A new report, "State of the Nonprofit Sector
Survey 2018," by the Nonprofit Finance Fund
reveals that finances in the nonprofit sector are
stretched thinner this year than in 2017. More
than four out of five nonprofit leaders surveyed
reported the need for services rising, but almost
60 percent of these leaders do not anticipate
meeting need.

HUMAN RIGHTS
Rounded Up and Left for Dead
Algeria has expelled some 13,000 migrants in the past 14 months, leaving them stranded in the Sahara
desert without food or water. Scores have perished as a growing number of sub-Saharan migrants—
pregnant women and children among them—are rounded up "dropped in the desert and pointed in
the direction of Niger," sometimes at gunpoint.
The uptick comes amid growing EU pressure for North African nations to crack down on migrants
headed north to Europe. 135 people were dropped at Niger’s border in May 2017, compared to 2,888
in April 2018, according to International Organization for Migration estimates.

Judge Orders Halt to Border Separations
The Trump administration must halt family separations at the border under a temporary nationwide
injunction passed down late Tuesday, a move likely to spark a high-profile legal battle with the Justice
Department.
Ruling on a lawsuit brought by the ACLU, California federal judge Judge Dana M. Sabraw also
ordered that all separated families be reunited within 30 days, that children under 5 be returned to
their parents within 14 days, and that all children be allowed to speak with their parents within 10
days. The order enables family border separations only if their families present an immediate threat to
their safety.

Fishing Scandal—Thailand’s Audaciously Insufficient Tech Response
Thailand’s billion dollar fishing industry is rife with human rights violators who maintain horrific on-
board conditions, withhold wages, and engage in human trafficking of foreign workers who may not
know their rights. After threats of sanctions from the EU and the US State Department, Thailand is
investing in high-tech solutions like satellite surveillance and iris scans to curtail violations.
But technology alone can’t eradicate violators and the government’s reliance on it is troublesome to
worker’s rights groups. “It has become an excuse the government is using to justify they have done
something,” says Patima Tungpuchayakul, co-founder of Labour Rights Promotion Network
Foundation.

VIOLENCE
Where Women Walk in Danger
India is the most dangerous country for women in terms of sexual violence and slave labor, according to
a global poll by the Thomas Reuters Foundation of about 550 experts on women’s issues.
"India has shown utter disregard and disrespect for women ... rape, marital rapes, sexual assault and
harassment, female infanticide has gone unabated," said Manjunath Gangadhara, a Karnataka state official.
16
Afghanistan and Syria came in second and third, followed by Somalia and Saudi Arabia. Just one Western
nation appeared in the top 10: the US, which ranked joint third for a question on the risk of sexual
violence, coercion, and harassment.

INFECTIOUS DISEASES
Polio Returns to Papua New Guinea
A polio outbreak has been confirmed in Papua New Guinea—declared polio-free 18 years ago—after a
case surfaced in a 6-year-old boy.
The same virus was also isolated from stool specimens of 2 healthy children from the same community
“representing an outbreak,” according to the CDC.
PNG was certified polio-free in 2000 but polio vaccine coverage in Morobe province, the site of the
outbreak, has been low—just 61% of children have received the 3 recommended doses, and the WHO
noted inadequate sanitation and hygiene in the area as well.

CONFLICT
South Sudan’s “Carousel of Violence”
The people fleeing villages in South Sudan due to renewed conflict brought the possessions they could
carry and the horror they endured.
Thousands are believed to have been displaced from villages in Leer and Mayendit counties by troops
who destroyed food stores and medical clinics, seizing women and burning some people alive,
according to eyewitnesses.
“Largely lacking any ideological basis, South Sudan’s conflicts have become a carousel of violence
pitting rival elites from different tribes and political groupings against each other as they compete for
power,” writes reporter Peter Beaumont.

Pathogens Without Borders
In conflict zones the trinity of prevention, detection and response to epidemics has collapsed to
devastating effect.
20 countries are at high risk for pandemics, but frameworks encouraging countries to unite against
them aren’t rigorous, Paul Wise and Michele Barry argue in the latest issue of Daedalus.
Syria’s civil war has seen the first polio cases since 1999, due to a bungled response from the regional
17
WHO and politically motivated withholding of vaccines. Early stages of epidemics like SARS were
willfully underreported for fear of economic implications.
Writ large, politicizing epidemics will have a global impact, says Barry: “Pathogens know no borders.”

