❖ Risk -Safety quantification: Absolute risk
는 4%로 표기되나 relative risk는 1.5라
는 형식으로 표시되므로 일반인이 이해하
기에는 absolute risk가 용이하다.
❖ Perception of teratogenic risk by women:
depession을 가진 임신부는 25% 임신 중
약제 투여를 중단하고 65%만이 지속한
다. (Anxiety, Lack of knowledge,
Framing, Maternal depression)

❖ Risk communication: 발생율등을 표기할 때 1300/10,000을 26/100 보다 자주 발
생하는 것으로 인식한다.
❖ "Risk"라는 표현은 negative한 면을 강조하는 인식을 주므로 chance나 probability
라는 표현이 더 적합하다.
❖ 결과를 표기할때 bar graph는 risk비교에 유리하며, pie chart는 risk proportion설
명에 좋다.
❖ 같은 분모로 표시하는 것이 이해가 빠르다 (40/1000 vs 5/1000 > 1/25 vs 1/200).
Ratio나 % 보다는 natural number의 사용이 이해하기가 좋다 (100명 중 5명 >
5% ).
❖ Relative risk 로 설명을 해야 하는 경우에는 base line risk 를 꼭 설명하여야 한다.

Financial Benefit of a Typical Teratology Information
❖ Preconception counseling, prohibition of smoking & alcohol, supply of
folic acid & iodine으로 약 25%의 birth defects를 예방할 수 있다.
❖ Depression 환자의 33%가 임신 중 투약 중단  relapsing rate가 60%로
투약군보다 약 2배이다
(110,000 uro/year: preterm birth 에 인한 비용은 포함되지 않음).
❖ Inflammatory bowel disease 는 약 66% 만이 임신 중 투약 유지 (20,000
uro/year: preterm birth 로 인한 비용은 포함되지 않음).
❖ 1.6 million uro/year in Austria / 16 million uro/year in Germany

8. Effects of Intrauterine infection, vaccines and
biologicals(1) Update on teratogenic infections
❖ CMV infection은 10-20%에서 태아에 이상을 일으키는데 ganciclovir는
hear loss의 치료에서 일부의 효과만 나타낸다.
❖ Toxoplasmosis는 1-2%에서 임신 중 감염 시 태아에 이상을 일으키며 10-
24주 이후에 노출될 수록 위험율이 증가한다.
❖ Intrauterine infection은 mental retardation, autistic spectrum disorder(1
세 이후 발병), learning difficulties (school age 에 나타난다), hearing loss
(2-3세에 나타난다).
❖ Influenza의 임신 중 감염은 autistic spectrum disorder & developmental
delay가 2배 증가한다.

9. Effects of Intrauterine infection, vaccines and
biologicals(2) Update on teratogenic infections
❖ 임신 중 subclinical primary CMV감염이 가장 큰 문제로 이는 확실한
intrauterine therapy 도 없으며, 진단도 어렵기 때문이다 (Ig M이 높은 상
태로 1년까지 지속될 수도 있다).
❖ Vaccinations in pregnancy to prevent neonatal infection: 임신부에게 임
신 제 3 삼분기에 pertussis에 대한 vaccination(Tdap)을 하는 것이 권장
된다. 1세 이하의 신생아가 pertussis에 감염되는 경우 pneumonia로 진
행되는 비율이 높다.

❖ ACOG guideline: 임신 27-36 주에 Tdap vaccination을 every pregnancy

❖ 약제의 사용 후에 15년이 지난 지금 일부 연구에서 neonatal cardiac
malformation증가를 보고하고 이에 따라 임상의사 및 환자가 임신 중 약제
투여를 중단한다.
❖ Cold turkey syndrome: 우울증을 앓고 있는 임신부 (20%)가 약제 투여 중
단 후에 안절부절 못하고 우울증의 증상이 심하여 지며 종국에는 자살로
이어진다.
❖ 일반적으로 젋은 여성에서 사망하는 경우의 가장 큰 원인이 자살이며 ,
대부분의 원인이 우울증이다.

❖ Poor neonatal adaptation syndrome: 임신 중 장기간 우울증 약제에 노출
된 태아가 출생 후에 약제 중단으로 인하여 나타나는 증상으로 약 10 -
30%에서 발생한다. 주로 호흡 증상으로 나타나며 약 3-5일 후에 자연 소
실된다.
❖ Postpartum depression 의 가장 나쁜 indication은 임신 중 조절되지 않은
우울증이다.
❖ PPHN은 임신 중 SSRI에 노출된 신생아의 약 1-2%에서 발생하며, 출생
후 적절한 치료로 아직까지 SSRI와 연관된 PPHN으로 사망한 예는 없다.

❖ SSRI를 수유 시 복용하는 경우에는 임신부가 투여 받는 양의 5%미만이
모유로 배출되며, 이는 신생아의 기관이 미숙한 것을 고려하더라도 적은
양에 노출되게 된다.
❖ 우울증을 앓고 있는 산모를 출생 후에 1-2일 이상 충분히 관찰 후에 퇴원
을 시키는 것이 귀가 후에 postpartum depression 이 심하여서 재입원하는
확률을 줄일 수 있다 (Ontrio 주는 출산 후 36시간 이내에 퇴원시킨다).

❖ Flouxectine의 반감기는 2-3day이며 paroxetine의 반감기는 21hr로 출
생 직후 수유 시에는 long half life 약제가 neonatal poor adaptation SD
의 발생을 감소시킨다.
❖ 수유 지속 시에 반감기가 긴 약제가 신생아의 체내에 잔류하는 시간이 길
어지는 것을 우려하여 약제를 반감기가 짧은 것으로 바꾸는 경우가 있는
데, 산모의 우울증을 악화시킬 수 있는 소지가 있으므로 더 위험하다고 할
수 있다.

