• Phải chắc chắn không phải giả phình mạch
• Phải có kế hoạch trước khi can thiệp
– Mủ dẫn lưu to
– Dịch trong nhuộm Gram, nuôi cấy
– Không có dịch hoại tử bơm rửa nước muối nuôi cấy

• Mục đích của dẫn lưu trong viêm tụy cấp:
– Giải phóng các chất hoại tử do VTC gây ra
– Giảm áp lực ổ bụng
• Kỹ thuật
– Dưới hướng dẫn của siêu âm
– Dưới hướng dẫn của CLVT

• Chỉ định khi tụ dịch tăng lên, tình trạng bệnh nhân có
các dấu hiệu lâm sàng xấu, nhiễm trùng, tăng áp lực ổ
bụng do khối dịch…(>12mmHg)
• Không dùng khi tụ dịch ở giai đoạn sớm
• Có thể dùng để đánh dấu cho phẫu thuật
• Đường vào: qua gan, qua dạ dày, qua thành bụng, qua
khoang sau phúc mạc

– Chiến lược can thiệp tối thiểu
– Chỉ can thiệp khi cần thiết
– Đường vào can thiệp: qua gan, dạ dày, sau phúc

– Can thiệp dưới SA: làm tại giường, ….
– Can thiệp dưới CLVT: phức tạp, ưu tiên những
vùng sâu, siêu âm khó khăn…
