Summary of personal statement
My career objective is based on being successful and progressive in life and to ensure that I give
the best and make positive impact to the organization or group I happen to be part of.
I have two years working experience with Family Health International FHI360 as a volunteer. I
have worked for two years on M&E unite and have had experience working on hygiene, sanitation
promotion and community mobilization. I started my career in NGO-FHI360 few years ago as a
data entry clerk in Specialist hospital one of the state comprehensive facility where FHI give
services. I am also a trained and practicing workshop facilitator and have organized and delivered
workshops to various groups and organizations on RH, HIV/AIDS, WASH, Peace building,
Community building, Leadership and Wisdom of Traditions
I was born on 2nd Feb. 1987, brought up in Adamawa state, I have had several travel experiences
to different states within the country, my language fluency in English and Hausa is at great level
of proficiency, while I understand Fulfulde i.e. Fulani language and Egbira at average.
Professionalrelevantworking experiences
2015-present Volunteer, FHI360, State Secretariat Complex 1st Floor SMOH Yola,
Adamawa State.
Working as a team in M&E unite, we have just recently completed Cohort
Analysis and TB data mop up/update in all the Primary Health Care centers
(PHCs) for the year 2014 and 2015 and in all the comprehensive sites within
the state capital and the local government areas. For the Cohort work
analysis, we get to fine out the total number of those who started on ART
in a facility. Here, M&E team seek to find out and document the total
number of HIV positive patients initiated on ART during the month in the
same facility. Information about patients who are transferred inn,
transferred out, lost to follow up, and stopped treatment, death, cohort alive
and on treatment 12 months after, and percent of cohort alive and on
treatment are all sorted out in the cohort analysis.
I have on several occasions worked with the FHI360’s technical officer,
health system and strengthening in organizing and carrying out trainings on
hygiene and sanitation promotion and community mobilization. I have had
sessions of presentation in some days of the trainings, and have lead a team

on practical demonstration of community mobilization towards hygiene
promotion.
I also work to Support the State M&E team in providing backstop to all
LAMIS-lafiya management and information system, and NOMIS- national
OVC management information system sites across the state. (OVC means
orphans and vulnerable children). Collation of Monthly Data in all the
support facilities thematic areas within the State Capital and all the Local
Governments (M&E, Pharmacy, RH/FP, TB Dots, Laboratory, HCT, and
PMTCT). Conduct data quality assessment (DQA), in all the support
facilities within the state capital and L.G.A.
I have Attended M&E Work plan meetings and also contributed in the
development of facility work plan, (budgeting and costing of facility
activities). Under SIDHAS Project for FY15 and FY16.
2014-2015 M&E DEC, Specialist Hospital Facility Jimeta-Yola, Adamawa State.
Carried out Clients counselling and enrollment on ART Commencement
(ART means anti-retroviral therapy). Generate Monthly Summary Forms
(MSFs). At the end of every working month, I generate MSFs from ART
Register, HCT, Pharmacy and PMTCT (Prevention from Mother to Child
Transmission) Registers. Often I get contracted to conduct Folder audit
(Radet and LAMIS) of other facilities within the state. Conducts LAMIS
DQA by verifying, validating LAMIS Data Vs Source Documents for (Pre-
ART, ART, Transfer- In, Transfer- Out, Lost- to- Follow-Up and Death)
Registers.
2014-present Community services within the state metropolis as a member of the
SELL Team facilitator.
SELL stands for sharing education and learning for life and in our activities
I made Active participation in community/group mobilization and advocacy
for organizing workshop.
Active participation at workshops organized and delivered by Yola SELL
Team to various communities, organizations, and to the Internally
Displaced Persons I.D.Ps in Adamawa State on Peace Building and
Community Building.
I have organized and delivered workshops to different youth groups and
secondary school students on Leadership, Self-Awareness, Sexual Health,
HIV/AIDs, Wisdom of Traditions, Justice, Peace Building and Community
Building.

Delivered sessions on WASH, Infant Breastfeeding practices, and aspects
of nutrition improvement within a household in Bagalci Community, Girei
L.G Area of Adamawa State.
2014-2015 State Coordinator and Independent contractor/Atmospheric reporter
for PHASE ZERO SOLUTIONS INC. 8 Months Project work
Achievements and responsibilities carried out.
The project work was about collation of reports and as the state coordinator,
I Coordinate and ensure timely reports of coworkers from other Local
Government within the state. I also Walk along the streets of the town/local
governments within the state to listen to people’s conversation, get reports
on situations, people responds/complains or views concerning political and
religious issues in the present time. Compile about thirty (30) or more
reports within a week, type the report in a PDF file format and send the
report via mail to Phase Zero Solutions Inc.
