2. Introduction
• Teamwork is one of the most fundamental factors in rehabilitation
medicine.
• Different professionals, assessment and evaluations are brought
together to obtain a holistic view of the patients problems.
• This allows realistic rehabilitation measurements to be taken and
realistic objectives to be set in order to obtain the best possible
outcome.
• “The term refers to activities that involve the efforts of individuals from
a number of disciplines. These efforts are disciplinary- oriented and
although they may impinge upon clients or activities dealt with by other
disciplines. They approach them primarily through each discipline
relating to its own activities”.

3. Multidisciplinary
• Professionals in the multidisciplinary model usually work
independently to accomplish discipline specific goals. Sharing
information and making decisions based on that information these
team members regarding care planning.

5. Role of social workers
• The initial screening and evaluation of patients and families.
• Helping patients and family members to deal with the many
aspects of the patients condition- social, financial, and emotional.
• Helping patient and families understand their illness and treatment
options.
• Acting as an advocate for patients and families including as an
advocate for the patients health care rights.
• Aid and expedite decision making on behalf of patients and their
families.

6. • Educating patients on the roles of other members on their recovery
team including physicians, nurses, physical therapists.
• Crisis intervention
• Providing a comprehensive psycho social assessment of patients.
• Educating patients and families about post hospital care.
• Helping patients adjust to their inpatient rehabilitation setting.
• Coordinating patients discharge and continuity of care following
discharge.

7. Rehabilitation counselling
• Rehabilitation counselling is focused on helping people who have
disabilities achieve their personal careers and independent living
goals through counselling process.
• Rehabilitation counsellors can be found in private practice, in
rehabilitation facilities, hospitals, universities, schools,
government agencies, insurance companies and other
organizations where people are being treated for congenital or
acquired disabilities.

8. Role of social worker in rehabilitation counselling
• Provide individual and group counselling to help clients adjust to
their disabilities.
• Evaluate clients abilities, interests, experiences, skills, health and
education.
• Develop treatment plan for clients in consultation with other
professionals such as doctors, therapists and psychologist.
• Arrange for clients to obtain services such as medical care or
career training.
• Help employers understand the needs and abilities of people with
disabilities as well as laws and resources that affect people with
disabilities.

9. • Help clients develop their strengths and adjust to their
limitations.
• Locate resources, such as wheel chairs or computer programs
that help clients live and work more independently
• Maintain client records and monitor clients progress,
adjusting the rehabilitation or treatment plan as necessary.
• Advocate for the rights of people with disabilities to live in a
community and work in the job of their choice.

10. Conclusion
• The social workers role as an advocate also includes maintain open
lines of communication between the patient family and other
members of the health care team.
• He or she also will learn each family’s dynamics while
understanding its strengths and encouraging the use of these
strengths.
