  1. De vijfde eHealth-monitor is daar. Vijf jaar na de eerste monitor krijgen we een steeds beter beeld van de uitdagingen én de kansen die e-health biedt. Bewust keuzes maken over het waarom en voor wie is essentieel voor zinvol gebruik van e-health.
  2. Ik wil u vandaag vertellen over een aantal uitkomsten van de eHealth-monitor. De eHealth-monitor is een grootschalig jaarlijks onderzoek van Nictiz en NIVEL dat sinds 2013 de beschikbaarheid en het gebruik van eHealth in Nederland onderzoekt. Zowel onder zorgverleners als zorggebruikers en in verschillende sectoren. Het onderzoek heeft veel data opgeleverd. Het geeft ons de input uit de praktijk, die ons als expertisecentrum op eHealth de handvatten biedt om uitspraken te doen over het verder implementeren van eHealth. In plaats u te overladen met statistieken, wil ik u over eHealth aan het denken zetten. Dat zal ik proberen aan de hand van … prikkelende stellingen en vragen. Deze ‘prikkels’ zijn niet zozeer conclusies van het onderzoek, maar vinden wél hun oorsprong in de discussie van het onderzoek. Per ‘prikkel’ geef ik u relevante uitkomsten van de monitor.
  3. Prikkels Het onderzoek onderscheidt vijf clusters van toepassing van eHealth: Gemak en service voor zorggebruikers – online contacttoepassingen zoals het e-consult, online afspraken maken, herhaalrecepten digitaal bestellen. Ontsluiting medische gegevens voor de patiënt – online inzage in (delen van) het medisch dossier. Zelfmanagement en online behandeling – zelf meten en bijhouden van gezondheidswaarden, behandeling op afstand zoals eMental-health Begeleiding en ondersteuning op afstand – telemonitoring, beeldbellen Elektronische dossiervoering en communicatie tussen zorgverleners – uitwisselen medische gegevens Afhankelijk van doel(groep) presentatie: Ik richt me vandaag in het bijzonder op het algemene overzicht Voor een totaalbeeld, zal ik uit elk thema een voorbeeld noemen
  4. #1 Gemak en service voor zorggebruikers Over online contact: e-consult, herhaalrecepten, afspraakherinnering, afspraak maken, etc. Trend al langer zichtbaar: iedereen ziet voldoende voordelen, mogelijkheden zijn er, maar gebruik blijft achter. Prikkel #1A: ‘Terechte zorgen of zoeken we naar excuses om niets te doen?’)   Artsen zijn onzeker over veiligheid en wat in kader van privacy wel en niet mag. Ook onduidelijkheid over wanneer wat ingezet mag worden (t.a.v. vergoedingen) en twijfel over eigen competentie (in gebruik met online tools). Ook onder zorggebruikers twijfels over vertrouwen in veiligheid en toegevoegde waarde. Gebrek aan ervaring met techniek lijkt ook te leiden tot twijfel en dus minder gebruik.
  5. Prikkel #1B: ‘Online contact’: 100% aanbod = 100% gebruik? e-consult niet geschikt voor lichamelijk onderzoek en diagnose. Afspraak maken: spoed is telefonisch, anders misschien online (want dan meer keus). Soms is overleg nodig, dan weer bellen. Zorggebruikers en zorgverleners moeten weten voor wie en in welke situaties zij vormen van online contact kunnen inzetten. De vraag is wat een reëel gebruikerspercentage is waar we naar (zouden moeten) streven. Boodschap: online contact is geen doel op zich. Beter kies je er bewust voor waar en wanneer het past.  
  6. #2 Ontsluiting medische gegevens voor de patiënt Wel meer inzage bij medisch specialist en verpleegkundige, niet bij huisarts. Twijfel onder HA hierover lijkt toe te nemen. Opvallend: contrast met zorggebruikers die kennis hierover hebben. Prikkel #2A: Gegevens ontsluiten voor de patiënt: Focus op medicatiedossier! Voorbereiding en nalezen zijn de klassieke argumenten voor online inzage. Maar controle op volledigheid en accuraatheid (medicatieveiligheid) is misschien nog wel belangrijker.
