A self & peer evaluation form I currently use to evaluate group work participation.

1. Group Work Self & Peer Evaluation
I expect everyone to make a good faith contribution to his or her group and participate in a mutually
agreeable way. Use this form to evaluate yourself and each member of your group. These evaluations will be
confidential, and will not be disclosed to your colleagues. Evaluate according to the following:
 = did not contribute; or contributions were unacceptable
 = contributions were acceptable
 = contributions were outstanding; or deserves special recognition
NOTE: it works best to highlight the face you wish to indicate for each person’s evaluation. Please add
comments to help me understand any group dynamic issues, positive or negative, or other concerns you wish to share.
Failure to turn in peer evaluations may cause your individual assignment grade to be lowered by 3 points.
