4. 6. BRIEF RESUME OF THE INTENDED WORK:
6.1 Need for the study:
A healthy woman is a promise of a healthy family and a woman’s health status is a
complex arrangement controlled by a range of factors headed by her reproductive system.
The Concept of a healthy yoni has been asserted in various phases of a woman’s life from
puberty to marriage to child birth and thereafter 2
.
Infections pertaining to the yoni are a burning problem for women irrespective of
their age or socio-economic status. It hampers their day to day chores and most importantly
makes them go through the mental trauma of fear, guilt and depression. Most women
gradually go into isolation and the entire family is put to distress.
Sleshmala yoni vyapad is described as a condition where the vitiated Kapha reaches
reproductive system and causes Kandu, Alpa Vedana and Pichhila, Sheetha, Pandu Varna

along with arthava dusti and pandu . These features can be correlated to
trichomonas vaginalis which shows a clinical picture of pruritis, vaginal soreness and
abnormal vaginal discharge. A microscopic wet mount is done and if positive, then the
diagnosis is confirmed15
The current treatment protocol advice the use of Metranidazole which have their own side
effects like hypersensitivity reactions,headache,stomatitis etc. So a more appropriate &
effective treatment with more efficacy is the need of the study.
Arkaadi yoni varti –includes latex of arka , saindhava lavana n masha.
Latex of arka acts as shothahara, kandugna,kapha nissaraka, vedana sthapaka and
ropana.
Saindhava lavana has sukshma & teekshna properties which will help the easier
absorption of the drug20
.
Therefore a varti prepared with these drugs and masha (Vigna mungo) as a base will help
combat the local disease causing agent (tricomonas vaginalis) and simultaneously strengthen
the vagina. Moreover, the use of a topical medicine like varti will act directly on the vaginal
mucosa and enable easier and target specific reaction.
Nimba taila which acts as krimigna and kapha-pittahara helps in relieving the symptom like
kandu.18
.
Researches has showed that it has anti-bacterial & anti-microbial activities , also it acts as
immune-stimulant activity.
And tailas are the most preffered medium for pichu because of its retention ability.
Keeping all these facts in mind the present work a clinical comparative study has
been undertaken with following aims and objectives
Aims & Objectives:
1. Evaluate the effect of arkaadi yoni varti in relieving the sleshmala yoni vyapat caused by
tricomonas vaganalis.
2. evaluate the effect of nimba taila pichu in relieving the sleshmala yoni vyapat.
3.To compare the effect of arkadi yoni varti and nimba taila pichu in relieving the sleshmala
yoni vyapat.
5. 6.2 Review of the literature:
1. Charaka has mentioned lakshanas of picchilaam, sheetaam, kandugrasthaam,
alpavedanaam along with panduvarnata and pandupicchila artava4
2. Sushruta has mentioned yoni which is picchila along with kandu and atisheetala.6
3. Vagbhata has mentioned painlessness of the yoni along with lakshanas like
sheetalaam, kandulaam, pandu, picchilaam and artava with similar qualities.7,9
4. Madhavanidana, Bhavaprakasha, and Yogaratnakara have followed the opinion of
sushruta1
.
5.Baishajya Ratnavali contains the reference of nimba taila13
.

6.3 Objective of the study:
1. To study regarding sleshmala yoni vyapat.
2. To study the effect of Arkaadi yoni varti in shleshmala yoni vyapat.
3. To study the effect of nimba taila yoni pichu dharana in shleshmala yoni vyapat.
4. To study the comparative effect of arkadi yoni vyapat and nimba taila pichu dharana
in sleshmala yoni vyapat.
6. 7. MATERIALS AND METHODS
7.1) Source of data: Patients with sleshmala yoni vyapat diagnosed with tricomonas
vaginalis infection attending OPD of S.J.G Ayurvedic Hospital, Koppal shall be taken for
study.
7.2) Method of collection of data:
A proforma will be prepared with all points of history taking, physical signs and lab
investigations .The parameters of signs and symptoms will be scored and will be
analyzed by standard method of statistical analysis.
Diagnostic criteria
Diagnosis will be done on the basis of the local signs and symptoms of Sleshmala yoni
vyapat as described by Charaka. Patients with the following features:
Picchila (unctuous) and Sheeta srava
Kandugrastha(itching sensation in vagina)
Alpa vedana(mild pain)
Gandha (foul smelling)
For the final diagnosis wet mount preparation of trichomonas vaganalis should be positive.
Inclusion criteria
1. Married women fulfilling the diagnostic criteria of sleshmala yoni vyapat.
Exclusion criteria
1. Unmarried
2. Post menopausal
3. Cervical erosion.
4. Pregnancy.
5. Other Sexually Transmitted Diseases.
6. Diabetes mellitus
7. Anaemia
8. Under prolonged use of antibiotics
9. Local skin lesions.
Treatment Groups:
30 patients will be selected as per diagnostic criteria and made 2 groups:
Group A- For 15 patients- Varti will be applied as per following procedure
7. Group B- For other 15 patients-Pichu dharana will be done as per the following
procedure.
GROUP-A:
Plan of treatment:
Arkadi yoni varti includes latex of arka , saindhava lavana n masha. Latex of the arka is
mixed with an equal quantity of saidhava lavana and masha as a base. It is then rolled
into finger long wicks and dried those vartis .
Then dried vartis are put in vagina of that patient with aseptic measures, after 1 hour, it
cleaned with sterilized warm water, and this is repeated for 2-3 times /day.
This procedure will be done continuously for 7 days during the post menstrual phase.
During this period sex is not allowed.
Kaphaja aahara and viharas should be avoided.(snigda and picchila aahara, divaswpana
etc.)
Duration of treatment:
The Varti will be applied during the post menstrual phase for 7 consecutive days
GROUP-B:
Plan of treatment:
. Nimba taila which is prepared out of seeds of nimba .It is then soaked in piece of cotton
cloth or gouge piece and pichu dharana is done to patient in the time gap of pre void and post
void with aseptic measures then it is cleaned with sterilized warm water.
This procedure will be done continuously for 7 days during the post menstrual phase.
During this period sex is not allowed.
Kaphaja aahara and viharas should be avoided.(snigda and picchila aahara, divaswpana etc.)
Duration of treatment:
During the post menstrual phase for 7 consecutive days
Assessment Criteria:
Patients will be examined on the day 7th, day 14th and day 21st of the treatment. Detailed
history and gynecological examination will be performed on all the patients.
Assessment will be done on the basis of improvement of the clinical features
The follow up smear will be assessed for tricomonas vaganalis.
Subjective Parameters:
1. Picchilata

Objective Parameters:
1. profuse,thin creamy discharge.
2. Red and swollen vulva with evidence of pruritus like scratch marks.
3. Wet mount showing tricomonas vaginalis positive.
Any sign of irritation or any other side effects if encountered during the treatment period
will be noted.
Gradings should be given for subjective and objective parameters.