ANTIBIOTIC RESISTANCE
Grading Big Pharma
Pharmaceutical companies have a long way to go to combat antimicrobial resistance, but a new
ranking aims to hold them accountable.
Created by the Amsterdam-based Access to Medicine Foundation, the report unveiled yesterday at
the World Economic Forum in Davos credits GlaxoSmithKline with doing the most to combat the
crisis, followed by Johnson & Johnson.
Still, “There is quite a distance to go to keep up with what the companies have promised in Davos 2
years ago,” says Ramanan Laxminarayan of the Center for Disease Dynamics, Economics & Policy—
when they signed the "Davos Declaration," presented at 2016’s World Economic Forum.
Science
Fight the Resistance
An alarming 1.7 million health care-acquired antibiotic resistant infections (HAIs) spread in hospitals
each year. HAIs—which thrive in health care settings with heavy use of antibiotics, lots of staff in and
out of patient rooms, and high patient turnover—are tricky to manage and require multifaceted
solutions.
Daniel Evans calls for an all-hands-on-deck “antimicrobial stewardship” approach to combating HAIs,
including proper training for cleaning staff, limiting antibiotic prescriptions and support from diverse
stakeholders. Large-scale solutions for scourges like smallpox and polio, for example, required
commitment from governments, nonprofits, hospitals, private companies and other collaborators.
HAIs should be no different.

SEXUAL AND REPRODUCTIVE HEALTH RIGHTS
New Democracy, Ancient Traditions
Since elected president Adama Barrow took office, child marriage and female genital mutilation
(FGM), forcefully banned under Gambia’s autocratic former leader Yahya Jammeh, appear to be re-
emerging.
Despite misinformation circulating that the bans left along with the exiled autocrat, who imposed
heavy fines and jail sentences for the practices, the new administration says they remain in place.
Some human rights campaigners say the rituals were merely hidden under Jammeh. There are no hard
18
figures on whether these laws were obeyed during his 22-year rule. UNICEF said before the ban, 75%
of girls were cut and 30% wed before 18.

NONCOMMUNICABLE DISEASES
Obesity Begets Obesity
Obesity may be a “social contagion,” according to a new study of military families by USC and the RAND
Corp. which found relocating to heavier communities boosts chances of overweight and obesity.
Obesity clusters may result from like-minded people gravitating to areas with similar environmental and
dietary influences, but the researchers also observed a more nebulous element of social influence.
Incorporating data on food and exercise opportunities, researchers found families moving to Vernon
County, Louisiana, where 38% of adults are obese, made the condition 25% more likely among parents.
Inversely, obesity was less likely among families moving to less overweight areas.

CLIMATE CHANGE
Facing the Music
A robust response to climate change presents “the greatest global health opportunity” this century;
ignoring it could reverse decades of public health progress, according to the Lancet Countdown.
Building on the Lancet’s 2015 Commission on Health and Climate Change, the Countdown tracks
progress on the Paris Agreement and the health impacts of climate change.
It found climate change’s toll on human health is “potentially irreversible,” and concentrated in
vulnerable populations. Weather-related disasters increased 46% since 2000, the report shows.
But recent years have seen progress like the “inevitable” move to low-carbon energy and a tripling of
scientific papers on health and climate change since 2007.

HEALTH COMMUNICATION
When “Real News” Still Falls Flat
In information wars, the conflict isn’t always between “real” and “fake” news, but between facts and
values, writes David Scales in “French Lessons,” his aptly-titled piece about chronic Lyme disease for
Pulitzer Center for Crisis Reporting.
Scales looks at France’s ongoing information battle between patient advocacy groups and the medical
community, grounded in a lack of accepted clinical guidelines for the diagnosis and treatment of
chronic Lyme disease. An ensuing health ministry campaign to curb misinformation—and to denounce
“experts” touting purported alternative treatments—fell flat when advocates perceived a mere
“communications exercise” that failed to address a vital underlying issue: Patients are seeking relief,
19
not more—or better—information.

MEDICAL ETHICS
An Impossible Choice
A mere 1 in 200,000 births results in conjoined twins.
A case study of 22-month-old conjoined girls from East Africa, published in The New England Journal
of Medicine, reveals how in wealthy countries, the option of surgery still poses an agonizing dilemma.
At home, both would likely have died. Twin A’s congenital heart disease deemed her unlikely to
survive with or without the surgery; Twin B could live only if separated.
“How do you look at Twin A and say we’re going to save your sister?” asked MassGeneral Hospital for
Children surgeon Allan Goldstein, who ultimately operated, saving Twin B.

