2. Introduction:
The Capiz Community Healthcare Center Services, Incorporated is a domestic stock corporation existing
by and under the laws of the Republic of the Philippines, duly registered with the Securities and Exchange
Commission, and with principal offices and place of business at Capiz Doctors Hospital, Fuentes Drive,
Roxas City, Capiz. It consists of healthcare professionals such as physicians, nurses, caregivers,
midwives, and allied professionals such as lawyers and accountants.
These professionals have specific areas of expertise and they apply their pertinent knowledge, skills and
competencies to deliver personalized care and specialized services to patients who have recently been
discharged from the hospital, are chronically ill, or are requiring post-operative care.
Patients and their families may access the healthcare professions through the Capiz Community
Healthcare Services Office, where their needs will be assessed prior to assignment and deployment of the
healthcare staff to their homes.
In the future, the company hopes to provide these clients a wider range of additional healthcare services
such as Ambulance Services, Laboratory Testing, Clinic Services, and Health Insurance Coverage.
BRIEF HISTORICAL BACKGROUND:
Three of the founders of Capiz Community Health Care Services are medical health professionals. In the
course of their work, they saw the need for post- operative and chronically ill patients to be provided with
continuous medical care at home after hospital discharge. It is a dilemma for the patient’s families as to
who will provide care for their patients at home especially when the patient would require special
procedures such as NGT Feedings and wound dressings.
Their advocacy to provide professional health care services to the people of Capiz became a reality
through their group project. The Capiz Community Health Care Services was established in response to
the need of the Capizeños for affordable and quality healthcare services for home-bound patients and their
families. The organization also aims to create more jobs for healthcare professionals, providing them a
lucrative source of income while practicing their craft and improving their knowledge and skills.
By providing such services to the community, the founders have pioneered the provision of home
healthcare services in the province of Capiz, and have carved a lucrative niche in conducting their
business.
