2. DEFINATION
A removable partial denture or complete denture that
covers and rests on one or more of the remaining
natural teeth and/or dental implants.
It is also known as overlay denture, hybrid
prosthesis, telescoping denture or tooth supported
denture.
3. RATIONALE
Remaining roots offer the best available support for
occlusal force.
Accelerated rate of bone resorption is prevented.
It increases the patient’s manipulative skills in
handling the denture. As the patient’s proprioception
is maintained.
4. INDICATIONS
Periodontal disease
Few natural remaining teeth
Insufficient crown/root ratios
Severe attrition and loss of vertical dimension
Congenital or acquired intraoral defects e.g. cleft
palate, hypodontia
Mandibular arch where resorption rate is faster
Edentulous maxilla opposing dentate mandible
Post traumatic/post surgical cases
5. SELECTION OF
ABUTEMENT TEETH
The following factors need to be put into consideration;
1. Periodontal status.
2. Abutement location.
One per quadrant
No adjacent teeth
3.Endodontic and prosthetic status.
Usually mandibular cuspids and premolars
6. PREPARATORY TREATMENT
1. Construct an immediate treatment . For
esthetics and to retain jaw relations.
2. Remove hopeless teeth and insert the
removable prosthesis.
3. During the healing period, institute the
periodontal and endodontic treatment.
7. TOOTH PREPARATION
Remove sufficient tooth structure to provide
favorable root crown ratio.
Reduce the crown length up to 2 mm above the
gingival crest or extend a chamfer type margin
slightly beneath free gingival margin.
Taper the preparation occlusogingivally.
8. PLACEMENT
Overdentures can be placed either directly or
indirectly. The procedures are as follows;
I. Direct placement
Male or female premanufactured attachment is
cemented into root
Denture is made and inserted
Corresponding male or female attachment is
inserted in root
Attachment is picked up directly in the overdenture
with cold cure acrylic
9. PLACEMENT
II. Indirect Placement
Male or female attachment is cemented into root or
may need to be cast onto coping
Corresponding male or female transfer analog is
inserted into root attachment
Transfer impression is taken and models are poured
with transfer in place
Laboratory processes denture with corresponding
attachment in place