TECH AND INNOVATION
Not Your Everyday Lab Jacket
A Ugandan inventor is looking to end scores of missed pneumonia diagnoses with a very different
kind of lab jacket.
Shortlisted for the Royal Academy of Engineering Africa Prize 2017, inventor Brian Turyabagye says
his prototype biomedical jacket gives an accurate diagnosis 3–4X faster than a doctor.
Pneumonia kills half a million children under 5 each year in sub-Saharan Africa; 27,000 in Uganda
alone. Killing more children under 5 than HIV/AIDS, malaria and diarrhea combined, pneumonia
receives only 2 cents out of every UNICEF dollar.
Known as Mama-Ope (“Hope for the Mother”) the Bluetooth-enabled jacket reads body temperature,
lung condition and heart rate.

INFLUENZA
Scoring Strains
A relatively unknown flu strain, H7N9, outranks H1N1 and H5N1 (bird flu) on the CDC’s Most
Wanted list.
Scoring strains on a scale of 1 to 10, the list evaluates strains' pandemic potential.
Historically harmless in birds, H7N9 was 1st reported in humans in 2013. ¼ of 135 cases in China
were fatal. It has since mutated and become deadlier by the year in animals. This year’s “high-path”
20
strain has infected more people than 4 previous epidemics combined.
Research shows these more dangerous strains thrive among ferrets and mice—worrying scientists about
their potential human impact, for which data remains scarce.

VACCINES
Malicious Messaging
Muslims in the southeast Indian state of Kerala are being targeted for anti-vaccination propaganda via
social media, texts and WhatsApp messages.
The messaging falsely claims that vaccinations kill or sicken children—with specific fearmongering
toward Muslims. “What this misinformation on social media is telling them is: There’s a hatred of
Muslims in society, and these vaccines are a secret way to depopulate districts of Muslims,” says Shima
Azeez, MD.
During a diphtheria outbreak last year, Azeez and other physicians launched Info Clinic, a Facebook
page that provides factual vaccine information. Kerala launched a vaccine drive last month and is
cracking down on identified sources of misinformation.

FOOD
Where’s the Beef?
Argentina, where beef is typically what’s for dinner—and breakfast, and lunch—has introduced Vegan
Mondays.
While seen as a political move by opponents, the presidential palace serves only vegan options on
Mondays to all 500 employees, including President Mauricio Macri. The measure comes amid an
obesity crisis in Latin America where daily beef consumption is more than twice what’s recommended.
Beef production is also brutal for the environment with hefty greenhouse gas emissions, deforestation
and a near unquenchable thirst—15,000 liters of water to produce 1 kilogram of beef.
Globally, many countries are trimming meat from public menus, implementing meat taxes or limiting
exports in response to these concerns.

________Humanitarian Field Guide __________
“Change the World”
It always struck me that saying that sounded a lot like grandiose hubris, or at best, a dauntingly
overwhelming task. The utter impossibility of it seemed certain until I realized that it can mean
helping one person at a time. That is a theme you’ll see throughout this book and our websites and
our work. I have added some of my LinkedIn Influencer blogs/essays that I hope may be inspirational,
also. The format of this book is inspired by Brian Eno’s A Year with Swollen Appendices, not so much
the diary aspect but rather the overwhelmingly large collection of information in the various
appendices.
Open-Sourced Humanitarian Interventionism
It’s long been my goal to make life easier for those working in humanitarian and volunteer endeavors,
as well as those in need of help. Indeed, in one way or another, we all need help in one form or
another. So, just about everything you find herein and on the Center’s website, is free of charge, and a
lot you could also find for yourself. What I’ve tried to do is speed up the search, vet what has been
24
found, and then curate the results, making them as readily and easily available as I know how to. This
is my dream of open-sourcing humanitarian work.
Current Content, For Pretty Close To Forever
The reason for this “reverse engineering” is twofold. First, the amount of content and links on the
Center’s website may not always be apparent to the novel user. This book allows for near complete
exposure to the functional tools and content that await the online user. Second, this book will never
be out-of-date, in that when new content becomes available via uploads to the DropBox account, you’ll
be able to read that as well. All you have to do is email me and ask to be linked. I curate the content
constantly. You can also request being added to our mailing list via my email address as well if you’d
like to be kept up-to-date on events and other relevant content.
All proceeds from sales of this book will be donated to the Center for Global Initiatives.

