Mechanical hazards
Machinery, its parts, tools, objects and materials processed or
used in the work process are often a source of mechanical hazards leading
to injuries. Mechanical hazards are created by the powered operation of
apparatus or tools. The applied power may be electrical or human. Tools or
apparatushave three locationswhere mechanical hazardscanexist:
 the pointof operation
 the pointof powertransmission
 the area of movingparts

Electrical hazards
Machinery, along with power supply systems, can also create
electrical hazards, leading to severe or fatal accidents caused by
electric shocks. All of the following are electrically energized when
the power is “on”: electrical systems of machines, internal & external circuits, input
power exposed wires, and all other metal parts. Additionally, incorrectly installed or
improperly grounded equipment is a hazard. Electrical hazards are created by the
electricallypoweredoperationof apparatusortoolswhichcan be identified:
 shortcircuitor groundedwiringonamachine
 electricpowertransmission
 exposedwiring(livedwire)
Chemical hazards
A chemical hazard arises from contamination of an area
with harmful or potentially harmful chemicals. Possible sources of
chemical hazards include the burning of fossils, materials and
chemicals used in construction and industry, pollution of the
environment and water supply, chemical spillages, industrial
accidents,andthe deliberate release of toxicmaterials.
Environmental hazards
Environmental hazard' is a generic term for any situation
or state of events which poses a threat to the surrounding
environment. This term incorporates topics like pollution and
natural disasterssuchas stormsand earthquakes.

Healthhazards/ BioHazard
Biological hazards, also known as biohazards, refer to
biological substances that pose a threat to the health of living
organisms, primarily that of humans. Which can be inhaled,
swallowed or contact with the skin. The term and its associated
symbol is generally used as a warning, so that those potentially
exposedtothe substanceswillknowtotake precautions.

What to do nowas a DeputizedInspector?
 Ensure that workers use or wear the equipment, protective devices or clothing that the
companyrequirestobe usedor wornand that it isingood condition.
 Ensure that workers receive appropriate training to use or wear the equipment, protective
devicesorclothingthatthe companyrequires.
 Establishproceduresandpractices toensure thatworkerscancarry out safe andhealthywork.
 Ensure that workerscomplywiththe company’ssafetypolicyandprogram.
 Ensure that workers are aware of potential hazards and have dealt with, or are dealing with, the
actual hazardsinthe workplace.
 Planand communicate workassignmentstoenable workerstoproduce safety.
 Involve workersinworkplanningandproblemsolving.
 Provide orientationtonewcrew members.
 Conductweeklysafetytalksandweeklysiteinspections.

(JobDescription)
1. Be responsible foron-site accidentprevention.
2. Ensure that protective equipment,safetymaterials,andfirstaidsuppliesare provided.
3. Instructdeputizedinspectorsinthe workpracticesrequiredbythe company’ssafetypolicies
and bythe program,and ensure thattheyare followed.
4. Monitorthe healthandsafetyperformance of all departments,includingsubcontractors.
5. Reportaccidentsandinjuriestoauthorities/seniormanagementasrequiredbythe programand
regulations.
6. Investigate accidents(withforeperson)andtake actionto preventrecurrence.
7. Monitorsafetybehaviorandperformance of forepersons,crews,andsubcontractors.
DEPUTIZED SAFETY INSPECTOR
(JobDescription)
1. Inspectthe workplace byensuringthe programiscarriedoutat the worklevel.
2. Ensure that PPEsrequiredbythe program isusedand maintainedproperlybyworkersandthat
workersunderstandthe reasonsforitsuse.
3. Checkworkpracticesand workareas for hazardsand take corrective action.
4. Consult/coordinate all healthandsafety issuestothe SafetyInspector.
5. Ensure that injuriesare treatedandreported.
6. Identifysituationsthatmaybe a source of danger.
7. Assistinconductinginvestigationsandreportall accidentsandtake corrective action.
8. Acquaintthe newworkerwith hazardsandsafe workproceduresbyconductingToolbox
meetings.

DOCUMENTATION
Qualitydocumentationisamustin the Safety.Essentially,accurate documentationisnecessarywherea
physical processoractivityisconveyedthroughthe mediumof writingbyhavingitnoteddownina
documentof some kind.
Thisis a necessitydue toseveral reasons:
• It givesushistory
• Documentsto review
• Evidence forfollowupinvestigation
• It provesthat we are workingproperly
• Traceabilityof all events
The importantpart of managingsafetyistomake sure that we alwayshave clearprecise documentation
of everysingle importantevents,process,andactionthattakesplace.Thiswill allow youtohave a clear
reference pointwheneveranythingisinquestion.
Properdocumentationshallbe done andmaintainedregularly,safe keeping,storage and filedaccording
to itscategory.

