ในประเทศสหรัฐอเมริกา ในสวนที่เกี่ยวของกับการวินิจฉัยและรักษาผูปวยฉุกเฉิน
แนวทางการดูแลรักษาผูปวยนี้ไดทําการ review แนวทางการดูแลรักษา ผูปวย pulmonary embolism
( PE ) 3 แนวทาง โดย venous thromboembolism (VTE) นั้นจัดเปนภาวะที่พบไดบอย และพบไดตั้งแตอาการ
เล็กนอยจนถึงอาการรุนแรง ขอมูลจากการศึกษาที่มีผลชัดเจนตอการดูแลรักษาผูปวยฉุกเฉิน คือ ในผูปวย PE
ที่มภาวะ hemodynamic unstable มีโอกาสเกิด morbility และ mortality ไดสง โดยปจจุบันนี้พบผูปวยเสียชีวิต
ี ู
จาก PE ในประเทศสหรัฐฯ สูงถึง 300,000 รายตอป และ ในสวนผูปวยที่รอดชีวิตจาก PE ที่มีอาการรุนแรง ก็ยังมี
โอกาสเกิด morbidity จากภาวะ RV dysfunction ไดสูง หลักฐานจากการศึกษาในปจจุบันนี้ มีขอมูลสนับสนุน
การใหยา Thrombolysis มากขึ้น โดยเฉพาะในผูปวยอาการรุนแรง ดังนั้น การดูแลรักษาผูปวย PE ที่มี
ขนาดพยาธิสภาพใหญจึงมีจําเปนตองมีแนวทางการดูแลรักษาที่ชัดเจน โดยเฉพาะ เพื่อพิจารณาใชยา
thrombolysis เพื่อใหเกิดประโยชนสูงสุดจากการใชยาและมีโอกาสเสี่ยงตอภาวะแทรกซอนจากยาใหนอยที่สุด
Practice guideline impact แนวทางการดูแลรักษาที่สงผลตอแนวทางปฎิบัติในปจจุบัน
• การใหยา intravenous thrombolysis แนะนําใหในผูปวย massive PE เชน ผูปวย PE ที่มีภาวะ unstable
hemodynamic
• การวินิจฉัยภาวะ RV dysfunction เปนสิ่งสําคัญในการตัดสินใจวาผูปวยเปนผูปวย PE ที่มีภาวะ
hemodynamic stable และมีโอกาสเกิดภาวะแทรกซอนจากตัวโรคไดสูง ซึ่งผูปวยกลุมนีมีแนวโนม

ที่จะไดรับ ประโยชนจากการใหยา intravenous thrombolysis
• การใหยา intravenous thrombolysis ในผูปวย stable PE ที่มีภาวะ RV dysfunction ยังไมขอมูล
สนับสนุนที่ชัดเจน จะเห็นไดจากไมมีขอบงชี้ในการใชยาจากแนวทาการดูแลรักษาจาก American Heart
Association and the American College of Chest Physicians แตสําหรับแนวทาง การดูแลรักษาของ
American College of Emergency Physicians มีแนวโนมสนับสนุนการดวยวิธีดังกลาวมากขึ้น
2. Critical Issues In The Evaluation And Management Of Adult Patients Presenting To The Emergency
Department With Suspected Pulmonary Embolism
จาก Annals of Emergency Medicine, 2011
แนวทางการดูลรักษานี้เขียนขึ้นโดย committee และ subcommittee ของ American College of
Emergency Physician (ACEP) โดยการสนับสนุนทุนจากสมาคมเอง โดยทางกลุมไดพิจารณาตั้ง 6 คําถามเพื่อทํา
literature search และ review โดยใชผูเชี่ยวชาญจาก emergency physician, cardiologist, สมาชิกของ
American College of Chest Physicians, American College of Radiology, ACEP’s ultrasound Section,
ACEP’s Quality and Performance Committee อันเปนที่ที่มาแนวทางการรักษานี้
โดย evidence จัดลําดับเปน I, II, III และ X-fetally flawed
สวน Recommendation จัดลําดับตาม ความนาเชื่อถือของ evidence ดังนี้
A มีความนาเชื่อถือทางคลีนิคสูง
B มีความนาเชื่อถือทางคลีนิคปานกลาง
C จากการศึกษา class C หรือ ไมพบหลักฐานเพียงพอ อาศัยขอมูลจากความเห็นของจากที่
ประชุมเปนหลัก
คําถามที่เลือกมาใหทําแนวทางการดูแลรักษานี้มีที่มาจากการตีพิมพป 2003 โดยไดยกบางหัวขอ
ที่ยังไมมีขอมูลสนับสนุนชัดเจนระบุไวมาปรับปรุงเพิ่มเติม รวมถึงเพิ่มประเด็นที่หนาสนใจจากป 2003 แนวทาง