  • 15. 15 HUMAN RIGHTS Rounded Up and Left for Dead Algeria has expelled some 13,000 migrants in the past 14 months, leaving them stranded in the Sahara desert without food or water. Scores have perished as a growing number of sub-Saharan migrants— pregnant women and children among them—are rounded up "dropped in the desert and pointed in the direction of Niger," sometimes at gunpoint. The uptick comes amid growing EU pressure for North African nations to crack down on migrants headed north to Europe. 135 people were dropped at Niger’s border in May 2017, compared to 2,888 in April 2018, according to International Organization for Migration estimates. AP Judge Orders Halt to Border Separations The Trump administration must halt family separations at the border under a temporary nationwide injunction passed down late Tuesday, a move likely to spark a high-profile legal battle with the Justice Department. Ruling on a lawsuit brought by the ACLU, California federal judge Judge Dana M. Sabraw also ordered that all separated families be reunited within 30 days, that children under 5 be returned to their parents within 14 days, and that all children be allowed to speak with their parents within 10 days. The order enables family border separations only if their families present an immediate threat to their safety. The New York Times Fishing Scandal—Thailand’s Audaciously Insufficient Tech Response Thailand’s billion dollar fishing industry is rife with human rights violators who maintain horrific on- board conditions, withhold wages, and engage in human trafficking of foreign workers who may not know their rights. After threats of sanctions from the EU and the US State Department, Thailand is investing in high-tech solutions like satellite surveillance and iris scans to curtail violations. But technology alone can’t eradicate violators and the government’s reliance on it is troublesome to worker’s rights groups. “It has become an excuse the government is using to justify they have done something,” says Patima Tungpuchayakul, co-founder of Labour Rights Promotion Network Foundation. Thomson Reuters Foundation VIOLENCE Where Women Walk in Danger India is the most dangerous country for women in terms of sexual violence and slave labor, according to a global poll by the Thomas Reuters Foundation of about 550 experts on women’s issues. "India has shown utter disregard and disrespect for women ... rape, marital rapes, sexual assault and harassment, female infanticide has gone unabated," said Manjunath Gangadhara, a Karnataka state official.

Just one Western nation appeared in the top 10: the US, which ranked joint third for a question on the risk of sexual violence, coercion, and harassment. Thomson Reuters Foundation INFECTIOUS DISEASES Polio Returns to Papua New Guinea A polio outbreak has been confirmed in Papua New Guinea—declared polio-free 18 years ago—after a case surfaced in a 6-year-old boy. The same virus was also isolated from stool specimens of 2 healthy children from the same community “representing an outbreak,” according to the CDC. PNG was certified polio-free in 2000 but polio vaccine coverage in Morobe province, the site of the outbreak, has been low—just 61% of children have received the 3 recommended doses, and the WHO noted inadequate sanitation and hygiene in the area as well. Channel NewsAsia CONFLICT South Sudan’s “Carousel of Violence” The people fleeing villages in South Sudan due to renewed conflict brought the possessions they could carry and the horror they endured. Thousands are believed to have been displaced from villages in Leer and Mayendit counties by troops who destroyed food stores and medical clinics, seizing women and burning some people alive, according to eyewitnesses. “Largely lacking any ideological basis, South Sudan’s conflicts have become a carousel of violence pitting rival elites from different tribes and political groupings against each other as they compete for power,” writes reporter Peter Beaumont. The Guardian Related: Girls of South Sudan on gunshot wounds, hunger and hope – in pictures – The Guardian Pathogens Without Borders In conflict zones the trinity of prevention, detection and response to epidemics has collapsed to devastating effect. 20 countries are at high risk for pandemics, but frameworks encouraging countries to unite against them aren’t rigorous, Paul Wise and Michele Barry argue in the latest issue of Daedalus. Writ large, politicizing epidemics will have a global impact, says Barry: “Pathogens know no borders.” Stanford University ANTIBIOTIC RESISTANCE Grading Big Pharma Pharmaceutical companies have a long way to go to combat antimicrobial resistance, but a new ranking aims to hold them accountable. Created by the Amsterdam-based Access to Medicine Foundation, the report unveiled yesterday at the World Economic Forum in Davos credits GlaxoSmithKline with doing the most to combat the crisis, followed by Johnson & Johnson. Still, “There is quite a distance to go to keep up with what the companies have promised in Davos 2 years ago,” says Ramanan Laxminarayan of the Center for Disease Dynamics, Economics & Policy— when they signed the "Davos Declaration," presented at 2016’s World Economic Forum. Science Fight the Resistance An alarming 1.7 million health care-acquired antibiotic resistant infections (HAIs) spread in hospitals each year. HAIs—which thrive in health care settings with heavy use of antibiotics, lots of staff in and out of patient rooms, and high patient turnover—are tricky to manage and require multifaceted solutions. Daniel Evans calls for an all-hands-on-deck “antimicrobial stewardship” approach to combating HAIs, including proper training for cleaning staff, limiting antibiotic prescriptions and support from diverse stakeholders. Large-scale solutions for scourges like smallpox and polio, for example, required commitment from governments, nonprofits, hospitals, private companies and other collaborators. HAIs should be no different. Thomson Reuters Foundation NONCOMMUNICABLE DISEASES Obesity Begets Obesity Obesity may be a “social contagion,” according to a new study of military families by USC and the RAND Corp. which found relocating to heavier communities boosts chances of overweight and obesity. Obesity clusters may result from like-minded people gravitating to areas with similar environmental and dietary influences, but the researchers also observed a more nebulous element of social influence. Incorporating data on food and exercise opportunities, researchers found families moving to Vernon County, Louisiana, where 38% of adults are obese, made the condition 25% more likely among parents. Inversely, obesity was less likely among families moving to less overweight areas. USC News CLIMATE CHANGE Facing the Music A robust response to climate change presents “the greatest global health opportunity” this century; ignoring it could reverse decades of public health progress, according to the Lancet Countdown. Building on the Lancet’s 2015 Commission on Health and Climate Change, the Countdown tracks progress on the Paris Agreement and the health impacts of climate change. It found climate change’s toll on human health is “potentially irreversible,” and concentrated in vulnerable populations. Weather-related disasters increased 46% since 2000, the report shows. But recent years have seen progress like the “inevitable” move to low-carbon energy and a tripling of scientific papers on health and climate change since 2007. The Lancet (free registration required) HEALTH COMMUNICATION When “Real News” Still Falls Flat In information wars, the conflict isn’t always between “real” and “fake” news, but between facts and values, writes David Scales in “French Lessons,” his aptly-titled piece about chronic Lyme disease for Pulitzer Center for Crisis Reporting. Scales looks at France’s ongoing information battle between patient advocacy groups and the medical community, grounded in a lack of accepted clinical guidelines for the diagnosis and treatment of chronic Lyme disease. An ensuing health ministry campaign to curb misinformation—and to denounce “experts” touting purported alternative treatments—fell flat when advocates perceived a mere “communications exercise” that failed to address a vital underlying issue: Patients are seeking relief,

