9. 2009 Fall Official Practice FOOTBALL Equipment issue in the café on Sunday night August 9, at 6pm. Football camp starts on Monday night August 10, 5:30 – 7, on the field. All sports practices begin on, August 17th

11. Practice Schedule Practice takes place every day after school starting between 2:45 and 3pm, depending on sport and level. The length of practice is up to the individual coach. Practices usually run until 4:30 or 5pm. Varsity practices could be later. Practices can be expected Monday trough Friday and some Saturdays. Sunday practices may be scheduled for Varsity only on a limited basis.
12. Athletic Information An Information mailing, including Student/Parent handbook with signature required, will take go out in April/May. Signatures are required for a number of forms and must be in the office before the first practice. Required forms include: Part A-Health History Update, Part B-Comprehensive Physical. Forms are posted on Athletic Webpage. One physical is required every two years. For Fall sports, physicals are preferred after July 1 and before August 17th. If a student has an annual exam then their last exam will do until they receive a new one. There is a handout in your packet with information about low cost exams at REHAB 3. If you have questions our trainor will be at a table in the rear of the gym at the end of this meeting. The completed form, or a copy of an exam within the past two years, with doctor signature, must be in the school office before the first practice on August 17.
13. Forms All must be returned before the first practice. Student/Parent handbook agreement: This document states that the student and parent have read the handbook and agree to abide by it’s contents. Physical Exam: Previously mentioned Transportation form: If a student is either riding with another student, or driving a student, to games or practices.
14. Academic Eligibility First term: freshmen are free of any academic eligibility requirements. Second and following terms: freshmen may not fail more than one class to be eligible. Second year and beyond: sophomores to seniors must not fail more than one class in the last termof the previous year to be eligible for fall and winter sports in the coming year.
15. Academic Eligibility To be eligible for practices and/or games on a daily basis, a student must; Be in school prior to 8:20 am on a regular day and by 9am on a late start day. If dismissed from school for a doctors appointment, you must have a note from the doctor. May not miss a practice or game to play for a non Saint Thomas team. (AAU, ODP, etc.)
16. Saint Thomas Travel Release Travel Release Forms must be used whenever an athlete is not riding the team bus. When parents transport athletes home they must sign a form for the coach before leaving the event. Athletes cannot drive to/from an event; parents must driveunless approval in advance from the Athletic Director. Forms must be submitted two days prior to event and signed by Athletic Director. Forms are posted on Athletic Webpage.
17. Athletic Training Program Saint Thomas Trainer: Mrs. Nikki Minnis Provided by REHAB 3 Center for Athletes Training Room is open everyday at 2:00 PM. Injuries must be reported to the coach/trainer when they occur. If an athlete sees a doctor for an injury, a note is needed for return to sport participation with directions including; Restrictions, Therapy, Pre-participation taping if necessary.
18. TREATMENTPATHWAYS INITIAL INJURY ATHLETIC TRAINER EVALUATION (determine injury severity/need for further medical evaluation) PARENT CONTACT CALL 911 TREAT IN HOUSE FAMILY PHYSICIAN (If athlete is seen outside of school, a note stating the injury, treatment protocol, limitations, and a follow-up or “return to play” date MUST be returned to the athletic trainer before the athlete is allowed to participate again.) A.T. REHAB/TREATMENT (Athletic Trainer works under standing orders from Doctor or Orthopedist re: treatment protocol. PHYSICAL THERAPY ATHLETIC TRAINER RETURN TO SPORT (Functional testing and medical clearance dictate return to participation.) (Taped or braced as needed. Possible restrictions still enforced.)
