Introduction:
The Capiz Community Healthcare Center Services, Incorporated is a domestic stock corporation existing
by and under the laws of the Republic of the Philippines, duly registered with the Securities and Exchange
Commission, and with principal offices and place of business at Capiz Doctors Hospital, Fuentes Drive,
Roxas City, Capiz. It consists of healthcare professionals such as physicians, nurses, caregivers,
midwives, and allied professionals such as lawyers and accountants.
These professionals have specific areas of expertise and they apply their pertinent knowledge, skills and
competencies to deliver personalized care and specialized services to patients who have recently been
discharged from the hospital, are chronically ill, or are requiring post-operative care.
Patients and their families may access the healthcare professions through the Capiz Community
Healthcare Services Office, where their needs will be assessed prior to assignment and deployment of the
healthcare staff to their homes.
In the future, the company hopes to provide these clients a wider range of additional healthcare services
such as Ambulance Services, Laboratory Testing, Clinic Services, and Health Insurance Coverage.
BRIEF HISTORICAL BACKGROUND:
Three of the founders of Capiz Community Health Care Services are medical health professionals. In the
course of their work, they saw the need for post- operative and chronically ill patients to be provided with
continuous medical care at home after hospital discharge. It is a dilemma for the patient’s families as to
who will provide care for their patients at home especially when the patient would require special
procedures such as NGT Feedings and wound dressings.
Their advocacy to provide professional health care services to the people of Capiz became a reality
through their group project. The Capiz Community Health Care Services was established in response to
the need of the Capizeños for affordable and quality healthcare services for home-bound patients and their
families. The organization also aims to create more jobs for healthcare professionals, providing them a
lucrative source of income while practicing their craft and improving their knowledge and skills.
By providing such services to the community, the founders have pioneered the provision of home
healthcare services in the province of Capiz, and have carved a lucrative niche in conducting their
business.

STAFF
1. BOARD OF DIRECTORS
Qualification Standard:
 His/her profession must be related to healthcare or related business
 Must own at least one share of the common stock of the corporation
Job Description:
 Creates policies and guidelines for the organization
 Provides directions for managing the corporation
 Prepares plans for approval of shareholders/incorporators
 Reviews long and medium range plans
2. CHIEF EXECUTIVE OFFICER/GENERAL MANAGER
Qualification Standard:
 Must be a licensed medical doctor
 Must have had at least 10 years of current active medical practice
 Must have held a supervisory or managerial position for at least 3 years
 Must have a Master’s degree in Management or Business Administration
Job Description:
 Implements policies and guidelines created by the BOD
 Oversees operations of the company
 Assigns tasks to supervisors
 Determines which department handles specific jobs
3. LEGAL COUNSEL
Qualification Standard:
 Must be a lawyer
 Must have 5 years of experience in active legal practice with a strong background in corporate
law

 Provides legal advice
 Reviews policies formulated by the BOD
 Represents the corporation in all legal matters
 Act as Corporate Secretary
 Prepares and reviews documents and contract
4. HUMAN RESOURCE DIRECTOR
Qualification Standard:
 Must have a degree in Psychology
 Must have educational background in management or business administration
 Must have at least 3 years of relevant human resource experience in the field of health care

5. HUMAN RESOURCE STAFF
Qualification Standard:
 Must be a graduate of any 4-year business course
 Must have at least 1 year of clerical experience