A case study of 22-month-old conjoined girls from East Africa, published in The New England Journal of Medicine, reveals how in wealthy countries, the option of surgery still poses an agonizing dilemma. At home, both would likely have died. Twin A’s congenital heart disease deemed her unlikely to survive with or without the surgery; Twin B could live only if separated. “How do you look at Twin A and say we’re going to save your sister?” asked MassGeneral Hospital for Children surgeon Allan Goldstein, who ultimately operated, saving Twin B. NPR Goats and Soda TECH AND INNOVATION Not Your Everyday Lab Jacket A Ugandan inventor is looking to end scores of missed pneumonia diagnoses with a very different kind of lab jacket. Shortlisted for the Royal Academy of Engineering Africa Prize 2017, inventor Brian Turyabagye says his prototype biomedical jacket gives an accurate diagnosis 3–4X faster than a doctor. Pneumonia kills half a million children under 5 each year in sub-Saharan Africa; 27,000 in Uganda alone. Killing more children under 5 than HIV/AIDS, malaria and diarrhea combined, pneumonia receives only 2 cents out of every UNICEF dollar. The Atlantic VACCINES Malicious Messaging Muslims in the southeast Indian state of Kerala are being targeted for anti-vaccination propaganda via social media, texts and WhatsApp messages. The messaging falsely claims that vaccinations kill or sicken children—with specific fearmongering toward Muslims. “What this misinformation on social media is telling them is: There’s a hatred of Muslims in society, and these vaccines are a secret way to depopulate districts of Muslims,” says Shima Azeez, MD. During a diphtheria outbreak last year, Azeez and other physicians launched Info Clinic, a Facebook page that provides factual vaccine information. Kerala launched a vaccine drive last month and is cracking down on identified sources of misinformation. The National FOOD Where’s the Beef? Argentina, where beef is typically what’s for dinner—and breakfast, and lunch—has introduced Vegan Mondays. While seen as a political move by opponents, the presidential palace serves only vegan options on Mondays to all 500 employees, including President Mauricio Macri. The measure comes amid an obesity crisis in Latin America where daily beef consumption is more than twice what’s recommended. Beef production is also brutal for the environment with hefty greenhouse gas emissions, deforestation and a near unquenchable thirst—15,000 liters of water to produce 1 kilogram of beef. Globally, many countries are trimming meat from public menus, implementing meat taxes or limiting exports in response to these concerns. Economist
