ACCOMMODATION REQUEST FORM
ALL INFORMATION IN THIS MEMO IS CONFIDENTIAL
STUDENT NAME: Maria Silva ID: xxx-xxx-xxx SEMESTER: SPRING 2015
The Americans with Disabilities Act and Section 504 of the 1973 Rehabilitation Act require
educational institutions to provide reasonable accommodations to all students who need them.
Services for Students with Disabilities has reviewed documentation from this student who has been
determined to be eligible for the following accommodations:
1. Extra time on tests and quizzes (1.5x)
2. Testing in a quiet environment (ASD can provide proctor for tests or use the ASC
tutor/building)
3. Extra time on assignments if it doesn’t compromise the integrity of the course.

Instructions: the ASD will email the accommodation letter to professor and copy the student.
Student must contact professor to discuss accommodations. Instructors may review it, discuss
with the student privately either face-to-face or via email/canvas, write down deadlines or any
information student must know in the space above, sign (keep copy), and return the original to
ASD immediately after receiving it or at most by 10 days of date email was sent. If you have
questions regarding the accommodations listed above, please contact the ASD Office at 575-
439-3724 immediately.

Extra Time On Tests And Quizzes
• All students that request extra time through the ASD qualifies for
the services
• Extra time for testing is not optional
• Not all students will use the extra time
• Unless the accommodation letter is updated, continue to provide
the extra time, even if student is not using it

Flexible Attendance and Deadlines
• Student has a scheduled doctor’s appointment:
Instructor must reschedule test
• Student missed test due to illness (disability):
Instructor must make-up test
• No permission from ASD is required.
Instructor should coordinate with student and ASC
(if needed) to arrange make-up test.
College Responsibility to Provide Assistive
Technology to Students
• College is responsible to provide assistive technology to
student in classroom and campus buildings
(ASC, Library, Student Services, etc.)
• College is NOT responsible to provide assistive technology
for students to take home
• ASD is allowed to provide free e-books/pdf to students if
proof of book purchase is provided
A Notice of Understanding
Memo will be sent to the faculty
that did not return the
accommodation letter properly
signed to ASD.
When All Other Options Have Failed
If there is a claim from student that instructor has not
provided accommodation, ASD will contact OIE and
Gerald Nevarez Gerard Nevarez will initiate the taking of
an internal complaint pursuant to Policy Chapter 3.25.
Instructor will have the burden to prove that she/he has
provided the accommodation.
Temporary Accommodation
If student contacts instructor and requests accommodation
o If instructor has time to contact ASD for accommodation, please direct
student to ASD and explain that student SHOULD follow the ASD
process to request accommodation
o If student request accommodation with short notice:
Instructor must accommodate student AND refer him/her to ASD
Instructor can ask the reason student is requesting the accommodation
Instructor may postpone test if not sure if accommodation can be
approved
If the disability is not visually identifiable, instructor must explain to
student that accommodation will be placed for this specific instance,
but for future, she/he needs to provide proof of disability to ASD
Temporary Accommodation Cont.
Bottom Line: Accommodations cannot be
postponed or withheld due to a
bureaucratic process.
If student goes to ASD and does not have a visually verifiable
disability, and request accommodation, ASD must provide
student with a Temporary Accommodation Letter
ASD Coordinator will consider the request on an individual basis
ASD Coordinator will establish a time frame for student to provide
documentation of his/her disability, but this time frame is flexible
and if student request more time and provide reasonable motive,
it has to be granted
If student fails to complete the process to request
accommodation, the accommodations can be
revoked and instructor will be notified to stop
providing the accommodations

Services for Students with Disabilities has reviewed documentation from this student who has been determined to be eligible for the following accommodations: 1. Extra time on tests and quizzes (1.5x) 2. Testing in a quiet environment (ASD can provide proctor for tests or use the ASC tutor/building) 3. Extra Time On Tests And Quizzes • All students that request extra time through the ASD qualifies for the services • Extra time for testing is not optional • Not all students will use the extra time • Unless the accommodation letter is updated, continue to provide the extra time, even if student is not using it

  • 8. Extra Time On Assignments • Instructor can discuss this accommodation with the student and explain why extra time is not necessary in this class • It is necessary to state in the accommodation letter why accommodation was not granted

Recording Class • No need for notes • If student needs to leave classroom, he/she will not miss material • Student can focus on lecture instead of taking notes • Students can transfer recording to notes o Smart pens o IPad and IPhone – e.g. Free Voice Record Pro o Regular recorder • Pictures of the white board
  • 11. Flexible Attendance and Deadlines • Student has a scheduled doctor’s appointment: Instructor must reschedule test • Student missed test due to illness (disability): Instructor must make-up test • No permission from ASD is required. Instructor should coordinate with student and ASC (if needed) to arrange make-up test.
  • 12. College Responsibility to Provide Assistive Technology to Students • College is responsible to provide assistive technology to student in classroom and campus buildings (ASC, Library, Student Services, etc.) • College is NOT responsible to provide assistive technology for students to take home • ASD is allowed to provide free e-books/pdf to students if proof of book purchase is provided
  • 13. A Notice of Understanding Memo will be sent to the faculty that did not return the accommodation letter properly signed to ASD. When All Other Options Have Failed If there is a claim from student that instructor has not provided accommodation, ASD will contact OIE and Gerald Nevarez Gerard Nevarez will initiate the taking of an internal complaint pursuant to Policy Chapter 3.25. Instructor will have the burden to prove that she/he has provided the accommodation.

If student contacts instructor and requests accommodation o If instructor has time to contact ASD for accommodation, please direct student to ASD and explain that student SHOULD follow the ASD process to request accommodation o If student request accommodation with short notice: Instructor must accommodate student AND refer him/her to ASD Instructor can ask the reason student is requesting the accommodation Instructor may postpone test if not sure if accommodation can be approved If the disability is not visually identifiable, instructor must explain to student that accommodation will be placed for this specific instance, but for future, she/he needs to provide proof of disability to ASD Temporary Accommodation Cont. Bottom Line: Accommodations cannot be postponed or withheld due to a bureaucratic process.
  • 16. If student goes to ASD and does not have a visually verifiable disability, and request accommodation, ASD must provide student with a Temporary Accommodation Letter ASD Coordinator will consider the request on an individual basis ASD Coordinator will establish a time frame for student to provide documentation of his/her disability, but this time frame is flexible and if student request more time and provide reasonable motive, it has to be granted If student fails to complete the process to request accommodation, the accommodations can be revoked and instructor will be notified to stop providing the accommodations Temporary Accommodation Cont.
  • 17. Instructors and Department Heads CAN request ASD for information on student’s accommodations if it is needed in order to better understand and serve the student Confidentiality Update
  • 18. Confidentiality Update Nobody is allowed to comment on student’s disability o in classroom o with others faculty, students or staff o for any other reason other than providing accommodations
