Required Fields Optional Fields • Strategic plan • CEO/ Board Chair statement • Board attendance rate • Bios for senior staff • Conflict of Interest Policy • Background statement • Needs statement • Risk management provisions • ALL program fields (These fields are visible whether or not you complete them)