ใหการรักษานี้มจําทําขึ้นเพื่อจุดประสงคเปน evidence-based guideline ใหแพทยที่ปฏิบัติงานในหองฉุกเฉิน

ใชดูแลรักษาผูปวย ที่มาดวยอาการสงสัย PE โดยการแนวทางการรักษานี้ไมไดรับ การสนับสนุนทุนการศึกษาใดๆ
จากภาคเอกชน ใชงบประมานในการดําเนินงานทั้งหมดจากสมาคมเอง
กลุมเปาหมายของแนวทางการดูแลรักษานี้ คือ แพทยที่ปฏิบัติงานในหองฉุกเฉินของโรงพยาบาล
หรือหองฉุกเฉินที่มีหนวยสังเกตุอาการ โดยแนวทางการดูแลรักษานี้สามารถใชไดกับผูปวยผูใหญที่สงสัย PE
ไมวาจะเปนผูปวยหัวใจหยุดเตน ผูปวยตั้งครรภ แตแนวทางใหการรักษานี้ไมเหมาะกับผูปวยเด็ก หรือ
ผูปวยที่ไมสงสัย หรือไมมีอาการของ PE

Critical Question 4: อะไรคือประโยชนของการทํา computed tomography pulmonary angiogram
เพื่อยืนยันวาผูไมมีภาวะ PE
• Level A Recommendations : none specific
• Level B Recommendations : ในผูปวย low pretest probability หรือ Well score นอยกวาหรือเทากับ 4
ที่ตองการการตรวจวินิจฉัยเพิ่มเติม (เชน รายที่ผลตรวจ D-dimer ผิดปรกติ, หรือไมสามารถตรวจ high
sensitive D-dimer ได) หากผลการตรวจ computed tomography pulmonary angiogram
ปรกติสามารถยืนยันไดวาไมมีภาวะ PE
• Level C Recommendations
1. ในผูปวย intermediate pretest probability ที่ผลตรวจ computed tomography pulmonary angiogram
ใหผลปรกติแตอาการของผูปวยนาสงสัยวาจะมีภาวะ PE และยังไมไดทําการตรวจ CT venogram
แนะนําใหทําการตรวจเพิ่มเติมเพื่อยืนยันวาผูปวยไมมีภาวะ PE ดังนี้ คือ D-dimer, Lower extremities
imaging, VQ scaning, หรือ traditional pulmonary arteriography
2. ในผูปวย high pretest probability ที่ผลตรวจ computed tomography pulmonary angiogram
ใหผลปรกติ และยังไมไดทําการตรวจ CT venogram แนะนําใหทําการตรวจเพิ่มเติมเพื่อยืนยันวา
ผูปวยไมมีภาวะ PE ดังนี้ คือ D-dimer, Lower extremities imaging, VQ scaning, หรือ traditional
pulmonary arteriography
ดังนั้น ถาหากผลตรวจ highly sensitive quantitative D-dimer ใหผลปรกติ รวมกับ multidetector CT
pulmonary angiogram ใหผลปรกติ สามารถสรุปไดวา post test probability ของ VTE ในผูปวยรายนั้น
นอยกวา 1%
Critical Question 5: อะไรคือขอบงชี้ในการตรวจ venous imaging ในผูปวยสงสัย PE
• Level A Recommendations : non specified
• Level B Recommendations : การตรวจ venous imaging เบื้องตน ถาผลตรวจพบ blood clot
ในเสนเลือดดํา รวมกับอาการที่สงสัย PE สามารถใหการวินิจฉัย PE เบื้องตนได เและยังชวยลดการตรวจ
imaging อื่นๆ ในหองฉุกเฉินได
ตัวอยางผูปวยรายที่แนะนําการตรวจ venous ultrasound เชน ในผูปวยที่สงสัย PE และมีอาการแสดง
สงสัย deep venous thrombosis (DVT) และมีขอบงหามในการตรวจ CT scan (เชน คาการทํางาน
ของไตผิดปรกติ หรือผูปวยตั้งครรภ)
4. • Level C Recommendations :
1. ในผูปวย intermediate pretest probability หากผลตรวงจ CT angiogram ใหผลปรกติหากยังไมได
ทำการตรวจ CT venogram การตรวจ lower extremities venous ultrasound เปนการตรวจเพิ่มเติมที่
แนะนําเพื่อยืนยันวาผูปวยไมมีภาวะ PE
2. ในผูปวย high pretest probability หากผลตรวงจ CT angiogram ใหผลปรกติ และยังไมไดทําการตรวจ
CT venogram การตรวจ lower extremities venous ultrasound เปนการตรวจเพิ่มเติมที่แนะนํา
