Forcan (Generic Fluconazole Tablets) is used to treat
certain types of fungal or yeast infections. Forcan
tablets are used for the treatment of the following
Systemic candidiasis, Mucosal candidiasis,
Cryptococosis and prevention of fungal infection in
patients with malignancy.

Forcan Tablets are manufactured by Cipla Limited, India
in the strengths of 50 mg, 150 mg and 200 mg.

Forcan tablets is Fluconazole. Each uncoated
Forcan tablet contains 50 mg, 150 mg or 200 mg
of Fluconazole respctively.

Uses of Forcan 50 mg, 150 mg and 200 mg tablets:
Forcan (Generic Fluconazole Tablets) is used to treat serious fungal
or yeast infections, including vaginal candidiasis, oropharyngeal
candidiasis (thrush, oral thrush), esophageal candidiasis (candida
esophagitis), other candida infections (including urinary tract
infections, peritonitis [inflammation of the lining of the stomach], and
infections that may occur in different parts of the body), or fungal
(cryptococcal) meningitis. This medicine works by killing the fungus
or yeast, or preventing its growth.
Fluconazole is also used to prevent candidiasis in patients having
bone marrow transplants who receive cancer or radiation treatment.

Recommended Adult Dosage
The dose of Forcan tablets (Generic Fluconazole tablets) to
treat Systemic Candida Infections in adults is 400 mg initially,
followed by 200 to 400 mg once daily. Treatment duration is
based on clinical and mycological response, but is usually at
least 6 to 8 weeks in cryptococcal meningitis.
To prevent relapse after a primary course of treatment for
acute cryptococcal meningitis in AIDS patients: 100 to 200 mg
daily.

Recommended Adult Dosage
The initial dose to treat Mucosal Candidiasis in adults is 50 mg
daily, which may be increased to 100 mg daily if needed. The
treatment duration is 7 to 14 days (oropharyngeal candidiasis,
except in severely immunocompromised patients), 14 days
(atrophic oral candidiasis associated w/ dentures), 14 to 30 days
(other mucosal candidiasis including oesophagitis).
The initial dose to treat Cryptococcal Infections in adults is 400
mg, followed by 200 to 400 mg once daily. Treatment duration is
based on clinical and mycological response, but is usually at least
6- to 8 weeks in cryptococcal meningitis. To prevent relapse after
a primary course of treatment for acute cryptococcal meningitis in
AIDS patients: 100 to 200 mg daily.

Recommended Adult Dosage
For the Prophylaxis of Fungal Infections in
Immunocompromised adult patients the dosage is 50 to
400 mg daily.
The recommended Fluconazole daily dosage for the
prevention of candidiasis in patients undergoing bone
marrow transplantation is 400 mg, once daily.

Contraindications
➔
Forcan tablets are contraindicated in patients with a known
hypersensitivity to Fluconazole or any component of the tablet.
➔
There is no information regarding cross-hypersensitivity
between Forcan Tablets and other azole antifungal agents.
➔
Caution should be used in prescribing Forcan Tablets to patients
with hypersensitivity to other azoles.
➔
Co-administration of Terfenadine is contraindicated in patients
receiving Forcan Tablets at multiple doses of 400 mg or higher
based upon results of a multiple-dose interaction study.

Contraindications
➔
Co-administration of other drugs known to prolong the QT
interval and that are metabolised via the enzyme CYP3A4,
such as Cisapride, Astemizole, Pimozide and Quinidine, are
contraindicated in patients being administered Fluconazole
tablets.

Storage Instructions
Forcan Tablets (Generic Fluconazole Tablets) have to be stored at
controlled room temperature i.e. from 20°C to 25°C (68°F to 77°F) with
excursions permitted between 15° to 30°C (59° to 86°F).
Keep this as well as all other medicines away from children and pets.

Warnings
Fluconazole should be administered with caution to patients with liver
dysfunction. Fluconazole has been associated with rare cases of
serious hepatic toxicity, including fatalities primarily in patients with
serious underlying medical conditions. In cases of Fluconazole-
associated hepatotoxicity, no obvious relationship to total daily dose,
duration of therapy, sex, or age of the patient has been observed.
In rare cases, anaphylaxis has been reported.
Exfoliative skin disorders during treatment with Fluconazole have been
reported. Fatal outcomes have been reported in patients with serious
underlying diseases. Patients with deep seated fungal infections who
develop rashes during treatment with fluconazole should be
monitored closely and the drug discontinued if lesions progress.

Warnings
Fluconazole should be discontinued in patients treated for superficial fungal
infection who develop a rash that may be attributed to Fluconazole.
Case reports describe a pattern of distinct congenital anomalies in infants
exposed in utero to high dose maternal fluconazole (400 to 800 mg/day)
during most or all of the first trimester.
Forcan tablets should be administered with caution to patients with renal
dysfunction. Adrenal insufficiency has been reported in patients receiving
azoles, including Fluconazole. Reversible cases of adrenal insufficiency have
been reported in patients receiving Fluconazole.
Patients on long-term treatment with Forcan tablets and Prednisone should
be carefully monitored for adrenal cortex insufficiency when fluconazole is
discontinued.

Possible Side Effects
Most common Forcan side effects with an incidence rate of
greater than 10% are headache, abdominal pain, diarrhoea,
nausea, vomiting, alanine aminotransferase increased,
aspartate aminotransferase increased, blood alkaline
phosphatase increased and rash.

Possible Side Effects
The following side effects occurred in greater than 1/1000 to less
than 1/100 of patients and include Anaemia, Decreased appetite,
Somnolence, insomnia, Seizures, paraesthesia, dizziness, taste
perversion, Vertigo, Constipation, dyspepsia, flatulence, dry mouth,
Cholestasis, jaundice, bilirubin increased, Drug eruption, urticaria,
pruritus, increased sweating, Myalgia, Fatigue, malaise, asthenia and
fever.

Overdosage
There have been reports of overdose with Fluconazole accompanied
by hallucination and paranoid behaviour.
In the event of Forcan overdose, symptomatic treatment (with
supportive measures and gastric lavage if clinically indicated) should
be instituted.
Fluconazole is largely excreted in urine; forced volume diuresis would
probably increase the elimination rate.
A 3-hour haemodialysis session decreases plasma levels by
approximately 50%.

Forcan Tablets During Pregnancy
Fluconazole (Forcan Tablets) has been classified by the US FDA as
Pregnancy Category C for single 150 mg tablet use for Vaginal
Candidiasis and Pregnancy Category D for all other indications.
There are no adequate and well-controlled clinical trials of
Fluconazole in pregnant women.
A few published case reports describe a rare pattern of distinct
congenital anomalies in infants exposed in-utero to high dose maternal
Fluconazole (400 to 800 mg/day) during most or all of the first
trimester. These reported anomalies are similar to those seen in animal
studies.

Forcan Tablets During Pregnancy
Use in pregnancy should be avoided except in patients with severe or
potentially life-threatening fungal infections in whom Fluconazole may
be used if the anticipated benefit outweighs the possible risk to the
foetus.
In case this medicine is used during pregnancy, or if the patient
becomes pregnant while taking the medication, the patient should be
informed of the potential hazard to the fetus.
