2. The Problem
● The average patient has difficulty keeping blood sugar
'between the lines' and avoiding extended duration of high
blood sugar and severe low blood sugar
● Current technology for addressing these swings in blood
sugar require a lot of work and even then end up causing
problems of their own:
– Pump site failures
– Poor insulin absorption
– 12 to 20 minute blood sugar lags
– Missing / Inaccurate blood sugar data
4. Inherent Problems
● All AP Design Assumes A High Level of Effort
– Replace External Sensor > 50 x/yr
– Replace External Pump Site > 150 x/yr
– Up to 10% Failure Rate for Sensors & Pumps
5. The Open-Loop AP Is Already Here
●Insulin Delivery – Check!
● CGM – Check!
● Software/Firmware to Adjust Dosing re: BGs – Check!
● Training System to Teach AP Algorithms – Check!
6. Current Status
● Machines are not smart – People are!
● Machines break; pump sites and sensors fail!
● Research grants for closed-loop have been discontinued.
● All currently supported real-world research programs:
– subcutaneous hormone delivery
– subcutaneous glucose sensing
– limited approaches to dosing and safety algorithms
7. What's Missing?
● Personalized Algorithms vs. One Size Fits All
● Level Playing Field for Innovation - “The AP Cartel”!
● Funding for “Star Trek” like Revolutionary Technology
● Patient Access to Open-Loop Education Programs
