After graduating from the Armed Forces Medical in 1984 I was commissioned in the Indian Air
Force. I served at various air force stations as Medical Officer/ Senior Medical Officer .My
duties involved medical attendance to service personnel and their families, looking after health
and sanitation of air force station, immunization, conducting annual medical examination of
service personnel, and recruitment medicals for the civilians’ employees. I was in charge of
maternity and child health programme at the Air Force Station. I took active role in health
education programmes conducted for the benefit of the lady wives of the air force personnel. I
organized health promotion activities like immunization camps, well baby clinics, healthy baby
competition and cancer detection camps. The responsibilities also included training the Para
medical personnel on rendering life saving medical aid, use of resuscitation equipment,
evacuation of causalities by air craft or road ambulances. I was responsible for formulating
disaster management plans. During this period I was involved in the general administration also.
This involved management of personnel, procurement of medicine& equipment and planning for
future acquisitions.

I worked as Senior Consultant Anaesthesiology in the newly recently launched hospital. I took
over the almost barren operation theatre complex. I liaised with the various agencies for
completion of various works related to the Operation Theatre, CSSD and pipeline gases. I
organized the team of technicians and scrub nurses. We arranged the medical equipment,
procured drugs and disposables to make the operation theatres functional. I participated in the
installation and setting up of expensive electro medical equipment. I also started the operations
of the CSSD. I revised all the procedures and protocols for the operation theatre and the CSSD.
It was under my supervision that the first surgical procedures took place. The hospital was in the

I worked as Classified Specialist in this hospital which is also the second largest hospital of the
Indian Air Force . Surgical cases related to general surgery, orthopaedics, ENT, ophthalmology
and obstetrics & gynaecology were managed in the operation theatre daily. In addition
anaesthesia was provided for cases undergoing ECT. For two years I was the head of the
department. Duties and responsibilities include overall administration of operation theatre (OT)
and ICU. Besides providing perioperative care to the surgical patients I took part in planning for
postoperative analgesia. The hospital did not have a critical care specialist. I provided critical
care and ventilator support to patients presenting with multiple organ failure syndrome. My
administrative responsibilities included supervision of the operation theatre staff, and ICU staff. I
had younger colleague under my supervision. I was on emergency call on alternate days and on
many occasions I did call duties on a daily basis. I was involved in the planning for new
facilities and for upkeep and maintenance of the existing facilities in the OT and ICU. I planned
the replacement of central air conditioning system and installation of medical gases pipelines
and central suction facility for OT&ICU. I participated in the evaluation of new equipments,
drugs and devices for use in the OT and ICU. The hospital conducted training for operation room
assistants. Two levels of training programmes were carried out. ORA Class III is the beginner
level training programme for medical assistants and ORA Class II is an intermediate level
training programme. I was actively involved in these these training programmes. I chaired the
hospitals teaching programme which was based on medical audits. The results of these audits
were used to introduce changes in the hospital practices. The hospital started DNB in Family
Medicine. I was the coordinator for the DNB programme.

Research
1.Haemodynamic Responses in Air Oxygen Halothane Anaesthesia in Cardiothoracic
Surgery: A clinical study.
Dissertation submitted for MD Anaesthesiology. Haemodynamic responses to events like
intubation, incision, and sternotomy were studied.
2.Non-invasive ventilation in acute/chronic respiratory insufficiency. Patients use of
variable positive airway pressure.
This project was done to evaluate use of non invasive ventilation in patients presenting with
acute respiratory failure. Encouraging results were found in patients who had Chronic
Obstructive Pulmonary Disease and had presented with acute respiratory failure.
3. To evaluate efficacy of intermittent subglottic suction in prevention of
ventilator associated pneumonia
One hundred patients expected to be mechanically ventilated for >48 hours were randomized.
Group I (n=50) were intubated with HiLo EvacTM
tube with facility for subglottic suctioning and
Group II (n=50) were intubated with HiLo ContourTM
tube without such facility. In the study
group, subglottic suctioning was performed every 2 hours. Standard VAP prevention measures
were instituted in both groups. Incidence of VAP, mean ventilator days, ICU stay and mortality
were compared. Quantitative variables were reported as means ± standard deviation and were
compared by Student t test or Wilcoxon rank-sum test. Qualitative variables were reported as
percentages and were compared by chi-square test or Fisher’s exact test, as appropriate. P value
< 0.05 was considered as statistically significant.

Results:
The two groups were similar with respect to demographic characteristics but for
APACHE II Scores. VAP was seen in 6% patients receiving intermittent subglottic suctioning
and 22% patients of control group (p=0.021, significant). Both early-onset and late-onset VAP
were significantly reduced in test group. Mean ventilator days were significantly more in control
group with p=0.006 (significant) (8.32 vs 6.52 days). Though not statistically significant, there
was a trend towards reduction in ICU stay (11.32 days vs 14.04 days; p=0.070) and less
incidence of mortality in test group (36% vs 48%, p=0.224).
Intermittent subglottic suctioning was found to reduce the incidence of VAP including
late onset VAP in patients receiving mechanical ventilation.
