II. Nursing Service: A.Guidelines/ Policy: 1. Tardiness/ Absenteeism  ,[object Object],	No grace period for 2-10 and 10-6 shifts ,[object Object],	1st offense: Verbal Reprimand 	2nd Offense Written Reprimand 	3rd Offense 1-2 days suspension Note: The Senior Nurse will collect the Daily Time Record to be submitted to the Nurse Supervisor/ Charge Nurse on duty.
	C. Absence without official leave or notice: 		1st Offense: Written Reprimand 		2nd Offense: 3-4 days suspension 		3rd Offense: 1 schedule suspension Note: ,[object Object]
Absente should submit an excuse letter.
Notice of absence should be 8 hours prior to specified time (schedule).
Informing of absence should be through telephone and not through text.
Supervisor or Charge Nurse on duty is responsible for receiving telephone call for notice of absence.
Emergency leave or Absent (scope of emergency leave/ absent: Life and death situation in immediate family member/s ONLY). Should submit a written letter and necessary documents for approval. All emergency leave/ absent is still subject for approval.,[object Object]

  • 5. Absente should submit an excuse letter.
  • 6. Notice of absence should be 8 hours prior to specified time (schedule).
  • 7. Informing of absence should be through telephone and not through text.
  • 8. Supervisor or Charge Nurse on duty is responsible for receiving telephone call for notice of absence.
  • 9.
  • 10. Sleep/ nap at rooms, hospital bench, and pantry will automatically subject for suspension.
  • 11.
  • 12. Penalty to be given by the hospital administrator/ chief
  • 13.
  • 14. Habitual absences with or without prior notice
  • 15. Those who did not notify absence 8 hours prior to specified time of duty
  • 16. Once the individual proven for the integrity issue, he/she will have an automatic suspension
  • 17.
  • 18.
  • 19. Saturday, Sunday, Holidays, 10-6, and PM shifts: scrub suit
  • 20. IDs should be STRICTLY worn within the hospital premises.
  • 21. FOR FEMALE: hairnet and clinical cap should be worn while on duty
  • 22. FOR MALE: should be properly groomed
