It was adopted by the World Medical Association (WMA) in 1964 and has been revised multiple times since then, with the most recent revision occurring in 2013. The Declaration of Helsinki outlines several key principles and considerations for researchers and physicians involved in human research, with a focus on protecting the rights, safety, and well-being of research participants. Some of the core principles and points covered in the Declaration of Helsinki include: Informed Consent: Research participants must provide voluntary, informed, and written consent to participate in a study. They should be fully informed about the nature of the research, its purpose, risks, benefits, and alternatives. Beneficence and Non-Maleficence: Researchers should aim to maximize the benefits of research while minimizing harm to participants. The well-being of the research subjects should be the primary concern. Research Ethics Committee Review: All research involving human subjects should undergo ethical review by an independent committee. This review ensures that the study meets ethical and scientific standards. Scientific Validity and Ethical Conduct: Research should be scientifically rigorous and designed to answer important questions. Researchers must conduct their work with integrity and honesty. Privacy and Confidentiality: Participants' privacy should be protected, and their data should be kept confidential. Personal information should not be disclosed without informed consent. Equitable Distribution of Benefits and Burdens: Research should benefit society, and the selection of research subjects should be fair, without exploitation or discrimination. Continuing Review: Ethical review of research should continue throughout the duration of the study, with particular attention to any new information that may affect the research's ethical considerations. Access to Medical Care: Participants should have access to medical care and treatment, and they should be compensated for any injuries resulting from their participation in research.

Ethical concerns have also played a significant role in shaping the history of medical research, especially in response to various ethical violations, such as the Tuskegee Syphilis Study and the Nuremberg Trials. Resolving conflicts between these principles often requires careful consideration, ethical analysis, and, in some cases, consultation with ethics committees or boards. The specific course of action may vary based on the individual circumstances and ethical frameworks employed by healthcare professionals and researchers. Ethical guidelines and regulations also play a significant role in addressing and preventing these conflicts in medical research.

The utilization of clinical decision support (CDS) is increasing among healthcare facilities which have implemented computerized physician order entry or electronic medical records. Formal prospective evaluation of CDS implementations occurs rarely, and misuse or flaws in system design are often unrecognized. Retrospective review can identify failures but is too late to make critical corrections or initiate redesign efforts. Real-time surveillance of user responses and patient outcomes comprises one approach to give immediate feedback to CDS designers and help operate a safety net which intercepts CDS failures. We outline four types of CDS (passive alerts, interruptive alerts, order sets, and complex ordering advisors) and describe common failures with surveillance applications for high-alert medications such as aminoglycosides, anticoagulants, and insulin. We then present a computerized tool for high-alert medication prescriptions which serves developers, clinical pharmacists, and institutional physician leaders. The tool has two views: the surveillance view allows users to scan all CDS failures and prioritize high-risk scenarios, and the patient detail view provides context for understanding CDS failures. Entries on the surveillance tool populate automatically when CDS is used or ignored, allowing verification of prescription safety and accuracy.

