Details Summary Last Saturday, 10 March 2002, eight of us were playing a game of basketball at the basketball court when Tan Wei Mun fell over and broke his arm in the course of the game. On Monday, 28 June 2009, a group of boys were crossing Siglap Link when Derek Siou tripped over his basketball and had a head on collision when a stationary car leaving a huge dent in the car.

Events leading to Incident We had finished our Co-Curricular Activity (CCA), the National Cadet Corps (NCC). Instead of going straight home, we decided to play basketball with some of our friends. The time was about 12.30 p.m. An altercation occurred earlier in the morning when Hanif and another boy Philip Loo, fought over one of the single chairs at the library during the reading period. Then Hanif argued with Derek Wee over borrowing of a book. Mrs Rama also remonstrated with Hanif over his failure to hand in his Reading Workbook for the sixth week.

Main Events As we were lining up to leave the library at 10.10 a.m., Hanif pushed his way to the top of the line only to find that Philip and Derek were in front. As he approached, Philip asked him where he was going and Hanif said that he wanted to get to the Chicken rice stall first. A commotion ensued involving shoving, grunting, hissing and then a punch was thrown, causing Hanif to lose his balance and fall into the pond.
Main Events The game had been quite normal up to the moment when Wei Mun leaped close to the basket to try to score. However, the ball bounced off the ring and the three boys who were nearby tussled with Wei Mun to grab the ball. Tan Sin Min, John Lee and Chan CheeKuan all fell to the ground together with poor Wun. Standing about three metres away, I had a good view of what happened. No one pushed Wei Mun or deliberately tried to knock him over. One moment, they were all reaching up for the ball, the next, they were all lying on the ground. Unfortunately, Wei Mun was at the bottom of the pile.

Immediate Follow-up For a second or two, a mixture of giggles and laughter mixed with groans and complains were heard. Then just when all the other voices subsided, the painful wailing continued. We all sensed immediately that something serious had happened when we saw Wei Mun still lying on the floor after the rest had gotten up.

Actions undertaken At first we all crowded around him. Then I realised that someone should get one of the teachers and I sent the fastest runner among us, MohdSharjun, to the office while the rest of us tried to give our injured friend what help we could. I also got everyone to move away from Wei Mun, and give him room to breathe because we all crowded too close to see what had happened.
Paragraphs Six Opinion of who was to be blamed What should and should not have been done
Conclusion and Opinions I feel that we were equally at fault in that we should have gone straight home. However, I do not think any one of us in particular should be blamed for the fall as it was just an unfortunate accident. I feel that although we were all involved in playing catch, it was Derek who  recommenced the game in street, took it onto the road and dinted the man’s car with his head.
Aspects to note Each paragraph should indicate a new idea. Use consistent past tense in report except the last paragraph. Be specific like giving full names where possible. Do not use acronyms or short form for a proper noun if you mention it for the first time. Discourse indicators must be used at the beginning of each paragraph to signal what is to follow to the reader
