14. Emergency reversal of antithrombotic treatment:
Unfractioned Heparin
• Time until restoration of hemostasis after cessation of
therapeutic dose: 3-4 h
• Antidote: Protamine sulphate
- 1 mg per 100 anti-Xa units of heparin
given in the last 2-4 h
- Dose should be maintained 100 mg over 2 hours; 50%
of dose can be administered initially with subsequent
doses titrated according to bleeding response
- Slow administration (5 mg/min) is advised to reduce
the risk of protamine-induced hypotension and
bradycardia (+ vasodilation and dyspnea)
- Immediate reversal;
the neutralizing effect of protamine can be
monitored by measuring the aPTT

clearance della creatinina è raccomandato l’uso di una dose ridotta, pari
a 1.5 mg i profilassi, mentre nei pazienti con clearance < 20 ml/min il
farmaco è controindicato (non indicazioni precise per la terapia)
Emivita terminale = 15-20 h
Farmacocinetica lineare
non influenza significativamente il sistema del citocromo CYP450 (no
interazioni significative con altri farmaci).

• 4.9 Sovradosaggio
Non esiste antidoto per dabigatran.
Dosi superiori a quelle raccomandate espongono il pz ad
un aumentato rischio di sanguinamento.
Nell’eventualità di complicazioni emorragiche il trattamento
deve essere sospeso e la causa del sanguinamento
indagata.
Poiché dabigatran è escreto soprattutto per via renale,
deve essere mantenuta un’adeguata diuresi.
Considerare: emostasi chirurgica e trasfusione di plasma
fresco congelato.
Dabigatran è dializzabile anche se manca esperienza
clinica che dimostri l’utilità di questo approccio