เพื่อยืนยันวาผูปวยไมมีภาวะ PE
Critical Question 6: อะไรคือขอบงชี้ของการใหยา thrombolytic ในผูปวย PE
• Level A Recommendations : non specified
• Level B Recommendations :
1. แนะนำใหยา thrombolytic ในผูปวย PE ที่มีการตรวจยืนยันการวินิจฉัย และมีภาวะ unstable
hemodynamic รวมถึงไดรับการพิจารณาแลววาประโยชนของการใหยามีมากกวาโทษจากโอกาส
เสี่ยงของภาวะแทรกซอนจากใหยา โดยเฉพาะโอกาสเสียงตอภาวะเลือดออกผิดปรกติ

2. การทําฝาตัด (surgical or mechanical thrombectomy) ถือวาเปนอีกทางเลือกหนึ่งของผูปวยที่มี
โอกาสเสี่ยงตอภาวะแทรกซอนเลือดออกผิดปรกติจากใหยา หากมีศักยภาพทําได
• Level C Recommendations:
1. พิจารณาใหยา thrombolytic ในรายที่มีภาวะ unstable hemodynamic โดยใหในผูปวยที่เขาไดกับ PE
แตไมสามารถทําการตรวจวินิจฉัยยืนยันภาวะ PE ได
2. ปจจุบันนี้ยังไมหลักฐานสนับสนุนเพียงพอในการใหยา thrombolytic ในกลุมผูปวย stable hemodynamic
ที่มีภาวะ RV dysfunction ถึงแมวาจะมีหลักฐานสนับสนุนวาการใหยา thrombolytic สามารถชวยเพิ่ม RV
function ไดในเวลารวดเร็ว แตไมพบการศึกษาใดที่แสดงวายา thrombolytic มีสวนชวยเพิ่มโอกาส
รอดชีวิตในผูปวยในกลุมดังกลาว
Editor comment
การใช Well score, Geneva score, Kline rule, Pisa model หรือ Gestalt clinical assessment
เพื่อพิจารณาโอกาสการเกิด PE เปน low, intermediate และ high pretest probability ลวนแลวแตมี
ความสําคัญในการวินิจฉัยและกําหนดแนวทางการรักษาผูปวยทั้งสิ้น เชน Level B recommendation แนะนําวา
PERC rule ควรใชสําหรับผูปวย Low pretest probability เทานั้น และหลักฐานสนับสนุนการสงตรวจ D-dimer
ขึ้นกับ pretest probability เทานั้น แตอยางไรก็ตามไมวาจะใชหลัก criteria ใด ไมพบขอมูลสนับสนุนวาวิธีใด
มีความชัดเจนและความนาเชื่อถือในการวินิจฉัยมากวาวิธีอื่นๆ
ในเรื่อง D-dimer testing มีการเปลี่ยนแปลงที่ชัดเจนของแนวทางการดูแลรักษาป 2011 จากป 2003 คือ
มี evidence base level C สนับสนุนการตรวจ D-dimer ใน intermediate risk เพิ่มขึ้นมา โดยอาศัย evidence
base จาก consensus (ไมมีขอมูลจาก evidence base ระดับที่สูงกวานั้น) รวมถึงมีการศึกษา retrospective 2
การศึกษา จาก Righini ที่แสดงวา ผลตรวจ D-dimer ที่ใหผลปรกติจะมีสวนชวยในการวินิจฉัย PE จะลดลงตาม
pretest probability จาก low ไป intermediate risk ถึงแมการศึกษาดังกลาวนี้ไมมีผลตอแนวทางการรักษา
แตการศึกษาดังกลาวมีผลแสดงใหเห็นวา specificity ของ D-dimer ขึ้นกับ pretest probability ของ PE
อยางไรก็ตาม D-dimer ก็สามารถสงตรวจไดในผูปวย intermediate และ high pretest probability
แตตองสงตรวจ CT angiography เพื่อวินิจฉัย PE ในทางกลับกันเนื่องจาก CT angiogram อาจใหผล false
6. ซึ่งอาศัยการทํา ultrasound ตรวจหาภาวะ RV strain หรือ RV dysfunction ที่เขาไดกับ submassive หรือ massive
PE (พิจารณาตรวจ echocardiography เพื่อหา RV infarction, ตรวจ electrocardiogram EKG เพื่อหา
incomplete right bundle branch block หรือ anteroseptal ST T change หรือ ตรวจ brain natriuretic peptide
BNP, troponin เพื่อชวยพิจารณาการวินิจฉัยภาวะ PE) และ พิจารณาตรวจ focus cardiac sonography โดย
