• Anaphylactic shock

• Unresponsive patient with a strong odor of alcohol
• Hypoglycemia with a change in mental status
• Incubated head bleed with unequal pupils
• Child that fell out of a tree and is unresponsive to painful stimuli

Severe pain criteria to justify an ESI level 2
• A patient with 10/10 flank pain who is writing
at triage
• An 80-year-old female with 7/10 generalized
abdominal pain
• A patient in acute sickle cell pain crisis
• An oncology patient with severe pain
• Any full or partial thickness burn
High risk situations
• Active chest pain suspicious Acute coronary
syndrome
• A needle sticking a health care worker
• Signs of stroke
• A rule out ectopic pregnancywith
hemodynamics stable
• A suicidal or humicidal patient
