3. Background
####
- A prolonged life of fissure to prevent
invulnerable newly erupted teeth
- The use of preparatory techniques
bur introduction 1) fissuratomy bur
2) pumice prophylaxis
to reduce microleakage
4. Microleakage
Any sign of microleakage….
-Considered the weak point
-Failure to isolate pit and fissures
-providing the retention of bacteria, nutrients,
and their acidic metabolic products
##eventually leading to Sealant failure!!##
6. Materials and Methods
90 freshly extracted healthy human maxillary
premolar teeth randomly divided into
3 groups (each for 30 teeth)
Group A a fissuratomy bur on a high speed
reduction on all the occlusal fissures
Group B 20 seconds thorough prophylaxis
pumice on a low speed
reduction on all the occlusal surfaces
Group C no preparation
7. Continued ….
1) Teeth etched for 20s with phosphoric acid gel and then
washed and dried for 20s
2) Teeth received an adhesive
3) LED light cured
4) Sealant applied to all to the prepared fissures through
a syringe tip
5) Thermocycled with 1,000cycles between 5~55C for 20s
6) Covered with two layers of nail varnish and then with
a layer of sticky wax
7) immersed in a 3% basic fuchsin dye solution for 72 h.
8) washed under the tap water for 1 min to remove
excess dye
9) embedded in acrylic blocks and sectioned in the bucco-
lingual plane through the sealant for two identical sections
using a water-cooled diamond disk on an Isomet saw
8. result
- Examined under a stereomicroscope
dye penetration score
# 0 - no dye penetration
# 1 - dye penetration to the outer half of the ena
mel-sealant interface
# 2 - dye penetration in the inner half of the ena
mel-sealant interface
# 3 - dye penetration into the underlying
fissure
**Kruskall-Wallis Analysis of Variance and Mann
- Whitney U-tests
10. Conclusion
- Findings of this study indicate that
the use of fissurotomy bur along with etc
hant gel enhances adhesiveness of seala
nt to the tooth
- The use of prophylaxis paste would be
advantageous in microleakage reduction
to the use of etching alone
