The caroticocavernous fistula is a specific type of dural arteriovenousfistula characterized by abnormal arteriovenous shunting within the cavernous sinus. A caroticocavernous fistula results in high-pressure arterial blood entering the low-pressure venous cavernous sinus. This interferes with normal venous drainage patterns and compromises blood flow within the cavernous sinus and the orbit.

BMS were developed to mitigate elastic recoil and negative remodeling, but they remain prone to NIH. DES were developed to prevent NIH, and these devices (especially first-generation DES) can be accompanied by delayed reendothelialization, which has been associated with stent thrombosis. Even in the contemporary era of percutaneous coronary intervention using drug-eluting stents, ISR remains a common problem, occurring in 5% to 20% of cases, depending on several patient and lesion characteristics. The cumulative rates of DES failure have created a major clinical problem so that > 10% of all PCIs done in the United States are to treat ISR, and the number of ISR interventions appears to be increasing year over year

A coronary bifurcation consists of a flow divider (carina) and three vessel segments: The proximal main vessel (PMV) The distal main vessel (DMV) and The side branch (SB). A bifurcation lesion is a major epicardial coronary artery stenosis next to and/or including the ostium of a significant side branch A significant SB is a branch whose severe narrowing or acute occlusion before or during intervention can cause considerable ischemia or a new infarction area that will worsen the clinical course of a particular patient. Significant unprotected left main (LM) coronary artery disease is present in <10% of patients undergoing coronary angiography. In autopsy research, a mean LM length of 10.8 mm ± 5.2 mm (range 2–23 mm), mean LM diameter 4.9 mm ± 0.8 mm and mean angle between the left anterior descending (LAD) and left circumflex (LCx) of 86.7° ± 28.8° has been described. This angle value positively correlated with LM length.2 Further studies showed that long LM developed stenoses more frequently near the distal bifurcation compared to near the ostium (77% versus 18%).7 It is also worth emphasising that LM bifurcation disease is rarely focal and that both sides of the carina are almost never disease-free. Furthermore, continuous plaque from the LM into the proximal LAD artery has been reported in 90% of cases.8 Summarised below are the most crucial LM peculiarities (in comparison with non-LM bifurcations), which should be taken into consideration when distal LM stenosis PCI is planned:

Isolated monomorphic premature ventricular complexes (PVCs) without structural heart disease are generally benign. ► Frequent PVCs can cause reversible cardiomyopathy or aggravate an existing cardiomyopathy. ► Short coupled PVCs can trigger sustained ventricular fibrillation. These are often from the Purkinje tissue or rarely the outflow tract. ► Beta blockers are considered first-line therapy but have low efficacy. Catheter ablation and AADs are reasonable to suppress PVCs in appropriate patients. ► Ablation is often curative and success depends on location and accessibility of PVCs. ► Implantable defibrillators are reasonable in patients at higher risk of sudden cardiac death.

What is the best 2-stent technique?Most of bifurcation lesion can be treated with the provisional approach, but still we have some cases we have to consider 2-stent technique. There have several trials to find the best elective 2-stent techniques, but the results are quite variable. Bifurcations Bad Krozingen (BBK) II trial found that culotte technique is better than T-stenting in terms of restenosis rate.41) But culotte technique showed a similar result compared with crush technique in NORDIC Stent Technique study42) and was even inferior to double kissing (DK)-crush technique in DK-CRUSH III trial.43) I think the best 2-stent technique is the technique you are most familiar with. Maybe the optimal result especially in term of stent expansion is much more important than the selection of a specific 2-stent technique. Currently most popular techniques are T-stent and small protrusion, mini-crush technique, mini-culotte technique,and DK-crush technique. I prefer T-stenting and small protrusion technique, because it is simple, provisional in nature, and above all the most familiar to me
