consistent definition of a near miss and non consistent data being
captured for near misses
- A possible consensus definition is:
âą A commonly accepted (but not universally) definition is: âA sequence of
events and/or conditions that could have resulted in a lossâ
- A good starting point for data reporting include:

âą What happened, Where, When, and in What sequence
âą What were the potential losses and their severity?