  7. Prikkel 2B: Het doen = Het ervaren Het is van belang de meerwaarde te gaan ervaren. Dat bereik je door te gaan doen en niet door te informeren of uit te breiden. Meer aanbod leidt niet tot meer bekendheid en meer gebruik onder patiënten. Beter zet je in op vergroten van kennis (best practices en argumenten) onder zowel zorggebruikers en zorgverleners. Zoek ambassadeurs, zet ze in. Ervaringen delen met mede zorgverleners of mede zorggebruikers
  8. #3 Zelfmanagement en online behandeling Veel zorggebruikers zoeken informatie over ziekte of behandeling online op. Het meten en bijhouden van gezondheidswaarden onder zorggebruikers blijft nog beperkt, maar bij chronisch zieken en kwetsbare ouderen ligt dit op of boven de 50%. VWS (2014): ‘75% van de chronisch zieken en kwetsbare ouderen die dit willen en hiertoe in staat zijn, voert binnen 5 jaar zelfstandige metingen uit’. Overall: gaat de goede kant op! Aantallen nemen toe. Prikkel 3A: Doelstelling behaald. (En nu?) Er schuilt nog een kans in een (kleine) groep mensen die aangeven niet te meten, maar wél te willen. Die worden nu buiten beschouwing gelaten in de VWS doelstelling Er is ook een (grote!) groep mensen (een derde van de chronisch zieken en kwetsbare ouderen) die wil niet meten. Soms is dat om geldige redenen. Soms niet. Wanneer hier sprake is van relevantie: mogelijke verbetering van kwaliteit van leven en gezondheid kan het van toegevoegde waarde zijn bij die mensen waar het relevant is.
  9. Prikkel 3B: eMental Health: Hoe krijgen we alles weer op een rij?. Focus op: POH’s-GGZ hebben moeite om de juiste tools (beslissingsondersteuning) te matchen bij de patiënt, maar zien wel meerwaarde. Willen ook graag onderzoek naar de effecten. POH’s-GGZ zijn enthousiast, zetten het in bij een deel van hun patiënten, maar hebben wel behoefte aan hulp bij wie wat in te zetten. POH’s-GGZ hebben behoefte aan ondersteuning bij beslissing inzet en informatie over effecten.
  10. NB wanneer het relevant is voor hun specialisme: bij medisch specialisten #4 Begeleiding en ondersteuning op afstand Op meeste onderdelen weinig verandering op e-health toepassingen op dit terrein. Idem op beleidsdoelstellingen van VWS. In ouderenzorg wel toename van toezichthoudende technieken en medicatiedispensers en groei van gebruik computers/tablets in contact met patiënt. Verpleegkundigen staan positief tegenover eHealth-toepassingen en hun rol in het begeleiden van patiënt hierbij. Prikkel 4A: Telemonitoring: het is maar net wat je wilt zien…/ waar wil je naar kijken? Telemonitoring kan leiden tot een vermindering van het aantal bezoeken aan een zorgverlener. Overtuigend bewijs dat telemonitoring leidt tot een betere gezondheid is echter nog maar voor weinig aandoeningen gevonden. Voor diabetes mellitus type 2 en hartfalen is dat in ieder geval wel het geval. Wenselijkheid is aanwezig onder zorgverleners, maar gebruik blijft laag. Eén van de oorzaken hiervan is dat patiënten vaak de noodzaak of persoonlijke meerwaarde van telemonitoring niet inzien. Maar ook om andere belemmeringen: behoefte aan scholing, verder ontwikkelen van telemonitoring, toepasselijkheid en het aanpassen werkprocessen. Lijkt verstandig om hier focus in aan te brengen, om zo meer ervaring op te doen én omdat het wél bewezen effectief is voor patiënten met diabetes type 2 en hartfalen
  11. Prikkel 4B: We moeten werk maken van digitale medicatie-ondersteuning Grootste behoefte van verpleegkundigen, o.a. in de ouderenzorg een wereld te winnen.
  12. #5 Elektronische dossiervoering en communicatie tussen zorgverleners Elektronische dossiervorming steeds brede toegepast in werkprocessen. (Goed) Uitwisselen van gegevens is nodig en belangrijk tussen zorgverleners, zoals bij overdrachtsmomenten. Accuraatheid en actualiteit zijn niet vanzelfsprekend, maar wel belangrijk. Prikkel 5A: Er is nauwelijks een gedeeld, accuraat en actueel patiëntbeeld op lokaal niveau Lang niet overal is digitale gegevensuitwisseling mogelijk. Zonder gegevensuitwisseling mis je cruciale informatie en een actueel en accuraat patiëntbeeld. Dat terwijl de eerstelijnszorg (lokaal/in de buurt) steeds belangrijker wordt. Diversiteit aan ICT systemen lijkt hier een bottleneck.
  13. Alignment / coordination Prikkel 5B: Waar blijven de digitale oplossingen voor verpleegkundigen (in de ouderenzorg)? Overdracht van informatie wordt belangrijk gevonden, ook door verpleging zelf voor betere kwaliteit en afstemming van de zorg.