Current Situation of Medical Errors Prepared by Asma Alshammari Alhanoof Alaniz Teflah Ali Mai Alrweeli Munyfaa Aldhafeeri Norah Almoteri Introduction Health care processes are increasingly being implicated in causing harm to patients. Medical errors and adverse events are primarily responsible for this harm. These errors, which may occur at every level of the custom are both common and diverse in nature. Medical errors can occur anywhere in the health care system in hospitals, clinics, surgery centers, doctors' offices, nursing homes, pharmacies, and patients' homes and can have serious consequences. Errors can involve medicines, surgery, diagnosis, equipment, or lab reports. Medical errors represent a serious public health problem and pose a threat to patient safety. As health care institutions establish “error” as a clinical and research priority, the answer to perhaps the most fundamental question remains elusive: What is a medical error? To reduce medical error, accurate measurements of its incidence, based on clear and consistent definitions, are essential prerequisites for effective action. Despite a growing body of literature and research on error in medicine, few studies have defined or measured “medical error” directly. Instead, researchers have adopted surrogate measures of error that largely depend on adverse patient outcomes or injury (i.e., are outcome-dependent). A lack of standardized nomenclature and the use of multiple and overlapping definitions of medical error have hindered data synthesis, analysis, collaborative work and evaluation of the impact of changes in health care delivery. Medical error is defined as “failure of a planned action to be completed as intended or use of a wrong plan to achieve an aim”. A medical error is a threat to patient safety and has a negative effect on health as well. Definition of Medical Error Medical error the term “error” has been variously defined. The Oxford Dictionary of Current English (1998) defines it as “mistake” or the condition of being morally “wrong”. Although the definition of “error” has its origins in behavioral psychology, the term is appropriate for medical usage. Experts agree that medication errors have the potential to cause harm within the pediatric population at a higher rate than in the adult population. For example, medication dosing errors are more common in pediatrics than adults because of weight-based dosing calculations, fractional dosing (e.g., mg vs. Gm), and the need for decimal points. “Research shows that the potential for adverse drug events within the pediatric inpatient population is about three times as high as among hospitalized adults,” (1) says Stu Levine, PharmD, informatics and pediatric specialist, Institute for Safe Medication Practices, an organization which serves as a resource for information on how to improve medication practices. “For this reason, health care providers must pay special attention to the specific challenges relating to the pediatric population.” A new study—the first to develop and evaluate a trigger tool to detect adverse drug events in an inpatient pediatric population —identified an 11.1 percent rate of adverse drug events in pediatric patients. This is far more than described in previous studies. The study also showed that 22 percent of those adverse drug events were preventable, 17.8 percent could have been identified earlier, and 16.8 percent could have been mitigated more effectively. (2) Children are more prone to medication errors and resulting harm because of the following: Most medications used in the care of children are formulated and packaged primarily for adults. Therefore, medications often must be prepared in different volumes or concentrations within the health care setting before being administered to children. The need to alter the original medication dosage requires a series of pediatric-specific calculations and tasks, each significantly increasing the possibility of error. Most health care settings are primarily built around the needs of adults. Many settings lack trained staff oriented to pediatric care, pediatric care protocols and safeguards, and/or up-to-date and easily accessible pediatric reference materials, especially with regard to medications. Emergency departments may be particularly risk-prone environments for children. (3) Children—especially young, small and sick children—are usually less able to physiologically tolerate a medication error due to still developing renal, immune and hepatic functions. Pharmacology: Professor Diana Rangaves, Santa Rosa Junior College PHARM 255 and PHARMM 256.1 Pharmacology is a branch of medicine that deals with the study of how drugs affect the body and brain. Studying drugs and their effects, side effects, and how they work, we are able to learn how to treat, manage and prevent disease and illness. In the study of these chemicals we look at the biological systems affected. This micro neural network of dendrites, neurons, receptors, chemical signaling all involved in the transmission of cellular communication. The field of pharmacology has substantially changed the way we view medications, disease and healing. Viewed as a Dance Party it has become possible to design chemicals that act on specific cellular signaling or metabolic pathways which control cell function and dysfunction. Knowing how drugs work is all the more important when it comes to serious conditions, life-threatening diseases and suffering. In these cases, knowing the specific actions of each drug before they are administered to patients leads to the highest possible outcome and patient medication safety. The primary neurotransmitters, Dopamine, Acetylcholine, GABA and Serotonin govern a broad range of clinical pharmacology, neuropharmacology, psychopharmacology, pharmacogenetics, pharmacogenomics, toxicology, posology, and behavioral and environmental pharmacology. These disciplines continue to grow and evolve. The field encompasses the best pathway to treat illness, disease and prevention; with an emphasis on pharmacokinetics and dynamics, mechanisms of action, clinical use, adverse reactions, drug interactions, contraindications, patient education and compliance. The courses are organized by modules. Each module has reading, videos, discussion posts and interactive case work to make it real. As facilitator, I curate the discussion, pulling out lines of inquiry and insights as they emerge. These interactions that occur between a living organism and chemicals are at the foundation of our human care and outcomes.
