Most Outstanding
Cooperative
Leaders
Eligibility/Qualifying Criteria
• Must be a cooperative member in good
standing for the past ten (10) years
immediately preceding the nomination
– with the share capital not exceeding 10% of the
total subscribed capital
– Actively participating in the SMCBU program if
their cooperative
– Regularly attending general assembly meeting
– Patronizing the services of cooperatives
• Presently or had been an officer of cooperatives for
at least five (5) years whether elective or appointive
Eligibility/Qualifying Criteria
• With no derogatory record / No administrative
or criminal record
• With exemplary contribution to cooperative
development
• Must be nominated/ endorsed by at least two
different type of organizations from the
following: cooperative. Government
organizations, local government units, private
sector, other institutions
DOCUMENTARY REQUIREMENTS
1. Nomination form
2. Personal data sheet
3. Profile of candidate
4. Certification from cooperatives as member in good
standing for the past ten years
5. Certification from cooperative of share capital,
savings, attendance to GA meeting and patronage of
cooperative’s business
6. Certification form other organization/s that he/she
has been or is currently an officer or member of such
organizations;
7. Proof of resource generated for the last three years
such as donations, grants, subsidies, Loans assistance,
etc.
DOCUMENTARY REQUIREMENTS
8. List of innovative programs/ projects being practiced by the
cooperative( e.g. marketing system, new product develop)
supported with BOD Minutes of meeting, annual report, Board of
resolutions, publications;
9. Certification from LGUs of his/her membership/representation in
the activities/ planning bodies and his/ her involvement
participation in the LGU/GOs activities
10. Board of Resolution approving affiliation of cooperative and proof
of membership or affiliation of cooperative during his/ her term.
11. Copy of materials developed and presented in for
a/conference/seminars (i.e. Publications, researches, etc.)
12. Proof of Awards, Certificates of Recognition / Citation received for
the past five years.
13. Number of technical assistance rendered to cooperatives for the
past five years;

14. Certification of cooperative that the leader has a major
involvement in the organization of successful cooperatives
15. Proof/evidence of cooperatives organized/served by the
cooperative leader with the increase any of the following
aspects:

  • 2. Eligibility/Qualifying Criteria • Must be a cooperative member in good standing for the past ten (10) years immediately preceding the nomination – with the share capital not exceeding 10% of the total subscribed capital – Actively participating in the SMCBU program if their cooperative – Regularly attending general assembly meeting – Patronizing the services of cooperatives • Presently or had been an officer of cooperatives for at least five (5) years whether elective or appointive
  • 3. Eligibility/Qualifying Criteria • With no derogatory record / No administrative or criminal record • With exemplary contribution to cooperative development • Must be nominated/ endorsed by at least two different type of organizations from the following: cooperative. Government organizations, local government units, private sector, other institutions
  • 4. DOCUMENTARY REQUIREMENTS 1. Nomination form 2. Personal data sheet 3. Profile of candidate 4. Certification from cooperatives as member in good standing for the past ten years 5. Certification from cooperative of share capital, savings, attendance to GA meeting and patronage of cooperative’s business 6. Certification form other organization/s that he/she has been or is currently an officer or member of such organizations; 7. Proof of resource generated for the last three years such as donations, grants, subsidies, Loans assistance, etc.
  • 5. DOCUMENTARY REQUIREMENTS 8. List of innovative programs/ projects being practiced by the cooperative( e.g. marketing system, new product develop) supported with BOD Minutes of meeting, annual report, Board of resolutions, publications; 9. Certification from LGUs of his/her membership/representation in the activities/ planning bodies and his/ her involvement participation in the LGU/GOs activities 10. Board of Resolution approving affiliation of cooperative and proof of membership or affiliation of cooperative during his/ her term. 11. Copy of materials developed and presented in for a/conference/seminars (i.e. Publications, researches, etc.) 12. Proof of Awards, Certificates of Recognition / Citation received for the past five years. 13. Number of technical assistance rendered to cooperatives for the past five years;
  • 6. DOCUMENTARY REQUIREMENTS 14. Certification of cooperative that the leader has a major involvement in the organization of successful cooperatives 15. Proof/evidence of cooperatives organized/served by the cooperative leader with the increase any of the following aspects: • Assets • Net worth • Income • Net Surplus • Membership

  • 8. I. PERFORMANCE AND CONTRIBUTION TO COOPERATIVE DEVELOPMENT (65) CRITERIA POINTS 1. Services rendered to cooperatives 35 a. introduced new or innovative programs/ projects/ systems that are still being practiced by the cooperative which resulted to: financial growth, expansion/ diversification of services, expansion of area of operation, job generation or membership expansion 5 or more - 8 pts. 3 to 4 - 5 pts. 1 to 2 - 2 pts. 8
