7. Defining the model...
h2cm is a conceptual framework
H2cm is an open conceptual space.
Open for the user to:
– Contextualise
– Situate
– Personalise & Populate
9. Individual
Personalised care is not a
given. It takes work in theory
and practice. This model
reflects that. It takes ‘effort’ to
place the individual at the
centre of what we do.

23. Hodges' model [h2cm] - Resources
Hodges' Health Career Model: Website
Hodges' original course notes
Care domains - indicative content (A4 page)
Four pages – one per knowledge (care) domain:
E.g. SCIENCES
&
POLITICAL
Three papers - two co-authored – are currently with journals (forensic
mental health; community informatics; and nursing). I am keen to hear
from people interested in using the model and explore collaboration,
writing projects.
Post-graduate study is also a possibility, personal studies are ongoing
exploring Hodges' model as a conceptual space(s) (Gärdenfors, 2000).
Notas do Editor
This is what the model is about. What do you see? The four corners – virgin space. What should (must) you see? What should you hear? The model's two main properties are there already... SITUATED PERSON-CENTRED The model is open to whatever situation or context is at hand. The model is open as we should be – making no judgements. h2cm acknowledges the need to be simultaneously task and person based. Although the individual is there putting them at the center takes effort. This work is necessary and reminds us of our values.