What isa Toolbox Meeting?
Toolbox meetingsare shortsafetymeetingscoveringa specificsafetytopic or type of hazard.
Commontopicsforthese meetingsinclude one ormore of the following:
1. Hazards of a jobthat is about to be started.
2. Causesandcorrective actionfor a recentaccidentor “nearmiss.”
3. Jobhazards that shouldbe emphasizedalongwithsafetycontrolsthatwill prevent
accidents.
4. General problemsconcernedwithbothon-jobandoff-jobsafety.
5. Seasonal safetyinformation
How large shouldaToolbox SafetyMetingbe?
The ideal size fora toolbox safetymeetingis6to 10 workers. Larger groupscan be used,
however,the workerswill be lesslikelytoenterintoanydiscussions.
How longshouldtheylast?
A toolbox safetymeetingnormallylastsapproximately10minutesif workersare notseated,if
the topicis well chosen,andif the discussiondoesnotgetoff ona tangent. Inpreparingfora
meeting,anditappearsthatthe interestwilllastmore than10 minutes,consideraseriesof 3 or 4
meetingsorholdthe meetinginaroomwhere the workerscan sitcomfortable andwhere noise and
otherdistractionsare absent.
How shouldaToolbox Meetingbe conducted?
1. As a deputizedsafetyinspector,itisagoldenrule thatyouare alwayspreparedonany toolbox
meetingsthatyouwill conduct.
2. Be sure that youwill have acopy of the toolbox topictobe discussed.
3. Start on time.
4. Keepthe meetingfromgoingoutof the topic.
5. Askquestionsaboutthe topictogenerate discussion.Allow time fordiscussionandquestions. If
youdon’tknowthe correct answer,don’tguessatit. Askforhelpfromthe AuthorizedSafety
Inspector/SafetySuperintendent/SafetyCoordinator.
6. Pointoutthingsthat are beingdone right aswell asproblemareas.
7. Use a real accidentcase to emphasize apoint. The more recentandthe more close
(geographically) toyourlocation,the more effective the examplewill be.Personal experiences
are usuallythe bestexample.

What isa SpotReport?
A spotreportis a written,narrative reportthatinformsthe higherofficeaboutthe accident/
incidentthathappened.
It will answer the followingquestions:
 What accident/ incidenttookplace?(Type of Accident/Incident)
 What Date and Time ithappened?
 Where didit happened?(Location)
 Who or What is involved? (person/equipment/environment)
 What is the Scenarioduringthe time of accident?(aShort story)
 Who are the witnesses?(If applicable)
GENERAL RULES:
1. Provide one (1) copyof thisreportto the Head Office within24 hoursafterthe incident.
2. A detailedaccident/incidentreportshall follow afterthe investigationwithin72hours.
3. Indicate time &date of accident,name of personor propertyinvolved,andnarrate the incident.

ACCIDENT/INCIDENT
REPORT
A rule of thumbin case of an accident/ incident:
FrasecVenturesCorporationrequiresall employeestoimmediatelyreport all accidentsand
incidentsresultingtoaninjuryorpropertydamage,andall near misseswiththe potentialforserious
injuryor propertydamage.Thisreportshall be informedtotheirsupervisor,immediatesuperior,safety
inspectorsforfurtheractionor emergencyresponse asneeded.
Each incidentshall undergoathoroughanalysistodetermine causesandcontributingfactors
and the analysiswill be usedtoreduce oreliminatethe riskof future incidents.
As a deputizedsafetyinspector,itisyour dutyto complythe requiredaccident/accidentreport
inorder to documentsuchevent.Thisreportwill serveasa guide forthe managementtoreview such
accident/incidentinordertodevelopsolutions,reviseprocedures,create apolicyforthe purpose of
preventingthe occurrence of suchaccident/incident.
What shouldyoureportand investigateincase anaccident/incidentoccurs?
FVCrequiresthatinjuriesandcertaincategoriesof accidentsmustbe reported.Suchreport
shouldspecifythe following:
 what isto be reported
 to whomit will be reported
 how itis reported
 whichincidentsare investigated

