1. SUPRAPUBIC
CYSTOSTOMY

2. SUPRAPUBIC CYSTOSTOMY
-Urinary drainage from the bladder is
established via a catheter inserted through a
suprapubic incision or trocar puncture into
the bladder.

3. INDICATION OF
SUPRAPUBIC CYSTOSTOMY
-If the bladder or urethra is injured by trauma
associated with bony and soft tissue injuries
of the pelvis,
-the bladder may be drained with a catheter
placed above the pubic arch.

4. -This method of drainage is also preferred after
some;
-ureteral,
-bladder,
-prostatic,

5. -urethral surgical procedures
--to decrease tension on sutures,
-ensure a patent route for urinary drainage,
-and minimize urinary retention.

6. -A supra pubic catheter also can be used when
the surgeon wants the patient to void some
urine voluntarily via the urethra to maintain
urethral function and tone,
-such as after bladder or vaginal repairs.

7. -Supra pubic catheters are connected to a
sterile,
closed,
constant drainage system
--before the patient leaves the OR
(operating room).

8. CATHETERS FOR CYSTOSTOMY
DRAINAGE
The following are the most commonly used
catheters for cystostomy drainage:
• Foley, 30-mL balloon, sizes 20, 22, or 24 Fr
• Foley, 5-mL balloon, 24 Fr,

9. -Three-way irrigating catheter
(third lumen can be used for continuous
irrigation)
• suprapubic catheter set

10. -If a biopsy is done, possible equipment needed
for the procedure includes
biopsy forceps,
-which are used for taking tissue specimens,
a Bug bee electrode, which is used to cauterize
bleeders.

11. The Bugbee electrode is
a monopolar,
flexible electrode and
is used in cystoscopes,
resectoscopes and
ureteroscopes.
-The Bugbee
electrode is used for the
ablation coagulation of soft
tissue.

12. -If the urethra is constricted for any reason,
possible equipment needed for the procedure
includes an Otis urethrotome,
-with blade to incise the stricture,
-and van Buren dilators used for dilating.

13. The Otis urethrotome
was routinely used to
incise urethral
strictures.

14. The Van
Buren Urethral Sounds
are used to dilate the
urethra and relieve any
obstructions to
maintain urinary flow.

15. A BRIEF DESCRIPTION OF THE
PROCEDURE FOLLOWS:
1. A sheath and obturator are lubricated and
inserted into the urethra.
2. An obturator is removed and the cystoscope
is inserted.

16. 3. Irrigation tubing is attached, and the bladder
is filled with solution for visualization.
4. A light cord is attached to the fiber optic unit.
