2. Health Requirements
All nursing students are required to submit an annual physical and immunizations.
You may be wondering why this is pertinent to beginning nursing school. The College of
Nursing and Health Sciences, Institute of Medicine, and affiliate hospitals that you will be
attending to gain your clinical experience require you to adhere to Health and Regulatory
Requirements set forth by your program of study. You will not be able to attend your
clinical experience if these requirements have not been met.
The College of Nursing and Health Sciences provides forms that students must utilize for
their annual physical and immunizations. You must have a doctor’s office, clinic or health
department fill out the medical areas of the form. An “official stamp” AND an official
signature from one of these entities must be included for this document to be
completed and approved.
3. Immunization requirements
• Influenza vaccine
• MMR (Measles and Rubella) – proof of two immunizations OR positive titers (lab results)
• Tetanus-Diphtheria (Booster with Tdap OR Td with the last ten years required)
• Varicella (Chickenpox) – proof of two immunizations OR positive titer (lab results). Please
note: History of chicken pox is not adequate proof of immunity to Varicella; a titer or
immunization records is required
• Hepatitis B Vaccination – proof of three injections OR positive titer (lab results) OR waiver
Refusal of Hepatitis B Vaccination (If Applicable) - Although it is not a requirement, we
encourage all students to take the series of injections to immunize them against
Hepatitis B. If you choose not to take the injections, complete the Refusal of
Hepatitis B Vaccination Form which is available at the Bridge to Barry Blackboard site –
click on the Health Forms folder and download the form.
4. TB (Tuberculin) screening
All students must submit evidence of a current tuberculin test (PPD).
• Students with a negative PPD must have a second PPD in 3 weeks to confirm
non-reactive status. Two PPD tests are required only for your initial TB
screening, unless otherwise requested by the health care provider. After the
initial TB screening, a PPD test must be completed once yearly.
• Students with a positive PPD must show documentation of a chest x-ray as
well as documentation of any required follow up. After the first year,
students must have a licensed health care provider complete a TB Symptom
Check List annually. An additional chest x-ray will only be required if it is
recommended by a licensed health care provider, a student exhibits
symptoms of active TB disease, and/or if a student with previously negative
PPDs tests positive.
5. Health Insurance
ALL students are required to have adequate health insurance for the duration of the program.
You must have proof of health insurance coverage in the state of Florida by the start of your
first semester. If you have insurance please submit proof of health insurance along with the
insurance affidavit to the Compliance Officer. Please note the insurance affidavit can be
downloaded the Bridge to Barry Blackboard site – click on the Health Forms folder and
download the form.

Student Health Services will assist you with enrolling in the student health insurance plan
provided by United Healthcare. You may contact them at 305-899-3750 or visit
www.UHCSR.com for more information. The open enrollment deadline to enroll for student
health coverage is two weeks after the semester begins.
6. CPR Certification
The College of Nursing and Health Sciences only accepts the American Heart
Association Basic Life Support for Healthcare Providers that is offered by the
Barry University’s/College of Nursing and Health Sciences (CNHS) and School of
Professional and Career Education (PACE) for undergraduate students only.
The CNHS CPR certification contact person is Lauren Schwal. She can be reached

certification contact person and new AHA center coordinator is Mr. Jason Smith
who is based at our Pembroke Pines campus. You can contact him by email
(jsmith@barry.edu) or telephone (954- 232- 8451) to find out when he has
classes.

A copy of your CPR card will be given sometime during your first semester by
one of your professors.
7. Background checks and drug
screening
All nursing students are required to have a state-wide background check, 10
Panel drug screening, and FBI fingerprinting administered via Florida
Department of Law Enforcements - VECHS systems. The statewide background
check and drug screening must be completed through American Databank. In
order to complete this process, please visit http://barryex.com/ and be sure to
purchase package #1. The fee for this package is $ 140.50 for undergraduate
nursing students.
American Databank will send you an email asking your permission to allow Barry
University to review your results, please make sure to grant us permission. Also,
for the drug screening and fingerprinting American Databank will send you a list
of acceptable sites. If you go to a site other than the one listed you will be
charged an additional fee. Additionally, American Databank will send you an
email regarding your digital fingerprinting instructions processed through VECHS
systems. Please be sure to provide American Databank with a valid email
address.
8. Complio – Immunization Tracking System
Complio is an online tracking and screening system selected by Nursing to hold
background check details and documentation proving your compliance.
You have been given a handout providing you with detailed information and videos
that will assist you in creating an account and placing an order. The link below will

• Click Get Started to begin placing your order.
• Select your program of study and year and click Load Packages.
• Select package 1: Background Check and 10 Panel Drug Screen and
select the immunization tracking package.
9. Professional Portfolio
Professional Portfolio: Students are expected to maintain a portfolio of documents frequently
required by clinical agencies. Also, you will need to keep copies or originals for yourself in this
portfolio. Leave original documents at home in a safe place. Using a 3-ring binder, this will include:
• UG Student Policy Procedure Manual – http://student.barry.edu. , link Academic Resources,
link College of Nursing and Health Sciences, and link UG Student Policy & Procedure Manual;
• CPR card (offered by Barry University’s - CNHS or PACE)
• Annual Physical Form
• MMR (measles, mumps, and Rubella vaccination or positive titers),
• Tetanus/diphtheria series and booster(within 10 years),
• Hepatitis B vaccination, positive titers, and/or waiver,
• Varicella (chickenpox) immunization or positive titers,
• Annual PPD (if results are positive a chest x-ray is required),
• New Influenza vaccination
• Copy of background, drug and FBI reports – request copies from American Databank
by calling them at 1-800-200-0853,
• Proof of health insurance with coverage effective in the state of Florida

KEEP A COPY OF YOUR RECORDS – Should anything, be amiss, you can easily refer to
what was submitted. This portfolio must be complete and available should an
instructor or clinical agency request to see proof of any of the above information.
Photocopies of all documents must be uploaded to Complio before the semester
begins.
Completion of these requirements is a prerequisite for admission to the program and
continuation in each clinical nursing course. Some clinical agencies may have
additional requirements which the student will be required to meet.
Students will NOT be permitted to engage in clinical experiences and if their
compliance documents are not current.
