8 Substance Abuse Counseling and Co-occurring Disorders CHAPTER OBJECTIVES After reading this chapter, you will be able to: 1. Recognize substance dependence and substance abuse. 2. Know key diagnoses and definitions from the DSM-IV-TR. 3. Be aware of the various co-occurring disorders that are common to substance abusers. 4. Understand the various screening and assessment tools that are used in the treatment of substance abuse disorders. 5. Know the 12 core functions associated with substance abuse treatment. 6. Be aware of the impact that denial has on the addicted population’s prognosis. 7. Understand the dynamics of relapse prevention. INTRODUCTION The prevalence of offenders suffering from substance use and abuse problems currently in the American Criminal Justice System is staggering. The massive increase in the number of convicted offenders suffering from substance abuse began in the 1980s and continues through the present. As Hanser (2006) points out, any informed discussion of drug offenders in the United States must begin with the war declared on drugs by the U.S. Government. As crack cocaine began to sweep through the nation in the early to mid-1980s an outcry shivered through the fabric of our society. Not only was the drug trade burgeoning and access to illegal substances becoming easier than ever, the violent crime rate was also increasing. A connection was quickly made between the expanding drug culture and the often violent incidents that occurred within its realm. This connection, along with societal upheaval, forced the government to take action in an attempt to rid ourselves from the evils and perils commonly associated with substance abuse and criminal behavior. The resulting action taken by federal and state lawmakers has been to draft laws aimed at corralling illegal substance–using offenders. And, law enforcement efforts have been somewhat successful—successful at least in its ability to arrest a sufficient amount of drug-related offenders so that nearly every correctional agency in America is at or beyond capacity. Once drafted these laws are enforced. In order to be enforced assets must be well equipped and mobilized. What is the primary ingredient for equipping and mobilizing assets? Money. As a country we have spent enormous amounts of money in attempt to halt the flow and usage of illegal substances. The money has primarily gone to two components of the criminal justice system: enforcement and corrections. Enforcement efforts are usually aimed at stopping the flow of illegal substances from entering our country; arresting those transporting and distributing illegal substances after they have entered the country; as well as, arresting those found to be using illegal substances. Enforcement efforts are carried out by a multitude of law enforcement agencies ranging from federal to state and local jurisdictions. America is one of the countries facing the misuse and addition of drugs and other illicit substances. Not only in America, but drug and substance abuse have been a relevant menace globally and require an immediate response. As a result, different governments have engaged in finding efficient mechanisms to evaluate and determine the causes and implement strategic interventions to minimize and control the use, distribution, and sale of drug and substance abuse across the globe. Ideally, the United States of America enacted the Controlled Substance Act (CSA) of 1970, a drug policy controlling and regulating addictive and illegal drugs such as narcotics and marijuana. Thus, the Controlled Substance Act represents the federal American government policy primarily on drugs. The drug policy is not only a multiplex but an intersection for a better lifestyle among Americans. The current drug policy in America prevents and controls the use, distribution, and sale of stimulants, hallucinogenic substances, and depressants that people might abuse. Besides, the drug policy aids in determining if the contents in these drugs possess any medicinal purpose. Notably, to remain effective in its goal, the Controlled Substance Act regulates and classifies drugs based on their possible abuse, medicinal benefits, and the harm they might impose on users. Narcotics and drugs are categorized in various classes and subclasses and scheduled depending on their abuse potential. Based on this drug classification, the high schedule implies strict laws in distributing and acquiring drugs. Several amendments have been made to the drug policy to change medications and pass laws in all states. Again, the Controlled Substance Act enhances intensive research to prevent drug reliance and substance abuse. Multiple procedures and frameworks for proper treatment and rehabilitation for drug addicts are well-defined in the act. Apart from the above-mentioned strengths, several weaknesses are attributable to the drug policy. For instance, the Controlled Substance Act restricted the access and sale to controlled substances among entities legal and registered to manufacture, distribute and sell the products. While the drug policy limits access to controlled substances, there is a registration exemption. Reduced findings on drug use and substance abuse and rehabilitated addicts would show the effectiveness of the drug policy. The paper will critically analyze the Controlled Substance Act of 1970 as one of the paramount drug policies in America. Various factors are noted to cause substance abuse. Some of these factors include genetic predisposition, drug use experimentation, concurring conditions and the environment (Benito, Ballesteros, Callado & Meana, 2014). Drugs can consist of a variety of substances. Some are legal although most are not. Abusing legal or illegal drugs is harmful to the human body and leads to physical, mental, psychological, or social harm to an individual or group. Some individuals use the excuse of using drugs because they are depressed, stressed, in pain or any other reason the person can create to excuse his behavior and drug abuse (Brick & Erickson, 2013). The world today does not realize that most people are all habitual drug users because they are continually ingesting nicotine, ethanol, and caffeine. Even simple medicines can be counted. There are different groups of drugs; Soft drugs that include tranquilizers, sedatives, cannabis, amphetamines, alcohol, and tobacco. The hard drugs are the ones to worry most about these are harmful to the body, and they include heroin, cocaine (opium), ecstasy, crack and several others. With the use of the hard drugs, it can lead to criminal activities, murder, sicknesses, and diseases. Research revealed that in 2011, 2.5 million emergency visits were resulting from drug abuse in the United States alone (Sipp, 2017). Besides, only 11 percent of those who are addicted received medical care. The remaining population of 89 percent is too vast to be without rehabilitation as they may result in more severe cases in future. According to Sipp (2017), the number of patients who are affected either directly or indirectly is growing. Therefore it’s essential to increase the rehabilitation centers at a higher rate so that the effects of drug abuse can be reduced through prevention than treatment. Addiction Poor relationships with parents also contribute to substance abuse. Peers and friends who are drug users influence other who becomes vulnerable to indulge in the behavior. Others enter into drug abuse to compensate for their in achievements such as failure to excel in education and other fields (Buccelli, Della, Paternoster, Niola, & Pieri, 2016). Continuous use of drugs leads to addiction (Longo, Wilson, Christopher & Grant, 2016). However the frequency of abuse that causes addition if different in different people. Some people react fast with the drugs hence developing a constant craving for them even after short periods of use. On the other hand, some individuals may use the drugs for a relatively long time before becoming an addict. Systems at homeostasis are stable; when homeostasis is disrupted, the system operates to restore it. initial sensitivity: The effect of a drug on someone using it for the first time. paradoxical: Contrary to what is expected. A paradoxical drug effect is opposite in direction to what is expected based on the drug’s chemical structure. placebo control: A type of control originating in drug research. Wizards are very useful for creating a good user experience. In all businesses, interactive sessions are most beneficial. To improve the user experience, wizards in Odoo provide an interactive session. For creating wizards, we can use transient models or abstract models. This gives features of a model class except the data storing. Transient and abstract models have permanent database persistence. For them, database tables are made, and the records in such tables are kept until they are specifically erased.