 Answers calls and prepare reports for jobs to GM for distribution to divisions

7. SUPERVISORS
Qualification Standard:
 Must be a licensed registered Nurse
 Must have at least 5 years of clinical practice in a specialty field (Pediatrics, OB-GYN, Internal
Medicine, Surgery)
 Must have held a supervisory or managerial position for at least 2 years in a healthcare facility
Job Description:
 Supervising the work performed by nurses, midwives and caregivers
 Determining client’s needs and assigning appropriate healthcare providers based on their
qualifications to meet specific client’s needs
 Creating staff schedules
 Reviewing client feedback, Evaluating performance of healthcare providers, and providing
constructive feedback to staff
 Ensuring that staff are up to date on latest developments in healthcare, and are providing safe,
responsive care by conducting orientations, trainings, and return demonstrations
8. NURSES
Qualification Standard:
 Must be a licensed Registered Nurse
 Must have at least 1 year of clinical experience with exposure in any of the following areas:
Pediatrics, Internal Medicine, Surgery and OB-GYN
Job Description:
 Monitoring Vital Signs, Intake and Output
 Administering Nursing Care, performing dependent and independent nursing care
 Administering medications
 Carrying-out doctor’s orders
 Performing bedside care, essential new born care, and perineal care as needed
 Performing dressing changes or operative site care as needed
 Performing oral, nasogastric, enteral or parenteral feedings as needed
 Provide health education to clients and their families
 Caring for and maintaining contraptions and sites such as colostomies, gastric tubes, and the like
 Caring for medical equipment used by clients
9. MIDWIVES
Qualification Standard:
 Must be a licensed Registered Midwife
 Must have at least 2 years of clinical experience with exposure in the areas of Pediatrics or OB-
GYN

 Provide breastfeeding support for new mothers
 Educating new mothers on care for their newborns
 Performing dressing changes on operative sites as needed
10. CAREGIVERS

 Must have at least 2 years of clinical experience with exposure in the following areas: Pediatrics,
OB-GYN, Internal Medicine and Surgery

Introduction: The Capiz Community Healthcare Center Services, Incorporated is a domestic stock corporation existing by and under the laws of the Republic of the Philippines, duly registered with the Securities and Exchange Commission, and with principal offices and place of business at Capiz Doctors Hospital, Fuentes Drive, Roxas City, Capiz. It consists of healthcare professionals such as physicians, nurses, caregivers, midwives, and allied professionals such as lawyers and accountants. These professionals have specific areas of expertise and they apply their pertinent knowledge, skills and competencies to deliver personalized care and specialized services to patients who have recently been discharged from the hospital, are chronically ill, or are requiring post-operative care. Patients and their families may access the healthcare professions through the Capiz Community Healthcare Services Office, where their needs will be assessed prior to assignment and deployment of the healthcare staff to their homes. In the future, the company hopes to provide these clients a wider range of additional healthcare services such as Ambulance Services, Laboratory Testing, Clinic Services, and Health Insurance Coverage. BRIEF HISTORICAL BACKGROUND: Three of the founders of Capiz Community Health Care Services are medical health professionals. In the course of their work, they saw the need for post- operative and chronically ill patients to be provided with continuous medical care at home after hospital discharge. It is a dilemma for the patient’s families as to who will provide care for their patients at home especially when the patient would require special procedures such as NGT Feedings and wound dressings. Their advocacy to provide professional health care services to the people of Capiz became a reality through their group project. The Capiz Community Health Care Services was established in response to the need of the Capizeños for affordable and quality healthcare services for home-bound patients and their families. The organization also aims to create more jobs for healthcare professionals, providing them a lucrative source of income while practicing their craft and improving their knowledge and skills. By providing such services to the community, the founders have pioneered the provision of home healthcare services in the province of Capiz, and have carved a lucrative niche in conducting their business. BOARD OF DIRECTORS Qualification Standard:  His/her profession must be related to healthcare or related business  Must own at least one share of the common stock of the corporation Job Description:  Creates policies and guidelines for the organization  Provides directions for managing the corporation  Prepares plans for approval of shareholders/incorporators  Reviews long and medium range plans 2. CHIEF EXECUTIVE OFFICER/GENERAL MANAGER Qualification Standard:  Must be a licensed medical doctor  Must have had at least 10 years of current active medical practice  Must have held a supervisory or managerial position for at least 3 years  Must have a Master’s degree in Management or Business Administration Job Description:  Implements policies and guidelines created by the BOD  Oversees operations of the company  Assigns tasks to supervisors  Determines which department handles specific jobs 3.
