Presentation includes data on veterans and mental health, substance abuse and sexual health outcomes. Also includes a review of comorbidities among veterans living with HIV, and a sample of evidence concerning the interrelationship between mental health and incarceration. Finally, a source for help - Veterans Coalition of North Central Texas as a resource for veterans and their families needing access to mental health services and a strong social support community.

Effects of Mindfulness Yoga vs Stretching and Resistance Training Exercises on Anxiety and Depression for People With Parkinson Disease : A Randomized Clinical Trial - Parkinson disease (PD) is the second most common chronic neurodegenerative disease with heterogeneous symptomatology.1 Although PD is characterized by 4 motor symptoms (resting tremor, rigidity, bradykinesia, and postural instability), patients with PD experience a variety of nonmotor symptoms, including neuropsychiatric problems, cognitive impairment, sleep disturbances, and autonomic dysfunction. Psychological distress, including anxiety and depression (frequently co-occuring), is common in patients with PD, with a prevalence of 40% to 50%,2 and is associated with care dependency, poor work and social function, fast physical and cognitive decline, increased dementia risk, and high mortality.

As our knowledge about addiction is increasing the association between mental illness and addiction is better understood. The controversy about the appropriateness of the term Dual Diagnosis to describe such a heterogeneous group of patients has sparked a debate on treatment and assessment models. It highlighted the fact that as far as treatment modalities are concerned, one size might just not fit all. Dr Mouton reviews current knowledge on comorbidity in the addiction field. Focusing on more than psychiatric comorbidity, he also looks at physical, social, psychological, spiritual and cultural components affected by addiction. Describing the role of the psychiatrist in addiction care he poses the questions: What if dual diagnosis is actually the key to better understanding of our patients? What if this knowledge leads to more individualised treatments? And are we ready for personalised treatment in the addiction field?

Mental illness is common and disabling but the evidence is that fewer than half of people seek any treatment and few receive any help from specialized mental health professionals. In Canada, there are long waiting lists to see psychological therapists face to face despite the importance of non-drug therapies. One way to address this problem is to use computerized e-therapies which deliver structured mental health treatment via a computer. Dr. Simon Hatcher, Psychiatrist at The Royal's Community Mental Health Program and Vice Chair of Research for the Department of Psychiatry at the University of Ottawa, lead a discussion about the role of technology in mental health treatment. Highlights include: the effectiveness of online mental health treatments and opportunities for innovation and policy change in field of mental health.

Our Conversations lecture 'Hope, Humanity and Empowerment: Strengths-focused Cognitive Behavioural Therapy for Psychosis (& Schizophrenia)' was presented by staff members of the Integrated Forensic, Recovery and Schizophrenia programs at The Royal. Psychosis can be associated with a variety of mental health problems, including schizophrenia, severe depression, bipolar disorder, anxiety, and post-traumatic stress disorders. While traditional treatments for psychosis have emphasized medication-based strategies, research now suggests that individuals affected by psychosis can greatly benefit from talk therapies such as cognitive behavioural therapy for psychosis (CBTP). This session included information on the collaborative work being done between The Royal’s Sexual Behaviours Clinic (SBC) and Ottawa Police Service’s High Risk Offender Unit (HROU). Dr. Paul Fedoroff was the moderator and began the presentations with an overview of innovative work being done within the SBC and the common goals of the Clinic and the HROU. Staff Sargent Dana Reynolds and Det. Mark Horton discussed the role of their team in the community based management of high risk sexual offenders. More specifically they discussed the role of the Unit and common legal designations utilized for high risk sexual offenders. Lisa Murphy, M.C.A. provided an overview of sex offender registries (SORs) and public notification and made comparisons between the approaches used in Canada and the United States. A discussion period followed the panel presentations.

What are Obsessions? • Intrusive, recurring, persistent thoughts, mental images, or urges that cause marked anxiety or distress • More than “excessive worry” •
A person tries very hard to get rid of the obesssion with another thought or behaviour • A person knows the obsession comes from his/her own mind Based on DSM‐IV‐TR (2000)

Some Common Obsessions • Excessive, intrusive thoughts about dirt, germs, illness, or contaminaZon • Fears of having accidentally caused harm •
Excessive doubZng of whether tasks were complete or accurate • UpsePng, “nonsensical” need for specific order or exactness • UpsePng aggressive, sexual, or religious thoughts that are out of character

Intrusive Thoughts are NORMAL • 90% of people report experiencing intrusive thoughts (Salkovskis, 1998) • It is not the intrusion that makes an obsession or OCD; it is our anxiety, distress, and reacZon to it

What are Compulsions? • RepeZZve behaviours or mental tasks a person must perform according to very set rules and/or in response to an obsession • Compulsions aim to bring down distress or prevent a bad thing from happening, although they are either very excessive or wouldn’t realisZcally work

ERP: The BoRom Line • Very challenging, but it works – Extremely well‐researched, established, and effecZve treatment for OCD –
ERP and similar therapies are effecZve for approximately 2/3 to 3/4 of individuals

OCD‐specific experience – Behavioural or cogniZve‐behavioural treatment approach • Unless the pracZZoner is a physician, his/her services will likely not be covered by OHIP / RAMQ; however, private insurance may assist