 what traininginvestigatorswill receive
 what recordsare to be kept
 what summariesand statisticsare tobe developed
 how oftenreportsare prepared
Accidentsandincidentsare investigatedsothatcorrective actionswill be providedforthe
purpose of preventingarecurrence of asimilarevent.Investigationrepresentsan"after-the-fact"
reactionforany particularmishap.
However,underathoroughinvestigationconductedbyateam of inspectors&safetycommittee
members,we mayuncoverhazards,potential risksorproblemsthatcan be eliminated"before-the-fact"
for the future.
Afteridentifyingthe causes,apromptfollow-upactionisrequiredtoachieve orcompletethe
purpose of the investigation.

Mechanical hazards are created by the powered operation of apparatus or tools. The applied power may be electrical or human. All of the following are electrically energized when the power is “on”: electrical systems of machines, internal & external circuits, input power exposed wires, and all other metal parts. Additionally, incorrectly installed or improperly grounded equipment is a hazard. Electrical hazards are created by the electricallypoweredoperationof apparatusortoolswhichcan be identified:  shortcircuitor groundedwiringonamachine  electricpowertransmission  exposedwiring(livedwire) Chemical hazards A chemical hazard arises from contamination of an area with harmful or potentially harmful chemicals. Possible sources of chemical hazards include the burning of fossils, materials and chemicals used in construction and industry, pollution of the environment and water supply, chemical spillages, industrial accidents,andthe deliberate release of toxicmaterials. Environmental hazards Environmental hazard' is a generic term for any situation or state of events which poses a threat to the surrounding environment. This term incorporates topics like pollution and natural disasterssuchas stormsand earthquakes.  Soil erosion/LandSlides  Fallingrocks/ Hangingwalls  Heavyrain causingFlood(Storm)  Earthquake  Quicksand / clayeysurface /swamp Healthhazards/ BioHazard Biological hazards, also known as biohazards, refer to biological substances that pose a threat to the health of living organisms, primarily that of humans. Which can be inhaled, swallowed or contact with the skin. The term and its associated symbol is generally used as a warning, so that those potentially exposedtothe substanceswillknowtotake precautions.

Health and Safety Department Prepared by Maximo Ginez III Felizardo Sarador Safety Coordinator Safety & Health Superintendent What to do nowas a DeputizedInspector?  Ensure that workers use or wear the equipment, protective devices or clothing that the companyrequirestobe usedor wornand that it isingood condition.  Ensure that workers receive appropriate training to use or wear the equipment, protective devicesorclothingthatthe companyrequires.  Establishproceduresandpractices toensure thatworkerscancarry out safe andhealthywork.  Ensure that workerscomplywiththe company’ssafetypolicyandprogram.  Ensure that workers are aware of potential hazards and have dealt with, or are dealing with, the actual hazardsinthe workplace.  Planand communicate workassignmentstoenable workerstoproduce safety.  Involve workersinworkplanningandproblemsolving.  Provide orientationtonewcrew members.  Conductweeklysafetytalksandweeklysiteinspections.  Reviewsafetyaspects of eachtaskwithcrew.  Conductaccidentor incidentinvestigation.  Encourage workersto reporthealthandsafetyproblems  Respond quickly and appropriately to worker concerns and cooperate in their correction. Be responsible foron-site accidentprevention. 2. Ensure that protective equipment,safetymaterials,andfirstaidsuppliesare provided. 3. Instructdeputizedinspectorsinthe workpracticesrequiredbythe company’ssafetypolicies and bythe program,and ensure thattheyare followed. 4. Monitorthe healthandsafetyperformance of all departments,includingsubcontractors. 5. Reportaccidentsandinjuriestoauthorities/seniormanagementasrequiredbythe programand regulations. 6. Investigate accidents(withforeperson)andtake actionto preventrecurrence. 7. Monitorsafetybehaviorandperformance of forepersons,crews,andsubcontractors. DEPUTIZED SAFETY INSPECTOR (JobDescription) 1. Inspectthe workplace byensuringthe programiscarriedoutat the worklevel. 2. Ensure that PPEsrequiredbythe program isusedand maintainedproperlybyworkersandthat workersunderstandthe reasonsforitsuse. 3. Checkworkpracticesand workareas for hazardsand take corrective action. 4. Consult/coordinate all healthandsafety issuestothe SafetyInspector. 5. Ensure that injuriesare treatedandreported. 6. Identifysituationsthatmaybe a source of danger. 7. Assistinconductinginvestigationsandreportall accidentsandtake corrective action. 8. Acquaintthe newworkerwith hazardsandsafe workproceduresbyconductingToolbox meetings. MODULE 2-B DOCUMENTATION Qualitydocumentationisamustin the Safety.Essentially,accurate documentationisnecessarywherea physical processoractivityisconveyedthroughthe mediumof writingbyhavingitnoteddownina documentof some kind. Thisis a necessitydue toseveral reasons: • It givesushistory • Documentsto review • Evidence forfollowupinvestigation • It provesthat we are workingproperly • Traceabilityof all events The importantpart of managingsafetyistomake sure that we alwayshave clearprecise documentation of everysingle importantevents,process,andactionthattakesplace.Thiswill allow youtohave a clear reference pointwheneveranythingisinquestion. Properdocumentationshallbe done andmaintainedregularly,safe keeping,storage and filedaccording to itscategory.