  14. Prikkel 5C: Luistert naar uw arts! Maak van informatie-uitwisseling een speerpunt bij optimaliseren EPDs Uitwisseling is vice versa Zonder gestandaardiseerde elektronische informatie-uitwisseling sta je er alleen voor Medisch specialisten voelen zich buitengesloten door hun ICT. Artsen zien voordelen van elektronische informatie-uitwisseling. Maar specialisten hebben vaak minder invloed op keuzes ICT-systemen (en zijn minder tevreden over ICT) dan huisartsen. Maar wellicht zijn er meer verklaringen (nader onderzoek nodig). Belangrijk om inzicht te krijgen in ervaringen en knelpunten, om gebruik EPDs te optimaliseren.
  15. Vijf aanbevelingen In het rapport leest u veel gedetailleerder over de specifiek gemeten situatie en een discussie daarop. De monitor doet uiteindelijk, op basis van het wikken en wegen van die onderzoeksresultaten drie (algemene) aanbevelingen die ik u nog wil meegeven Aanbeveling 1: Kies bewust voor eHealth Het lijkt zinnig om eerst goed stil te staan bij de noodzaak en meerwaarde van eHealth en duidelijkheid te krijgen over het waarom. Toepassing moeten relevant zijn voor zorgverlener en zorggebruiker. Voorbeelden zijn de prikkels over het hoe en waarom van online contact, de focus in het waarom en welke gegevens ontsluiten naar de patiënt en het nut en noodzaak van telemonitoring. Denk goed na over zulke vraagstukken, want het kan ertoe leiden dat u sommige dingen niet of anders doet. eHealth is geen doel op zich!
  16. Aanbeveling 2: Bevorder verbetering van digitale gegevensuitwisseling en medicatiecontrole Aan ICT zijde zijn zeker nog stappen te zetten. Het zou helpen wanneer er meer duidelijkheid komt over bijvoorbeeld gebruik van standaarden en verplichtingen ten aanzien van gegevensuitwisseling.   Denk aan de digitale dubbele medicatiecontrole in o.a. ouderenzorg en standaardisatie en betere elektronische uitwisseling tussen verschillende zorgverleners. Kaders helpen de ICT sector   om aan te kunnen sluiten op de behoefte van de (gezamenlijke) zorgverleners. Betere afstemming tussen systemen is nodig, bijvoorbeeld door het invoeren van standaarden.
  17. Aanbeveling 3: Stimuleer ervaring door integratie in curricula, (na)scholing en het aanbieden van demo’s en proeflabs Doen leidt tot ervaring. Ervaring is waardevoller dan informeren. Verdere groei van eHealth lijkt gebaat bij meer praktijk-ervaring en ambassadeurs  leren van peers. (Voorbeelden: ervaring stimuleren tijdens opleiding, bijvoorbeeld door proeflabs, online inzage van patiënten in medisch dossier, maar ook de zorgen die leven bij artsen en zorggebruikers over online diensten – valt het mee in de praktijk?)
  18. Aanbeveling 4: Informeer, communiceer en begeleid We zien dat er nog regelmatig sprake is van onbekendheid en onduidelijkheid, bijvoorbeeld over regels, werkwijzen en richtlijnen voor zorgverleners. Maar ook veel zorggebruikers zijn nog niet op de hoogte van de mogelijkheden op het gebied van eHealth. Goede informatie, communicatie en begeleiding op het juiste moment is nodig voor meer gebruik van eHealth. O.a. de zorgen die artsen en gebruikers hebben over online contact, maar ook t.a.v. gebruik en toepassing van eMental Health door POH-GGZ. En onbekendheid onder zorggebruikers over mogelijkheden en meerwaarde (nv PGO: 36 weet niet of hij hiervan gebruik wil maken).
  19. Aanbeveling 5: Onderzoek good/bad practices Naast het waarom, wat en wie, moeten we verder ontrafelen hoe. Meer onderzoek van praktijkervaringen in wat wel en niet werkt, is nodig om écht te begrijpen wat succesfactoren zijn voor eHealth Zoals blijkt uit behoeften in verpleegkundigen ouderenzorg en POH-GGZ t.a.v. eMental Health: een beter beeld hoe eHealth hier werkt is nodig.     Wanneer we beter zicht hebben op concrete succesfactoren in het praktisch gebruik van eHealth zal dit bijdragen aan verdere implementatie.
  20. Een zwarte dia. Waar is dat goed voor? Deze dia kan uw presentatie redden. Want PowerPoint heeft een nare eigenschap: klikt u een keer te vaak door, dan is de presentatie afgelopen en kunt u niet meer terug. Of er komen witte lettertjes in beeld die zeggen dat uw presentatie is afgelopen. En dan komt uw publiek misschien wel niet meer terug. Nu kunt u wel terug. En gaat uw publiek niet meer weg.
