4. INTRODUCCIÓN
El cáncer de esófago es el cuarto tumor más frecuente del aparato digestivo por detr
ás del (CCR, G y H) se sitúa entre los diez cánceres más frecuentes en el mundo.
España se encuentra, respecto al resto de Europa, en un término medio de inciden
cia ( aproximadamente 8/100.000 hombres y 1/100.000 mujeres).
Se estima que la incidencia anual de cáncer de esófago en España es de 1500 hom
bres y 250 mujeres. Dentro de España su incidencia es más alta en las comunida
des autónomas del norte (País Vasco, Asturias, Navarra…).
La edad habitual de presentación es entre los 55 y los 70 años, siendo infrecuent
es los casos en personas por debajo de los 40 años.
Menos del 50% de los casos de cáncer del esófago son carcinoma escamocelulares
. Los adenocarcinomas que típicamente aparecen en el esófago de Barrett represen
tan al menos el 50% de las lesiones malignas.
La rica red linfática del esófago y la ausencia de serosa contribuyen a la frecuent
e afectación ganglionar en el momento del diagnostico.
La supervivencia global a los 5 años es del 5% al 15% .

6. ESTADIFICACIÓN
El TAC : Representa la principal prueba complementaria para descartar enfermedad
metastásica . S: 70% y E: 60%
El PET : Está indicado en todos los pacientes M0 por TAC , ya que permite detectar
un 20% de casos con enfermedad metastásica en los que el TAC y la eco endosc
opia dan falsos negativos.
La Gastroscopia : Es el método inicial de diagnóstico de la mayoría de tumores.
La Ecoendoscopía :Asociada a PAAF es la mejor prueba diagnóstica para la valoración
loco regional en la enfermedad ( T y N) . S: 90% y E: 80 %
La Broncoscopia : Asociada a PAFF se recomienda en tumores situados a la alt
ura de la carina o por encima de ésta ( hasta 25 cm ) , sobretodo si el TAC no ha
podido descartar definitivamente la invasión traqueo-bronquial .
La Ecografía cervical con PAAF es útil ante la sospecha de adenopatías metastásicas
La laparoscopia : Puede ser útil en el diagnostico de extensión en tumores de la
unión esófago -gástrica y tercio distal del esófago.

14. Methods : 96 patients were included. Health-related quality of life
ques-tionnaires ( EORTC QLQ C-30 and QLQ OES18 ).
CT : derived size assessment of the primary tumours was performed.
Results : Karnofsky Index, occurrence of metastases (M-stage), Union Intern
a-tional Contre le Cancer-stage, computerized tomography-derived tumour.
In the multivariate analysis, 3 of the health-related quality of life questionnaire scales (
physical functioning, fatigue and reflux) were found to add prognostic informat
ion to M-stage, the single strongest predictor (HR 1.9, P < 0.01).
Conclusion : In addition to M-stage, the outcome of health-related quality
of life , questionnaires can sharpen the prediction of survival in patients with adva
nced cancer of the oesophagus or gastro-oesophageal junction and thus aid in the
choice of palliative treatment strategy.

17. CIRUGÍA CON INTENCIÓN PALIATIVA
Los tumores de esófago cervical a menos de 5 cm del músculo cricofaringeo
no se benefician del tratamiento quirúrgico . El tratamiento de elección en est
os casos es la radio-quimioterapia .
Tumores T4 se consideran irresecables cuando afectan el árbol traqueo-bron
quial . corazón grandes vasos , al contrario que sucede con la afectación pleur
al , diafragmática o pericárdica.
Tanto la esofagectomía por toracotomía (+ anastomosis en la cúpula torácica
tipo Ivor Lewis o cervical , tipo Mc Keonwn) ,como la transhiatal (Orringer )
son técnicas aceptadas en el tratamiento quirúrgico del cáncer de esófago .
Técnica de Kirschner : Interposición de colon o resección esofágica, liberaci
ón de estómago y anastomosis faringo-gástrica y estómago intratorácico, se uti
liza con frecuencia. elevada morbi-mortalidad, tiene un pronóstico y una esper
anza de vida significativamente mejores (1,9 vs 6,9 meses).