Emergency Physicians เพื่อพิจารณาขนาด ventricle หากตรวจพบวาขางขวามีขนาดใหญกวาขางซายนั้น
มีความจําเพาะเจาะจงเพียงพอในการวินิจฉัยภาวะ RV strain

คํานิยามภาวะตางๆที่เกี่ยวของกับ PE ของ American Heart Association
• Massive PE คือ Acute PE ที่มีภาวะดังตอไปนี้
1. ภาวะความดันโลหิตต่ํา (systolic blood pressure SBP<90 mmHg อยางนอย 15 นาที หรือ
ความดันโลหิตต่ําที่ตองไดรับยา inotropic) และผูปวยตองไมมีสาเหตุจากสาเหตุอื่นๆ ของความดันโลหิตต่ํา
เชน arrhythmia, hypovolemia, sepsis หรือ left ventricular dysfunction LV
2. หัวใจหยุดเตนหรือคลําชีพจรไมได
3. Bradycardia หัวใจเตนชากวา 40 ครังตอนาที รวมกับมีอาการหรืออาการแสดงของภาวะช็อค

8. • Submassive PE คือ acute PE ที่ไมพบภาวะความดันโลหิตต่ํา (systolic blood pressure SBP≥ 90
mmHg) รวมกับภาวะ RV dysfunction หรือ myocardial necrosis
• RV dysfunction หมายถึง ภาวะดังตอไปนี้อยางนอย 1 อยาง
1. พบ RV dilation (apical 4-chamber RV diameter divided by LV diameter > 0.9) หรือพบ RV systolic
dysfunction โดยการทำ echocardiography
2. พบ RV dilation (4-chamber RV diameter divided by LV diameter> 0.9) จากการทำ CT scan
3. ผลการตรวจ BNP มีคาผิดปรกติ (> 90 pg/mL)
4. ผลการตรวจ N-terminal pro-BNP มีคาผิดปรกติ (> 500 pg/mL) or
5. ผลการตรวจคลื่นไฟฟาหัวใจผิดปรกติ ( Electrocardiographic พบ new complete or incomplete right
bundle-branch block, anteroseptal ST elevation or depression, หรือ anteroseptal T-wave inversion)
• Myocardial necrosis คือ ภาวะที่ผลการตรวจ troponin I เพิ่มมากกวา 0.4 ng/mL หรือ
ผลการตรวจ troponin T เพิ่มมากกวา 0.1 ng/mL
• Low-risk PE คือ Acute PE ที่ตรวจไมพบอาการ อาการแสดง ของ massive or submassive PE
Recommendations For Fibrinolysis For Acute Pulmonary Embolism
1. Fibrinolysis เหมาะสําหรับผูปวย acute PE ที่พบภาวะ massive PE และโอกาสเสี่ยง
ตอภาวะแทรกซอนเลือดออกผิดปรกติอยูในเกณฑที่รับได (Class IIa; Level of Evidence B).
2. Fibrinolysis อาจพิจารณาใหในผูปวย submassive acute PE โดยพิจารณาแนวโนม prognosis
ของตัวโรค (เชน hemodynamic instability, worsening respiratory insufficiency, severe RV
dysfunction, or major myocardial necrosis) และผูปวยนั้นตองมีโอกาสเกิดภาวะแทรกซอน
เลือดออกผิดปรกติต่ํา (Class IIb; Level of Evidence C).
3. Fibrinolysis ไมแนะนําใหในผูปวย low-risk PE (Class III; Level of Evidence B) หรือ submassive
acute PE ที่พบเพียง minor RV dysfunction หรือ minor myocardial necrosis โดยไมมีอาการหรือ
อาการแสดงอื่นๆ (Class III; Level of Evidence B).
4. Fibrinolysis ไมแนะนําใหในผูปวย cardiac arrest ที่ไมทราบสาเหตุหรือไมสามารถยืนยันการตรวจ
วินิจฉัย Acute PE (Class III; Level of Evidence B).
ขอหามในการให Fibrinolysis
1. Absolute contraindications
• ประวัติเลือดออกในสมอง (ไมกําหนดระยะเวลา)
• ประวัติ structural intracranial cerebrovascular disease เชน arteriovenous malformation
• ประวัติ malignant intracranial neoplasm
• ประวัติ Ischemic stroke ภายใน 3 เดือน
• ผูปวยสงสัย aortic dissection