18. Gerald Briggs Research Symposium: New
frontiers in maternal-fetal toxicology (1)
❖ The placental perfusion model: micro CT- supercomputer 를 이용하여 태
반의 혈관 분포를 3D로 확인하고 혈관의 density를 계산할 수 있다. 
태아의 크기가 클 수록 vv density가 증가한다.

19. Cancer in Pregnancy (1)
❖ Incidence: 0.02-0.1% pregnancy/year
❖ Breast Ca, Thyroid Ca, Hodgkin's and NHL
❖ 임신부에게 CTX하기 위하여 약 54%에서 preterm birth하며 이중 90%
는 iatrogenic하게 발생한다.
❖ CTX를 임신 중 시행하는 경우 12%에서 PTL, 18%에서 SGA가 발생한다
.

20. Cancer in Pregnancy (2)
❖ Breast Ca로 진단 받은 임신부의 23%가 의료진으로부터 termination을
권유받으며, 이중 7%가 실제로 termination을 한다.
❖ 임신 중 CTX를 연기하는 것은 태아를 iatrogenic premature baby 로 만들
며, 임신부의 치료가 delay된다.
❖ 암으로 임신 중절은 prognosis를 개선 시키지 못하며, 임신 중 치료를 적
절하게 하는 경우 예후는 non-pregnant와 유사하다.

21. Cancer in Pregnancy (3)
❖ Alkylating agent: 2nd & 3rd trimester 에 사용하는 경우, congenital
anomaly를 증가시키지 않으나 IUFD는 일부 증가한다.
❖ Platinum: 2nd & 3rd trimester 에 사용한 자료가 없다.
❖ Antimetabolite: MTX를 제외하고는 2nd & 3rd trimester에 사용시에
congenital anomaly를 증가시키지 않는다.

22. Biologics and Other Unpronounceable in
Pregnancy(1)
❖ " Healthy mother is a healthy baby"
❖ TNF alpha inhibitor는 molecular weight가 대부분 크므로 임신 초기에는
태반을 잘 통과하지 않으나 3rd trimester에는 active transport로 태반을
통과한다.
❖ Infliximab (100%), Certolizumab (3%)
❖ Rheumatoid dz 임신부를 제외하고는 임신 30주 후 이후에 약제 투여를
중단하고 분만 후에 다시 투여를 한다.

23. Biologics and Other Unpronounceable in
Pregnancy(2)
❖ Belimumab: SLE에 주로 사용되며, 제약회사에서 registry를 만들었다.
❖ Rituximab: Spontaneous abortion 증가와 neonatal hematologic dz. 의 증
가를 보고하므로 투여 중단 후에 1년 뒤에 임신을 할 것 (?)을 권유.

25. FASD 40 Years Later: The Challenges(1)
❖ FAS (3.1/1000 live birth) + FASD (9.1/1000 live birth) = 1/100 live birth
❖ ARND는 아직까지는 NDPAE (neurobehavioral disorder associated with
prenatal alcohol exposure: DSM V) 의 진단 기준을 완벽하게 충족시키지는
못한다. 주로 minimal exposure 이상의 alcohol에 노출 시에 발생한다.

26. FASD 40 Years Later: The Challenges(2)
❖ Mouse에서 수태 후 7일째 이후에 노출 시에 태자에서 ARND의 발생 위
험율이 가장 높다. 이는 human에서 post-conception 3weeks에 해당된
다.
❖ Post-conception 6 weeks 에 노출 시에 FAS의 특징적인 얼굴 기형 및
microcephaly가 나타난다.
❖ 수정 후 6주에 노출된 경우에 birth length 감소와 연관성이 높다.
❖ 55.9%의 여성이 "drink anytime" 이 중 50.8%가 "binge drink"를 한다
(USA).

27. FASD 40 Years Later: The Challenges(3)
❖ 예방을 위하여 pregnancy test kit
를 술집이 밀집된 곳에 자판기로 판
매를 한다 (Minesota bar, USA).
❖ 현재 national intervention 을 3-10
세의 FASD를 대상으로 시행하고
있으며 약 1년 뒤에 결과가 나온다
.

28. FASD 40 Years Later: The Challenges(4)
❖ Low or moderate 양의 alcohol을 섭취한 임신부에게서 태어난 환아가
IQ가 더 높은 경향을 보이는 데, 이 이유는 아직 알 수 없다.
❖ Preliminary한 data이기는 하나 mid-pregnancy에 임신부에게 multiple
vitamin을 투여한 결과, 신생아에서 1세때 alcohol-associated neuro-cognitive
performance 에 protective effect 를 보였다.

29. FASD 40 Years Later: The Challenges(5)
❖ ADH1A: expressed in 1st trimester of fetal life, less active later in
gestation
❖ ADH1B: active in liver from 2nd trimester and gradually increases in
activity
❖ ADH4: expressed in liver from 2nd trimester, important at intoxicating
levels
❖ ADH7: expressed in stomach mucosa, act early to metabolize alcohol
before it reaches the blood

30. FASD 40 Years Later: The Challenges(6)
❖ Rare alleles at ADH1a rs2866151 & rs975833
❖ A rare allele at ADH1B rs4147536
❖ A rare allele at ADH7 rs284779
❖ 위의 3가지 alcohol metabolizing gene 에 태아가 genetic variant가 있는 경우에 8세
에 IQ감소와 연관이 있다.
❖ 이 현상은 태아가 임신 중 moderate (1-6 units of alcohol/week) 이상의 alcohol에 노
출된 경우에 나타난다.
❖ 임신부가 ADH4 rs148884에 genetic variant가 있는 경우에도 8세에 IQ감소를 보인
다.