Toolbox meetingsare shortsafetymeetingscoveringa specificsafetytopic or type of hazard. Commontopicsforthese meetingsinclude one ormore of the following: 1. Hazards of a jobthat is about to be started. 2. Causesandcorrective actionfor a recentaccidentor “nearmiss.” 3. Jobhazards that shouldbe emphasizedalongwithsafetycontrolsthatwill prevent accidents. 4. General problemsconcernedwithbothon-jobandoff-jobsafety. 5. Seasonal safetyinformation How large shouldaToolbox SafetyMetingbe? The ideal size fora toolbox safetymeetingis6to 10 workers. Larger groupscan be used, however,the workerswill be lesslikelytoenterintoanydiscussions. How longshouldtheylast? A toolbox safetymeetingnormallylastsapproximately10minutesif workersare notseated,if the topicis well chosen,andif the discussiondoesnotgetoff ona tangent. Inpreparingfora meeting,anditappearsthatthe interestwilllastmore than10 minutes,consideraseriesof 3 or 4 meetingsorholdthe meetinginaroomwhere the workerscan sitcomfortable andwhere noise and otherdistractionsare absent. How shouldaToolbox Meetingbe conducted? 1. As a deputizedsafetyinspector,itisagoldenrule thatyouare alwayspreparedonany toolbox meetingsthatyouwill conduct. 2. Be sure that youwill have acopy of the toolbox topictobe discussed. 3. Start on time. 4. Keepthe meetingfromgoingoutof the topic. 5. Askquestionsaboutthe topictogenerate discussion.Allow time fordiscussionandquestions. If youdon’tknowthe correct answer,don’tguessatit. Askforhelpfromthe AuthorizedSafety Inspector/SafetySuperintendent/SafetyCoordinator. 6. Pointoutthingsthat are beingdone right aswell asproblemareas. 7. Use a real accidentcase to emphasize apoint. The more recentandthe more close (geographically) toyourlocation,the more effective the examplewill be.Personal experiences are usuallythe bestexample. It will answer the followingquestions:  What accident/ incidenttookplace?(Type of Accident/Incident)  What Date and Time ithappened?  Where didit happened?(Location)  Who or What is involved? (person/equipment/environment)  What is the Scenarioduringthe time of accident?(aShort story)  Who are the witnesses?(If applicable) GENERAL RULES: 1. Provide one (1) copyof thisreportto the Head Office within24 hoursafterthe incident. 2. A detailedaccident/incidentreportshall follow afterthe investigationwithin72hours. 3. Indicate time &date of accident,name of personor propertyinvolved,andnarrate the incident.

Each incidentshall undergoathoroughanalysistodetermine causesandcontributingfactors and the analysiswill be usedtoreduce oreliminatethe riskof future incidents. As a deputizedsafetyinspector,itisyour dutyto complythe requiredaccident/accidentreport inorder to documentsuchevent.Thisreportwill serveasa guide forthe managementtoreview such accident/incidentinordertodevelopsolutions,reviseprocedures,create apolicyforthe purpose of preventingthe occurrence of suchaccident/incident. What shouldyoureportand investigateincase anaccident/incidentoccurs? However,underathoroughinvestigationconductedbyateam of inspectors&safetycommittee members,we mayuncoverhazards,potential risksorproblemsthatcan be eliminated"before-the-fact" for the future.