19. La quimioterapia de combinación logra tasas de respuesta del 30-50% .La .
mayoría de respuestas parciales y de corta duración ( 3 - 6 meses) .
El esquema más utilizado para el tratamiento del carcinoma Epidermoide
de esófago es la combinación de CDDP y 5-FU en infusión continua . Sin
embargo con el aumento de la incidencia de Adenocarcinoma de esófago , ha
crecido su inclusión en los ensayos de cáncer gástrico .

22. METHODS : Patients with metastatic esophageal cancer , wich had been treated J
anuary 2004 and June 2009 , were retrospectively investigated .
The treatment consisted of two courses of chemotherapy ( 5-Flouracil and cisplatin
) and concurrent irradiation of 40 Gy in 20 fractions to the esophageal tumor.
RESULTS : Dyspahgia score improved in 75% of the patients .
Disease control rate of the primary leson 95% ,incluiding 12 patients 30 % . RC
The median progression-free survival was 139 days .
Toxicities were generally well tolerated . Major toxicities (Grade 3 or 4 ) involved
Hemoglobin (23%) , leukocytes ( 15 %), Neutrophils (20 %) , Anorexia (10%) , Nause
a (3%) , esophageal perforation (5% ) and febrile neutropenia (3 %) .
CONCLUSIONS : Pallitiave chemoradiotherapy using 5-Fluorouracil plus Cisplatin
combined with concurrent 40 Gy irradiation effectively improve the symtoms of ,
dysphagia in stage IVB esophageal cancer with acceptable toxicity and favorable
survival .

26. Method: A prospective database of patients undergoing fluo
roscopic guided oesophageal stent insertion for malignancy bet
ween June 2001 and June 2009 was analysed.
Results: 273 stents were deployed using nine different type
s of SEMS. The median dysphagia score significantly improved f
rom 3 to 1 post-stent insertion (P < 0.001), with a technical succ
ess rate of 98%.
Conclusion: Oesophageal stent insertion provides good palli
ation for malignant dysphagia, however recurrent dysphagia re
mains a problem.

38. METHODS : A multicenter randomized controlled clinical trial with 215 patients f

There was a survival advantage for NON STENT treated patients

CONCLUSIONS : The treatment choise for patients with inoperable esophageal ca
ncer should be between a SEMS or a NON-STENT TREATMEN after consideratio
ns has been given to both, patients and tumor caracteristics and clinician and pat
ient preferences .
39. CONCLUSIONES
El cáncer de esófago es un problema de salud grave de diagnostico ,
habitualmente tardío y pronostico globalmente sombrío.
Se recomienda que cada institución elabore su guía de tratamiento y haya un
comité multidisciplinar para discusión de los casos y toma de decisiones .
La disfagia es el sintóma más común . Deteriora significativamente la calidad
de vida del paciente .
En los pacientes con buen estado general la quimioterapia paliativa no ,
aumenta la supervivencia pero puede mejorar la calidad de vida.
La radioterapia se usa efectivamente , muchos estudios reportan un
60% a 80% en alivio de la disfagia.
Después de la estadificación inicial y de quimio-radioterapia alrededor de
un 60% de los pacientes se consideran candidatos a una intervención
quirúrgica radical. Solo en 2/3 se puede realizar.
40. CONCLUSIONES
La quimioradioterápia es preferible a radiación sola para pacientes en estadio ava
nzado con buen estado general.
La experiencia entre Braquiterapia vs stent muestra que una paliación a largo ,
plazo es significativamente superior en pacientes tratados con braquiterapia.
Las fistulas traqueo-esofagicas se puede tratar efectivamente con stent cubierto .
Sin embargo la presencia de fistula es de mal pronostico.
Existen multiples metodos no quirurgicos para paliar la disfagia como sintoma
pricipal pero a pesar de multiples estudios no se ha logrado demostrar su
superioridad frente a los metodos convencionales.
