16. Tumor Micro Environment
Il microambiente tumorale è particolarmente complesso,
comprende popolazioni cellulari diverse, tra cui le cellule
tumorali, le cellule endoteliali della circolazione
sanguigna e linfatica , fibroblasti stromali e una varietà
di cellule derivate dal midollo osseo ( BMDCs ) tra cui i
macrofagi , cellule immunosoppressori mieloidi,
monociti e cellule staminali mesenchimali.
Queste cellule svolgono ruolo nella crescita del tumore,
nell'angiogenesi nell'invasione intravasale, nell'
immunosoppressione e nella crescita metastatica a distanza,
attraverso la produzione di enzimi, fattori cellulari
protumorali e chemochine che determinano un’azione
immunosoppressiva ed un’azione degradante sulla matrice
extracellulare.
17. Le cellule tumorali esprimono una varietà di fattori immunosoppressivi, come IL-
10, IL-6 e TGF-β, trasformando le cellule immunitarie in fenotipi
immunosoppressivi che interagiscono tra loro e sempre più proliferano
promuovendo un circolo vizioso favorevole allo sviluppo del tumore.
Inoltre le cellule tumorali consumano fattori proinfiammatori ( IL-2 , TNF-α),
promuovono l’apoptosi di cellule T, e riducono l' attività antitumorale di
cellule NK. Producono metalloproteinasi della matrice (MMP ) e fattori di
crescita endoteliali vascolari ( VEGF ), così , promuovendo l'angiogenesi e
l'invasione tumorale .

Le Cellule tumorali circolanti
(CTC) diffuse dal tumore
primario nel flusso sanguigno
rappresentano un biomarker
importante per la diagnosi, il
trattamento e la prognosi dei
tumori epiteliali.
Recenti evidenze riferiscono che l'aggressività del tumore è associata
con la presenza di CTC nel sangue; di conseguenza, diversi studi
hanno focalizzato la loro attenzione sull’ isolamento CTC, la
caratterizzazione, e significato clinico.
Finora, il sistema CellSearch (®) è l'unico approccio approvato dalla
FDA per rilevare CTC di origine epiteliale nel sangue intero e valutare
la prognosi del tumore metastatico della mammella, della prostata e
del colon-retto.

Combination drugs that affect multiple targets simultaneously are
better at controlling complex disease systems and are less prone
to drug resistance.
This multicomponent therapy forms the basis of phytotherapy or
phytomedicine where the holistic therapeutic effect arises as a result
of complex positive (synergistic) or negative (antagonistic)
interactions between different components of a cocktail.
In this approach, multicomponent therapy is considered to be
advantageous for multifactorial diseases.
The different interactions between various components might involve
the protection of an active substance from decomposition by
enzymes, modification of transport across membranes of cells
or organelles, evasion of multidrug resistance mechanisms
among others.
